Employee hit a troughing roller on a conveyor with a hammer and struck hand fracturing EE's right thumb and index finger, as well as lacerating EE's right index finger requiring seven stitches.

A large rock lodged in conveyor at head pulley, employee got on belt and kick belt rock out, when EE did the rock went out and EE's leg was trapped between the belt and head pulley, EMS showed up and cut the belt to get EE's leg out, the counterweight dropped and through them off and they were able to get EE's leg out.
Employee was using a drill to tighten the nuts on the lower side of the apron of a crusher when the load moved sideways to the ground catching the employee's finger causing a laceration and fracture.

The technician was repositioning a large trunnion cap for cleaning when it slipped from their grasp pinching left thumb between the cap and the catwalk. The Supervisor called Work Partners, they advised them to go to the hospital. The Foreman from the day-shift took them to the hospital where they were found to have a fractured thumb tip and received five stitches.
While working on conveyor employee hurt their left arm.
Employee jumped from a 992G loader in a panic thinking it was on fire injuring both heels. The loader was not on fire after turning the equipment off.
Employee was dismounting a small excavator at night and lost balance on the track and fell 2.5' to the ground breaking 4 ribs.

Employee reported back was hurting after washing off the dragline tracks. This injury did not become MSHA reportable until employee received treatment on 1/12/2022.

IE was moving the portable crusher by remote control when a small piece of metal broke from an undercarriage roller and struck abdomen. IE transported to hospital where a 2nd piece of metal was found in abdomen from 20 yrs ago. Dr decided newer piece of metal could remain but recommended removal of older piece for fear of working into bowels. Dr removed metal, not sure which piece.
The employee was injured in the left hand and leg when EE was struck by metal chips from a hammer.

The contract employee was removing drill steel when the pipe wrench made contact with the contract employees thigh causing a laceration. The work that was being done by the drilling contractor was being done for Dade County and was not mining related.
The employee had a piece of slag fall into ear while welding. The employee reported it to manager, but did not seek medical attention at that time. The employee later saw a physician and was advised the injury required surgery, which occurred on 5/9/17. We are reporting this injury within 10 days of the date of surgery.

The employee was removing an old shut off valve from the fuel truck while on one knee and two hands on the pipe wrench, his right hand pulled the wrench and he felt something on his arm/ shoulder joint that started to feel pain. This was being treated as a doctor visit injury until surgery was diagnosed to be scheduled 4/3/2013 he was working with restrictions.

Manitowoc 4600 unit 52 fell into eastern side of section 26 pit. At time of this 7000-1 submission the cause is still unknown & under investigation. Employee sustained minor injuries exiting machine. The injuries are as followed: stitches corner left eye, stitches near left arm pit and head trama ( concussion).
In the process of washing rock off of Grizzly Bars, the Bars became slippery. Individual had finished cleaning and was proceding to climb out of unit when he slipped before exiting. The individual landed on his back and right elbow. 5 ribs were hurt and a lacerated elbow.
While socketing cable clive pin it suddenly went free into place causing partial severing of fingertip (right ring fingertip).
While performing maintenace on Hazemag crusher; unexpectly, a hydralic hose bust very near employee's left hand. Even thought employee was wearig a protected leather glove the force of the hydraulic hose pressure penetrated the glove and injected hydraulic fluid into his left index finger.
While cleaning under the feeder motor with a chipping hammer the tip, of the chipping hammer broke off and hit his left thumb a piece of metal embedded in his thumb and it had to be medically removed.

While moving blowbars next to south Hazemag crusher EE used a 3 ft prybar to lift when prybar slipped falling on top of his right foot. This caused a laceration that needed 13 stitches to close. He was wearing steel toe boots.
While climbing over ramp berm employee lost footing & put arm down to break fall. Employee hurt his wrist but refused medical care. One week later employee went to personal physycian ( July 1st) and had X-Ray performed revealed a fracture of the left wrist. ( employee through out week refused medical treatment numerous times)
Employee was unplugging chute on Hazemag crusher when a rock fell down on right hand causing laceration

WHILE CLOSING THE VALVE ON PORTABLE PROPANE TORCH, PALM OF HAND SLIPPED ON ROUGH EDGE OF NOZZLE, CAUSING LACERATION.
While cutting rubber skirt, utility knife slipped cutting his left wrist requiring four stitches.
WHILE ASSISTING MAINTENANCE PERSONEL IN ADJUSTING A GEAR BOX, EE PLACED HANDS UNDER THE HOUSING OF THE BOX. THE BEAR BOX SLID DOWN THE ADJUSTING TRACK AND PINCHED INDEX FINGER ON RIGHT HAND CAUSING A LACERATION. EE WAS ADVISED NOT TO PLACE HANDS IN THIS POSITION.

While fastening the top deck of the #1 shaker with an impact gun, apparently air and some loose slag on the side wall of the shaker blew into his face and some foreign object into his eye. Employee was wearing safety glasses.
While opening watergate valve his right index finger got caught in between packing adjusting screw and handle.
THE MAINTENANCE SHOP SUPERVISOR WAS AT A JOBSITE REPAIRING A HYDRAULIC CHIPPER. WHILE ADJUSTING A TAP (THREAD CHASER) USING A PNEUMATIC SANDER, THE SANDING DISK FAILED & SMALL PIECES OF THE DISK STRUCK HIM ON THE LT. INDEX FINGER.
While moving boxes of parts in the shop employee strained his shoulder. Note: This event was treated as a plant first aid until surgery was diagnosed (with restrictions) on 7/2/04.

WHILE MOVING MARINE CABLE OVER A BERM HE STEPPED WRONG TWISTING KNEE, CAUSING HIM TO FALL. INCIDENT WAS NOT REPORTED UNTIL A WEEK AFTER IT OCCURRED.
WRENCH SLIPPED IMPACTING FINGER WITH STEEL PLATE CAUSING LACERATION.
EE WAS SQUATTING DOWN WHILE FUELING THE LOADER. WHEN HE FINISHED FUELING HE STOOD UP AND WAS COMING DOWN THE STAIRS & LOST HIS FOOTING. AFRAID HE MIGHT HIT THE LOADER HE PUSHED OFF LANDING ON HIS LEFT FOOT THEN SHIFTED HIS WEIGHT TO HIS RIGHT LEG WHEN HE HEARD A POP HE SAT DOWN COULD NOT MOVE HIS LEG WHEN HE TRIED GETTING UP. WHILE THIS WAS HAPPENING IT WAS RAINING HEAVILY.
TIGHTENING AND ADJUSTING NUT WITH A WEDGE ON THE NORTH PRIMARY BEARING. WHEN HE STRUCK THE WEDGE WITH A HAMMER, HE MISSED STRIKING HIS LEFT MIDDLE FINGER.
INSTALLING BLOWBARS ON CRUSHER, BAR SLID TOO QUICKLY CATCHING THUMB BETWEEN CRUSHER WALL AND END OF BLOWBAR.
MOVING HYDRAULIC POWER UNIT, FINGERS OF RIGHT HAND WERE CAUGHT BETWEEN STEEL TABLE AND BASE OF POWER UNIT.
