Employee was out of the loader manually moving the face drill cable out of their way so that they could clean up. While they were working the cable out of the way they felt a strain in their abdomen that did not ease up throughout the day. They came out and reported the incident around their quitting time.
EE was scaling the right rib with a CAT Excavator in 42 Face when a rock from the face fell and hit the FOPS of the machine and broke through the left rear glass and ended up on the seat behind EE. EE claims that EE does not believe the rock hit EE. However EE did have lower back pain before exiting the machine.

EE was trying to spread forks on the IT CAT Loader Equipment number 1195 when one fork came off of the rail and fell on EE's right ankle. The forks were in a raised position approximately 4 ft. off of the ground in the wire hole (Underground).
EE was walking through the shop between a wooden pallet and the vending machine when EE's left foot caught a loop that was coming off of the pallet that was holding down an electrical cable. EE tripped and fell on EE's face and put a small cut on EE's nose and bumped EE's forehead. EE tried to catch self with EE's left arm but still hit EE's face on the concreate.
EE had a chute plug and was trying to pry stone free with a pry bar on the 18 inch chute. As EE has doing this, a piece above broke loose and came down on EE's right hand.
Employee was climbing off loader when EE stepped on a rock and twisted EE's left knee. This happened on 4/26/23 but employee did not stop work and returned to work next day. EE went to doctor on 5/4/23 without informing us.

The worker was walking around EE's job area when EE slipped in mud caused by rainfall that happened earlier in the day. When the worker slipped EE tried to catch self but ended up twisting EE's right knee which caused pain in the inner portion of the knee.
The worker was using an air-powered chipping hammer to take out mixing plates at the asphalt plant. While the worker was performing EE's task a piece of debris flew into EE's right nostril and made EE's nosebleed. The worker was taken to Waterfront Urgent Care to get EE's nose cauterized to stop the bleeding.

In 34 Heading, climbing up 992C Loader, employee fell from the top deck of the loader, employee's foot slipped causing them to grab the hand rail and swing, striking their right shoulder and the back of the right elbow causing two lacerations against the back side of the front left tire and then hitting the ground again with their right shoulder.
Employee was shoveling #1 Sand Plant when EE felt a pain in the lower right part of EE's back.
Was unloading an acetylene tank when it slipped out of the employee's hands. When EE tried to catch it to regrip the tank, EE felt something pull in left arm where the bicep and elbow meet.

Employee was cutting a fastened guard when the utility knife slipped and cut a finger.
Employee was scaling the face of 20 heading using a scaling bar when the employee felt pain in employee's right shoulder and neck.
While scaling a rib in in the bottom area of 42 heading, a slab off the rib came down while scaling and struck the boom of the Gradall causing it to swing. The miner's left shoulder struck the inside of the cab. The miner did not notice any noticeable originally, but noticed swelling and stiffness in the left shoulder and clavicle.
While cutting a Gradall shank with an oxygen acetylene torch, a piece of molten metal flew into the employees ear.
Employee was using a ratchet, socket and tap to clean bolt hole threads on a 992c loader. The socket slipped, causing the employee to strike knuckle of left hand on the loader bucket.

The employee walked around the rear of welding truck where EE slipped on loose gravel and fell to the ground. EE felt a "pop" and sharp pain in left knee. EE got back up under own power and was taken back to the employee clockhouse. First aid was administered at the time and the employee was taken to the hospital by a family member for further examination.

While going back to do his pre-vehicle inspection report on his welding truck the employee discovered the welding truck adjacent to his was completely burnt as it appeared to catch fire between the afternoon shift and morning shift.
While operating gradall #890 the employee turned and felt pain in his back.

While cutting a piece of belting with a utility knife towards himself the employee accidentally stabbed himself in the right upper leg above the knee. The wound was approximately 3/8" to 1/2" long.
While loading a shot at the face the employee felt pain in his back. Although he felt OK the rest of the shift the next day he sought medical care as he states he could barely move.
While shoveling from under a beltline at the #2 mill the employee felt tightness in his lower back.
While hand scaling in the face the scaling bar slipped on a rock and caused pain in the employee's right arm.

While walking at the face the employee tripped and twisted his right knee.
While checking out the tail roll on the #12 belt due to a conveyor/chute plug up a rock fell off of the catwalk above him striking him on the side of the head. This incident was a result of a plug up which caused limestone material to back up in the chute and therefore spill onto the catwalk above.
While picking rock off of the conveyor the employee stated that his back started hurting him.

While washing off the scales the slack was pulled out of the hose causing the worker to be tangled and fall from a standing position to walking/work surface.
Worker was working in a group of 3 individuals using a vaccum truck to remove debris from around a conveyor belt on an elevated platform. As he was pulling the 4 inches of hose, slipped backwards falling down 3 stairs striking his tailbone. The injured worker indicated he was not paying attention to his surroundings.
While getting out of the basket on the tower truck, employee's ring on his left ring finger caught on the basket, ripping the skin loose all around his finger.
While using a cutting torch to remove a bearing the employee was burned by the torch tip after another employee tripped over the torch hose causing the torch flame to contact his left hand.
While shoveling at the sand plant the employee felt a burning sensation in his left side area.

While lifting a bearing to install a plow bolt in a take up pulley the bar slipped causing the bearing to come down on the employee's left thumb and index finger.
While lifting screens into a truck the employee felt something pull in his lower abdomen.
During operation of the Mack tower truck the EE felt something pop in his lower back while pushing in on the truck's clutch pedal.

While attempting to climb a vertical ladder the employee's feet slipped out from under him. The employee tried to stay on his feet and the ladder by grabbing hold with his right hand. However, the employee's grip slipped and he fell to ground level injuring his right shoulder and elbow.

While hitting a support beam for catwalk repair with a sledge hammer, the employee struck the metal and his hand slipped off the metal and got caught between the hammer handle and steel.
While the employee was working on the left rear brake drum on the mechanic's truck, the brake drum slipped and smashed his right index finger, causing a laceration to occur.
While greasing the tail section of a conveyor belt at the #1 mill the employee slipped and fell from the level concrete slab that served as an ankor point for the conveyor.
While cleaning conveyor belly guards under beltline a guard full of material debris dropped down, hit the manlift basket and then jammed the basket up and into the conveyor frame in a "spring up" manner.

While steam cleaning oil cooler on hi-lift, the employee was walking on the catwalk and slipped. He reported that he caught himself and felt something pull in his shoulder.

EMPLOYEE THROUGHT THE BREAKER SWITCH ON FOR THE TYROCK SCREEN. THIS WAS DONE UNDER A LOAD, BECAUSE THE STARTER SWITCH WAS IN THE ON POSITION FOR THE SCREEN. CONSEQUENTLY, THE BREAKER BOX BLEW UP ON THE EMPLOYEE.
EMPLOYEE WAS WORKING THE BREAKER BOX HANDLE TO THE TYROCK SCREEN IN AND OUT TRYING TO IMGAGE IT. EVIDENTLY, THERE WAS AN ARCH ACROSS THE CONTACTS CAUSING TO BOX TO EXPLODE.

EMPLOYEE DOES NOT KNOW EXACTLY WHAT HAPPENED. HOWEVER THERE WAS A FALL INVOLVED, FROM THE GRADALL HE WAS OPERATING. EMPLOYEE RECEIVED ABRASION ON RIGHT ELBOW, PUNCTURE WOUND ON UPPER LEFT ARM, LACERATION ON UPPER FOREHEAD, BRUISES TO LOWER BACK, AND 2 BLACKENED EYES. FRACTURES TO 3 TEETH.
