Employee was raising the inspection door on the secondary crusher when it slipped out of their left hand slamming onto their right hand. Resulting in a laceration/fracture to their right ring and middle finger.
Employee was drilling a hole through the frame of a hydraulic press to install a guard when the drill caught and spun causing the employees hand to spin and EE's index finger struck the metal bracket causing the laceration.
The employee was using a sledge hammer to put the pins in a bucket that they had just placed on an excavator and as EE swung the hammer EE heard a "pop" in EE's knee.

EE was filling the DEF tank on the loader when EE got EE's left hand caught between the door and the counterweight on the rear of the loader
EE was performing the maintenance at Glen Gardner Quarry. EE was shoveling under the sand screw. Shovel slipped and hand made contact with the sharp metal and resulted in a minor cut between the 2nd and 3rd finger of right hand.

while replacing tracks of an old excavator employee was working with colleague. while the affected employee was holding the roller in place, colleague was swinging the sledge hammer. during this process, a tiny piece of metal shot off the roller and pierced into employees hand (right hand wrist area - just above the glove line - where a wrist watch would be worn).

Freezing rain and Cold condition contributed to the incident. While walking toward pump to fuel unit in pit. Slipped due to freezing rain injuring left hand. Employee taking for treatment and was diagnosed with a hairline fracture of the left hand and is on restricted duty till further notice

While repairing damaged conveyor belt using utility knife, EE pulled utility knife into left knee. EE was take to the doctor office and received 3 stitches. EE returned to regular duty work same day.
Employee was disconnecting 4 inch coupling, as employee tried to pull it apart. The coupling struck employee in the mouth. chipping employee's tooth.
Tripped while stepping over elevated power line. Strained tended in right arm around elbow.

Employee clocked out at 3:30 pm and was in personal vehicle driving out of the facility. EE attempted to drive past the loader as it was backing and the loader hit EE's car. EE has pain in shoulder - an MRI did not indicate any problems but the doctor suggested some therapy sessions.

Miner was cutting rubber belting to be used as conveyor skirting when the razor slipped off the rubber and cut his right thigh. The laceration was approximately 1.5 cm long.

Trucker working for Vitell: trucker was trimming (knocking stones off side truck) his load slipped and fell 12 feet landing on his right side hitting his arm, leg and head on the ground which had stones there. A trucker on the scale saw him on the ground and went inside for help.
Area not used in Plant B. EE climbed onto the cat walk circumventing the blocked off stairway. EE of his own acord decided to clean the 3/8 flat converyor cat walk in this blocked off area. He stepped onto an area with no grating. While falling he grabbed the safety railing on the cat walk, pulling the muscles in his left shoulder.
EE was taking fan guard off a 988 loader using a 3/8" rachet to unbolt it. The rachet became unlocked, right hand banged into a metal post on the loader. Over time the knuckle became swollen. Pain in hand did not go away.
EE was servicing the haul truck. He was climbing the ladder. He hit the inside bone next to the knee cap on the corner edge of the step on the ladder. No lacerations occurred at the time of the accident. The injury point was sore. EE called in accident on 5/3/06, then sought medical treatment, because knee was stiffening up and not getting better.
All day employees were rebuilding crusher of B plant. Using crane to lower chute back in place between the bypass chute & chute, employee on the crusher itself by the bypass chute was guiding the chute piece into place when it became pinched between chutes. When he released the chute it swung down smashing his right hand between chute & bypass chute (x-ray; no fracture).

PRIMARY CRUSHER CHUTE PLUGGED UP. RAN DOWNSTAIRS TO UNPLUG. IT WAS AN EMERGENCY SINCE NEW TRUCKS HOLD MORE STONE THAN CRUSHER CAN HANDLE. WHILE UNPLUGGING A ROCK FLEW INTO RIGHT EYE. BLOOD SPO T WAS IN EYE. WENT TO EYE DOCTOR TO GET CHECKED ON.
WAS DRAINING OIL OUT OF OIL TANK ON PRIMARY CRUSHER. DRAIN LINE WAS PLUGGED, USING STICK TO UNPLUG CLOG CAME LOOSE AND FELL INTO OIL REFUSE BUCKET AND SPLASHED INTO RIGHT EYE.
