IW was throwing rocks off of the concrete platform when EE's finger contacted the angle iron of the X brace of the structure causing crushing injury to the tip of EE's left index finger. EE did not seek medical treatment until 3/18/2021.

While working to clear a bridge at the primary crusher of the portable plant the employee was using a bar to try to poke material free and smashed finger between the bar and the crusher. Employee did not inform management at the time of injury. Employee later went after work to ER and received 4 stitches to finger. Injury was reported to management on 8/21/17.
Two employees were working on guarding at the motor station for the crusher. Below 12 feet to the left two more employees were working on the lube system automation when a flat bar that was resting across the handrails above fell and struck one of the employees in the right lower leg. Resulting in laceration requiring 11 stitches.

Employee smashed finger while working with a piece of metal causing a fracture to the tip of his ring finger.

While assisting others doing maintenance on Conveyor #3 employee removed his safety glasses and wiped his face and apparently got dust in his left eye. The following morning the doctor removed small piece of foreign matter from employees left eye. Slight abrasion to eye diagnosed. Antibiotics administered.
Changing coller, the cooler dropped and pinched his finger.

Employee strained back while using a pry bar at grizzly box at primary crusher.
While replacing guard on crusher employees left index finger got caught between the guard and the clamping post on crusher causing a laceration to the top and bottom of finger.

The employee was holding a hammer against a bolt head that held a cutting edge in place with his left hand while using an impact wrench to loosen the nut on top. The hammer slipped off the head of the bolt allowing it to spin cutting the employees left index finger.

The employee was removing old conveyor belt and got his hand caught between the belt and handrail causing a laceration to his index and middle finger.
Zalco truck driver was trying to free a rcok from the truck's tailgate and smashed his finger. He received stitches to the finger. Zalco is a customer truck at Cabarrus Quarry. The injured was an employee of Zalco Trucking, not Vulcan Materials Co.

EE sprained his back while using a tool to turn a Euclid R05 Rear Dump 1710 Cummin engine at the shop.
The injured was handing a rock to a co-worker while inside the crusher clearing it out. It slipped out of the co-worker's hand and fell about two feet - hitting the injured's left shoulder.
The employee was walking from the employee building to the shop on relatively flat ground covered with base material. The employee stumbled and fell. The employee's right ankle was fractured along with fractures to her left fibula. These injuries may be further explained by employee being diagnosed with osteoporosis.

EE fractured his left hand after slipping and falling at foot level and trying to catch himself with his left hand. EE went to the doctor on 11/15/05.
Compression fracture of lower spine - while dumping pond fines the haul truck's front wheels rose off the ground and came back down. The EE did not report the injury until 5/17/05.

EE WAS WORKING ON EXHAUST SYSTEM OF A PICKUP TRUCK WHILE LYING ON HIS BACK. DIRT FELL FROM UNDERTHE TRUCK LANDING ON HIS FOREHEAD. THIS ALLOWED DUST TO GET IN BETWEEN HIS SAFETY GLASSES AND HI S FACE AND GETTING IN HIS EYE.
A BIN PULLEY WAS BEING REMOVED FROM CONVEYOR FRAME, ONE END WAS HOOKED TO A CRANE WITH CHAIN. THE PULLEY BECAME LODGED AND THE EE REACHED OVER TO PUSH ON THE PULLEY AT THAT MOMENT THE PULLED MOVED PINCHING HIS HAND BETWEEN THE CHAIN AND THE CONVEYOR FRAME BREAKING A BONE IN THE LEFT HAND OF THE EE.

EMPLOYEE WAS HELPING TRANSPORT TRUCK DRIVER UNLOAD A CRUSHER PART FROM THE BACK OF A TRACTOR TRAILER. THEY ATTEMPTED TO SLIDE THE PART INTO A BACK HOW BUCKET. AS THE PART SLID INTO THE BUCK ET, THE EE'S RIGHT PINKY FINGER WAS CAUGHT BETWEEN THE PART AND THE BACKHOE BUCKET.

AS THE HP 500 SH CRUSHER WAS BEING REASSBLED THERE WAS A HYD. CLAMPING CYLINDER LEAKING. THE REMOVED THE CYLINDER THE DUST SHELL WAS TAKEN FROM NORMAL POSITION AND SET ON THE CRUSHER. INJURED EE WAS DISCONNECTING HYD LINE WHEN THREE EES ATTEMPED TO REMOVE DUST SHELL COMPLETELY FROM CRUSHER-IN DOING SO THE DUST SHELL SLID INTO THE ORIGNIAL POSITION HITTING EE IN RIGHT HAND.

