Employee (EE) was using a box wrench and impact to adjust a slide gate on a rock hopper. EE was standing on toes and right foot shifted slightly causing EE to squeeze down on impact gun which rotated the box wrench pinching the employees finger between the wrench and the steel chute. EE lost soft tissue on the tip of the left middle finger.

Employee removing empty water jugs from flatbed service truck. EE lost their footing fell to ground from bumper of truck 18" from ground landing on left side, left shoulder. EE lost their breath for a moment. Employee seen by medical and confirmed fracture to left side rib. Employee was returned to work.

Loading customer trucks with front end loader. Employee noticed customer dump truck was leaking sand between truck dump body and tail gate. Employee exited loader and attempted to clean contact surface of tailgate and truck body to allow gate to close. Air powered locking mechanism was activated catching employees thumbs between gate and truck body. Injury required stitches
A plant operator was performing clean up duties around the sand heater. The operator noticed grease around the trunnion roller and climbed up on a cement block at the base of the equipment to get a better look at the machine in operation. In the process of climbing down from the cement block the employee's glove was caught by the trunnion roller and the left hand was pulled in.
2.10.2020 an employee was attempting to remove a clam gate on the metering bin for our wash plant. EE had two c-clamp vice grips holding the gate in place. EE removed all but one bolt, removed one of the vice grips, removed the remaining bolt. The remaining vice grip released under tension striking EE's left pinky finger, causing stitches to be needed.

When removing the cutting edges from a bucket, the employee was holding the bolts up with the hammer when the bolt slipped off and the sharp edge of the bolt head cut the tech's palm side middle finger.

Fire on a pit belt after operations had already ended for the day.
At 7am 2 torches were lit to help heat belts. One torch was turned off at 9:00am, but the other was left burning. At 5pm the employee in the area shut down the plant and left. The torch was left on, and the belt started on fire. The fire was noticed by a mechanic on site at 6:15, and the fire department was notified. The fire was extinguished around 7pm.

Employee reported this injury on 6/20/07. He stated that he was grinding inside a hopper for the power screen. He was climbing out when he slipped and caught himself with his hand. He heard a loud snap in his right shoulder. He said it didn't hurt at the time but became increasingly painful and he couldn't raise his arm above his shoulder.
