The crane operator was backing and was not aware the employee in the back had placed themselves behind the crane and the employee was pinned between the hand rail and the crane injuring their leg.

As the Excavator operator started to access steps to enter cab, the operator felt dizzy. Operator stepped back to ground level and leaned on tracks to regain stability. At that time, the operator was taken to the hospital for observation which resulted in a Medical Complication (low Blood Pressure).

A 10 PASSENGER VAN (WITH SUPERVISOR INSIDE) WAS STRUCK BY AN OFF-ROAD TRUCK. SUPERVISOR ESCAPED THROUGH WINDSHIELD WITHOUT PHYSICAL INJURY BUT WAS BADLY SHAKEN FROM THE ACCIDENT.
EMPLOYEE REPORTED TO WORK AND STARTED FEELING WEAK AND LOSING BLOOD. 911 WAS CALLED AND EE WAS TAKEN TO HOSPITAL BY AMBULANCE. THIS IS NOT DUE TO AN INJURY OR ACCIDENT. THIS IS A MEDICAL CONDITION. MSHA 800 NUMBER WAS CALLED AND THIS WAS INVESTIGATED BY INSPECTOR OF THE MACON, GA FIELD OFFICE.

Injured Employee was pulling a 1-1/2in x 15ft.cable,4ft. to attach to the back draw bar of his dozer that was stuck. As he attached the cable to the draw bar, he felt a sharp pain in his lower back.He continued operating the dozer until the end of the shift.

Employee was welding, cutting, and grinding plate metal and got a small metal flake in his eye. Employee reported that his eye felt irritated, so it was washed with an eye rinse solution. Employee was then sent to the doctor, and the metal flake was removed. Employee is expected to make a full recovery.
Employee stepped in pot hole caused by rain. Holes were obscured by mulching that covered hole.

Employee was assisting in relocating a section of pit pump cable and stepped into a hole as he was pulling the cable. Employee stated that he felt his left knee hyperextend. Employee was wearing all PPE required at the time.

Driver was lifting and releasing tailgate of dump trailer in an attempt to clear dirt and debris so tailgate would close. On the last release of the tailgate the driver's left hand ring finger was caught between the tailgate and the bed of the truck causing a laceration which required stiches.

Employee was attempting to open a water valve to put water in the pipeline and in do so, felt a stinging pain in their back/side.