INDIVIDUAL WALKED OVER 10 FT OF OVERSIZED MATERIALS THAT HAD BEEN BROKEN BY A DROP BALL. PILE WAS APP 1 FT HIGH 20 FT ACROSS. HE TRAVLES APP 10 FT BEFORE FALLING.
REMOVING DRILL STEEL FROM ANOTHER PIECE OF DRILL STEEL. ATTEMPTING TO TIGHTEN THE J WRENCH ON THE J WRENCH HELD J-WRENCH WITH LEFT HAND AND HIT WITH SLEDGE HAMMER IN RIGHT HAND. HIT HAND WITH SLEDGEHAMMER.

EMPLOYEE SQUATTED DOWN TO LOOK AT EQUIPMENT WHEN SHE FELT HER KNEE POP. SHE WAS STANDING ON FLAT CLEAN METAL SURFACE. LOST TIME BEGAN 12-27-98 WITH SURGERY ON KNEE.
EMPLOYEE WAS CUTTING TORCH RING ON HP 500 HEAD WITH DXY-ACT TORCH. SPARKS FROM THE TORCH CAUGHT HIS LONG SLEEVE SHIRT ON FIRE. BY THE TIME HE REMOVED HIS SHIRT, HE HAD BURNT HIS LEFT BICEP. TH E AREA OF THE BURN WAS 3" X 3", 2ND DEGREE. THE AREA WAS CLEANED AND DRESSED AT DOCTOR. EMPLOYEE RETURNED TO WORK.
INDIVIDUAL SMASHED RIGHT THUMB BETWEEN DRILL STEEL AND FRAME. INDIVIDUAL INCURRED A FRACTURE TO DISTAL TIP OF THUMB.
EE NORMALLY SETS UP SEISMOGRAPH FOR EACH BLAST. THIS REQUIRES PLACING A 50 LB. BAG OF SAND ON TOP OF UNIT. ON 6-1-98, EE HAD FINISHED OPERATING SEISMOGRAPH AND WAS PLACING UNIT AND SANDBAG BAC K INTO HER TRUCK WHEN SHE TURNED AND FELT A PAININ HER BACK. EE SUSTAINED STRAINED MUSCLE THAT WILL PUT HER ON LIGHT DUTY FOR 7 DAYS.

WAS UNDER THE PICKUP TRUCK INSTALLING A GASKET ON THE EXHAUST SYSTEM, WHEN HIS WRENCH STRUCK THE FRAME DISLODGING RUST AND DIRT. A PIECE OF RUST GOT INTO HIS RT EYE. HE WAS WEARING PRESCRIPTIO N SAFETY GLASSES WITH SIDE SHIELDS.
EE WAS CLEANING A BIN OVER THE CRUSHER TO PREPARE FOR A WELDING JOB. HE WAS CLIMBING ON THE SIDE OF THE BIN TO GET INTO THE FEEDER, WHEN THE CONDUIT HE WAS HOLDING ONTO BROKE LOOSE CAUSING HI M TO FALL. HE FELL TO THE CRUSHER AND THEN THE PLATFORM APPROX 10'.
WHILE SITTING DOWN, PULLING ON THE DUMP LEVER, EE FELT A POP IN HIS SHOULDER. HE WAS SEATED IN THE PROPER POSITION AT THE TIME OF THE ACCIDENT.
HE WAS CUTTING WITH A NON LOCKING KNIFE AFTER THE LOCKING HAWBILL GOT TOO DULL. THE KNIFE FOLDED UP CUTTING HIS RIGHT INDEX FINGER.
UPON OPENING UP THE SCALES, SHE TRIPPED STEPPING UP ONTO PLATFORM. SHE THEN FELL FORWARD HITTING HE WRIST ON THE COUNTERTOP. SHE WAS COMPLAINING ABOUT PAIN AND WANTED TO GO TO DR CONTUSION TO WRIST DR PUT IT IN AND SPLINT AND SHE RETURNED TO HER JOB.
EE WAS PREPARING TO MOUNT 50 TON HAUL TRUCK HIS FOOT SLIPPED DUE TO GREASE ON HIS BOOT CAUSING HIM TO FALL 2 FT HE TRIED TO CATCH HIM SELF WITH RIGHT ARM. THE SUDDEN JERK ON HIS ARM CAUSED A S TRAIN TO THE RIGHT SHOULDER MUSCLE.
