EE was operating an 850 Hitachi Excavator in our Marblehead Pit. The employee noticed the engine compartment door had swung open. While walking on the catwalk to close the door, one end of said catwalk broke loose causing the EE to fall to the ground. EE struck EE's head on rocks below which caused a broken orbital bone, broken nose, and loose tooth, with head lacerations.

Employee was clearing a plugged chute between 2VS1 Screen and 2BC3 belt conveyor using a bar. The plugged 3/4 inch crusher run broke free while employee was prodding the chute with the bar, causing the employee to mash EE's left thumb on the lip of the inspection door.
EE was holding a hose while another employee was cutting it. When the knife blade passed through the hose, the knife carried momentum and cut the back of EE's left hand.
Employee climbed out of a 988F loader for lunch break. When the employee's feet were on the ground, EE turned to walk towards the break room. As EE turned, EE's feet slipped out from under EE in the snow, and EE fell.

Maintenance crew was working to free up an outrigger pad on the UV 11 blasting truck that had seized to the shaft. The crew was using a strap hooked to the maintenance truck to pull on the seized pad. When the pad broke free, it spun approximately 180 degrees on the shaft and mashed the employee's fingers.

Employee was trying to help install a rotary airlock on the primary separator at kiln 1 baghouse when the airlock shifted on the forklift and pinched EE's right index finger between the flange of the airlock and the chute it was being attached to.
Employee was moving the basket of a powder rig from the face and swinging it towards the rib. EE let go of the controls, lost balance, grabbed the basket guardrail instead of the basket handrail and pinched right hand fingers between the rib and rail. This smashed EE's right hand middle finger and caused a laceration.
Employee climbed onto 850 Exc to speak with superintendent. Upon leaving 850, employee moved to change 3pt contact. Lifted wrong foot, slipped and fell approx 5'-6' to the ground. Incurred injury to right heel. No life threatening injury. No foreign material on platform.

Employee was performing work under a platform, caught EE's hard hat on the corner of the platform which caused EE's hard hat to fall off. At that time the employee struck EE's head leaving a gash on the top of EE's head in EE's hairline.
While attempting to place and weld a metal bracket for support a Porta Power slipped and caught hand.
The guard was removed from the ring gear and the cause of the fire was determined to be from oil leaking onto the hot kiln causing the oil to reach flash point and catch fire. The fire was discovered at approx. 5:50 AM during shift change and fire extinguishers were used to control and put out the fire.

Oil that lubricates the ring gear leaked onto the kiln which was determined to cause the ignition of the oil. The ring gear was fully guarded and the fire was under the guard. 911 was called immediately along with both Federal and State regulatory agencies. There was minimal damage found. There were no injuries.

Employee was unplugging spout #2 on the hydrate bin with a metal rod when the slide gate somehow closed and caused the rod to kick back into employee's hand. This caused a laceration that was handled with first aid before employee went to the doctor to get 3 stitches.

EE was cutting plastic shrink wrap off of a pallet of gear guard parts. EE cut the plastic with a pocket knife using right hand. EE reached down to pull the plastic from the pallet with left hand. There was a piece of the plastic that had not been cut. EE pulled the plastic with left hand, and when the plastic broke free left thumb contacted the knife blade.

While cleaning out a duct pipe, the plugged lime "puffed" up. Heat and lime surrounded the employee and lime went around the glasses into eyes.
Employee was dismounting truck and when foot hit the ground, EE felt knee pop.
Employee's glove was caught by clips on stacking conveyor and pulled into the pinch point between conveyor belt and stacking platform.

A hydrate bag fell off the end of the hydrate bag conveyor belt and busted when the bag hit a corner of a wood pallet. The bag busted causing hydrate lime to go into the employee's eyes. It went into the right eye more than the left eye. It caused eye irritation for which the employee was prescribed ointment and eye drops.
Employee received steam burns to both hands while opening sampling door on the hydrator.
Driving up the lime dump road the operator of the articulated truck noticed an orange glow coming from the right back corner of the truck cab. The driver then got out, discharged EE's fire extinguisher, and called on the radio for help. Two other employees brought up 5 fire extinguishers, emptied them, and called the fire department. The fire department extinguished the fire.
When the spout at the Hydrate Bagging Machine was plugged the employee went to open it while under pressure and lime released from the spout getting into the employee's eyes.
While cleaning frozen mud off of the gate of the diverter at the secondary screen the employee struck left hand against the door of the diverter.

While filling up hydrate lime bags, the machine malfunctioned and had to be reset. While going to reset the machine, the bag being filled burst causing hydrate lime to get into the employee's right eye causing irritation. The employee was wearing safety glasses at the time of the incident.

Employee was cleaning ash rings in the kiln when a piece of lime got into the employee's eye causing irritation. The employee was wearing safety glasses at the time of the incident.
Employee was walking back to his truck when he slipped on wet lime. The employee landed on his forearm and shoulder. No first aid or medical treatment was administered at the time of the accident. Later a torn muscle in the shoulder was diagnosed.
Driver became dizzy while strapping down a load of bagged rockdust. She then passed out as another employee was helping her.
After taking a drink of soda from the refrigerator the employee felt like the drink was caught in her throat and passed out hitting her head on the window sill. She received two staple stitches at the ER.

Employee was walking from portable plant to break room by quarry shop for lunch. He slipped in snow and fell backwards and hit his head on the ground. His hard hat fell off as he was falling.

Driver was walking from truck to loading facility when he stepped on rock twisting left ankle.
While attempting to open a plugged air line by taking a quick connect fitting off the pressure on the air line pushed dust into the operator's eye.
The operator dropped the white belt conveyor tray interrupting the proximity switch to remove a bag that was jammed. When he reached in to remove the bag the valvomat dropped down onto the operators wrist/hand pinning his hand between the valvomat and the white belt conveyor tray.
