An employee fell while dismounting their loader. The employee landed on their back in a sand pile. The employee's initial pain was a 3/10. After a week of self-care treatment, the employee was not improving, so EE was seen at a clinic on 7/21. X-rays indicate that the employee has a compression fracture on their L2 vertebrae.

EE stated they fell injuring a shoulder but when asked could not state how they fell, over a chalk ball or their own feet. There where employees within 100 feet of them at that time that did not see them fall. Once taken to the clinic the clinic informed them the issues with their shoulder from birth and not work related.
EE was walking a man lift down a pit road when the tires hit a 3-4 inch deep washed out rut caused by rain while traveling 2-3 feet off the ground. The rut caused the basket to bounce causing EE to be lifted into the air, when EE came down EE's right foot landed on the throttle assembly causing EE's right knee to twist.

Employee was opening the main gate to the Butler Mine and injured right hand (two fingers). When the employee went to pick up the gate and move it aside and place the gate post in the holder, that gate slipped out of employee's hand and two fingers were pinched/injured.

Employee was swinging a sledge hammer to drive a pin into a set of rails when a piece of the hammer fragmented into three small slivers and lodged into left upper thigh.
Employee started work shift at 4:00 a.m. 7/5/17. EE had went to do ee walk around and plant area checks. EE was walking between the Greens Mix tank and the Concrete Screw. EE hit head on a cross brace. Reported the injury to another ee who works in the same area with ee. Then reported to Safety Director@8:00a.m. EE was taken to the emergency room for evaluation for head injury.

Employee was helping move an 8" HDPE Pipe and strained his back.
Employee was cutting metal on 04/14/2014. Slag went into left ear. Employee seen on 04/17/2014. Otitis externa, Otitis, serous (Inner Ear infection) diagnosis. Continued to have problems with the ear. Workmans Comp set up appointment 06/13/2014 with ENT. ENT showed ear drum had been penetrated and surgery scheduled. Awaiting date for the surgery. No work missed.

Employee was working the gates in the tunnels when the injury occured. He was hitting the gates to loosen the sand and make it come out on to the belt. Was swinging a 2lb hammer that caused the problem in his left shoulder.

Employee was removing a water gate at the Plant/Bins. When he pulled the board there was "a bee sting like feeling", in his right shoulder. He reported this to P.K. (supervisior). At the time he thought that he could "just work through it." He finished Friday and worked on Sat. Reported the injury to the safety director on 09/17/09 @ 10:10 a.m.
