Employee was removing a pin connecting the portable crusher to the loader. When tapping out the pin with a hammer and pry bar, the tongue of the portable crusher moved towards the employee causing the pry bar to lacerate the employee's left index finger. The employee required stitches to treat the laceration.

EE drilling in sheet metal, bit hung up twisted hand/arm. No medical at time, later reported pain, had treatment. EE advised of strain shoulder/upper arm. EE returned, regular job duties. Follow up 3/9 added "no operating heavy equip." EE unable to perform the regular job duties. 7000-1 reported within 10 business days of new restrictions. Terminated on 3/14, unrelated to injury.

As the employee was ascending an excavator, EE experienced pain in knee. Because there was no accident, the injury claim was initially denied but later accepted as a work related injury. This injury is being reported within 10 days of the determination that it was a work related injury.

Equipment operator stepped into a depression in the ground which aggravated his back. Emergency room doctors determined that the employee had a preexisting degenerative lumbar disk condition which was aggravated when stepping from the excavator into the depression. No workdays were missed, but the Dr. placed the employee on limited duty for recovery.

Lower back strain when lifting a box of paper. Went to the doctor and was release for duty. Had follow up visit on 10-18-06 and was prescribed 12 session of physical therapy.

Was removing mill bearing on Cedar Rapids Crusher. He was using a piece of metal round stock to loosen bearing race from mill bearing housing. With continued blows of the round stock to the bearing race he has developed acute tendinitis in his right elbow.
Was standing behind a truck rolling hose, lost footing, slipped forward, tried to catch the back of the truck, back began to hurt.

While climbing on crusher ee stepped on tire that was off the ground. The tire spun around causing injury to neck.
EE FELL TO THE GROUND WHEN THE STEP GAVE WAY ON THE WATER TRUCK. HE LANDED ON HIS LEFT SHOULDER. NOTE: THIS WAS REPORTED AS A DOCTOR FIRST AID UP UNTIL 6/10/04 WHEN THE MRI RESULTS INDICATED THAT SURGERY WOULD BE REQUIRED. RECLASSIFIED AS A RESTRICTED DUTY ON 6/10/04.

LEFT INDEX FINGER WAS CAUGHT IN AIR OPERATED CYLINDER FOR SWING BRAKE WHILE CLEANING IT, RESULTING IN A COMPOUND FRACTURE. THERE WAS NO RESTRICTED DUTY OR LOST TIME AS A RESULT OF THIS INJURY.
