As the Excavator operator started to access steps to enter cab, the operator felt dizzy. Operator stepped back to ground level and leaned on tracks to regain stability. At that time, the operator was taken to the hospital for observation which resulted in a Medical Complication (low Blood Pressure).

IW took sample upstairs to work it up. EE followed all procedures, washed the screen and went to put solids on the run sheet. EE began to itch intensely and broke out in hives.

EMPLOYEE DESCENDING FROM LOADER WHEN EMPLOYEE SLIPPED AND FELL DOWN STEPS/LADDER AND HURT BACK.

A 10 PASSENGER VAN (WITH SUPERVISOR INSIDE) WAS STRUCK BY AN OFF-ROAD TRUCK. SUPERVISOR ESCAPED THROUGH WINDSHIELD WITHOUT PHYSICAL INJURY BUT WAS BADLY SHAKEN FROM THE ACCIDENT.

EMPLOYEE REPORTED TO WORK AND STARTED FEELING WEAK AND LOSING BLOOD. 911 WAS CALLED AND EE WAS TAKEN TO HOSPITAL BY AMBULANCE. THIS IS NOT DUE TO AN INJURY OR ACCIDENT. THIS IS A MEDICAL CONDITION. MSHA 800 NUMBER WAS CALLED AND THIS WAS INVESTIGATED BY INSPECTOR OF THE MACON, GA FIELD OFFICE.

Injured Employee was pulling a 1-1/2in x 15ft.cable,4ft. to attach to the back draw bar of his dozer that was stuck. As he attached the cable to the draw bar, he felt a sharp pain in his lower back.He continued operating the dozer until the end of the shift.

EMPLOYEE WALKING DOWN WET CLAY RAMP WHEN HE SLIPPED AND FELL ON HIS UPPER LEFT BACK SUSTAINING FIVE (5) BROKEN RIBS.

Employee was welding, cutting, and grinding plate metal and got a small metal flake in his eye. Employee reported that his eye felt irritated, so it was washed with an eye rinse solution. Employee was then sent to the doctor, and the metal flake was removed. Employee is expected to make a full recovery.

Employee stepped in pot hole caused by rain. Holes were obscured by mulching that covered hole.

Employee was sitting in air-conditioned controlled room after experiencing chest pains. THIS WAS REPORTED TO MSHA AS SOON AS WE KNEW CONDITION WAS RELEVANT TO CALL.

EMP WAS DESCENDING LOADER, WHEN HE MISSED HANDRAIL, CAUSING HIM TO SWING AROUND HITTING HIS LEFT KNEE AND CAUSED HIM TO FALL AND INJURE HIS RIGHT ANKLE. HIS SUFFERED A DISLOCATED KNEECAP AND TORN RIGHT ACHILLES ANKLE/HEEL.

Employee was assisting in relocating a section of pit pump cable and stepped into a hole as he was pulling the cable. Employee stated that he felt his left knee hyperextend. Employee was wearing all PPE required at the time.

Driver was lifting and releasing tailgate of dump trailer in an attempt to clear dirt and debris so tailgate would close. On the last release of the tailgate the driver's left hand ring finger was caught between the tailgate and the bed of the truck causing a laceration which required stiches.

DRIVER OF DUMP TRUCK SAID HE HIT A DIP IN THE PIT, BOTTOMED SEAT OUT JARRING HIM AND HURTING HIS BACK.

Lineman was installing used arrestor on power line, when arrestor energized it exploded causing electrical arc. Lineman received 2nd degree burns to upper right arm.

EMPLOYEE WAS CLEANING TAIL PULLEY ROLLER ON INCLINE BELT CONVEYOR. WHILE ATTEMPTING TO TURN ROLLER WITH BAR, BAR SLIPPED CAUSING EMPLOYEE TO FALL FORWARD STRIKING BAR WITH RIGHT ARM RESULTING IN A BREAK TO HIS FOREARM.

Employee was attempting to open a water valve to put water in the pipeline and in do so, felt a stinging pain in their back/side.

EMPLOYEE WAS RE-ROUTING CHEMICAL LINE WHEN AMA 415 CHEMICAL SPLASHED ON UPPER ARM CAUSING A RASH TO APPEAR.

EE WAS HELPING UNLOAD CASES OF ANTI-FREEZE FROM A TRUCK. AS HE WAS PICKING UP CASES HE EXPERIENCED A SHARP PAIN IN HIS NECK AND SHOULDER. HE THOUGHT IT WOULD BE OKAY BUT PAIN KEPT PERSISTING & WE ADVISED HIM TO SEE A WORKERS'COMP. PANEL DR. HE HAS HAD PRE-EXISTING CONDITIONS IN THIS AREA.

WHILE MOVING PIPE THE JOINT OF THE PIPE ON ONE END STRUCK A PILE OF DIRT CAUSING THE OTHER END OF THE PIPE TO DROP, STRIKING EE ON RIGHT FOOT

WHILE TIGHTENING A CLAMP AND GASKET ON PIPE OVERHEAD A FOREIGN MATERIAL FLAKED OFF AND GOT IN LEFT EYE CAUSING IRRITATION. PRESCRIPTION GIVEN.

EE WAS STANDING ON PIPE TO HOOK CALIPERS, THE TRACKHOE NEEDED TO REPOSITION. WHEN DOING SO THE BUCKET STRUCK THE EE ON THE RT SIDE.

EE WAS OPERATING LOADER AT MINE. EE WAS CLIMBING DOWN OFF LOADER WHENM HE STEPPED ON BOTTOM STEP AND HIS FOOT SLIPPED CAUSING HIM TO FALL ON HIS RIGHT SHOULDER CHIPPING BONE AND INJURING THE R OTARY CUFF.

EMPLOYEE OPERATING DUMP TRUCK IN ABOVE MINE ATTEMPTED TO DUMP LOAD OF CLAY THAT WAS APPARENTLY WET ENOUGHT TO STICK IN DUMP BODY MOST OF THE LOAD ON THE RT SIDE OF THE DUMP STUCK IN BODY CAUSI NG THE UNIT TO OVERTURN ON ITS RT SIDE EMPLOYEE RECEIVED BRUISES OVER UPPER BODY & RT ARM/SHOULDER

WHILE PUSHING TREES OVER WITH BULLDOZER, LIMB STRUCK EMPLOYEE ON FOREHEAD CAUSES HIM TO HAVE SITICHES. THE LIMB CAME THROUGH THE ROPS OF THE DOZZER.

EMPL WAS USING BAR TO SLIDE 12" PIPE OVER TO LINE UP WITH 12" FLANGE BAR SLIPPED CAUSING IT TO SMASH L HAND RINGER FINGER AGAINST PIPE

KNOCKING INSULATION OFF VALVE WITH CHISEL AND HAMMER. FRAGMENT FROM END OF CHISEL CHIPPED OFF WENT IN L LEG BELOW KNEE

I NAME WAS BHLADING UP A DUMP WITH A DOZER & HE RAN INTO A DITCH THE SUDDEN JOLT THREW I NAME INTO THE TOP OF THE CAB IMPACT OF HIS HEAD TO THE TOP OF THE CAB CAUSED LACERATION TO THE TOP OF I NAME HEAD HE WAS TAKEN TO HOSPITAL FOR STITCHES& XRAYS

EMPL WASD DRILLING HOLE IN PIECE OF METAL WHEN DRILL BIT HUNG UP CAUSING DRILL TO TURN SLAMMING HIS HAND INTO METAL FRAME. EMPL CUT MIDDLE FINGER ON RIGHT HAND.
