Mechanic used rag to check for debris on matting surface of yoke. Metal splinter entered left ring finger. Splinter pulled out mechanic said was fine. On Monday 5/19/14 mechanic reported to plant manager that his finger was hurting and requested medical treatment. Metal splinter was removed by doctor, there was abscess so prescription antibiotic was prescribed

EMPLOYEE STATES: LLOADED BACK OF TRUCK WITH LOADING POLES THAT WERE ABOUT 10'L. ABOUT 2' WERE STICKING OUT OF THE BACK OF THE TRUCK. EMPLOYEE WALKED AROUND THE BACK OF THE TRUCK AND THE POLE STRUCK EE JUST BELOW HIS RIGHT-EYE. CAUSING HIM TO NEED 9 STITCHES AND AN X-RAY. THERE WERE NO FRACTURES OR SINUS DAMAGES.

WHILE THE MECHANIC WAS PLACIN G A BLOCK UNDER THE PLATE THE STEEL DROPPED AND MSHED THE MECHANIC'S FINGER BETWEEN THE WOOD BLOCK AND STEEL PLATE.
THIS ACCIDENT HAPPENED WHILE PERFORMING ELECTRICAL UPGRADES TO THE PLANT'S MASTER CONTROL CENTER. PRIOR TO BEGINNING WORK TASKS IN THE MASTER CONTRO CENTER, THE 2 MAIN POWER BOXES WERE DISCONN

THE ACCIDENT IS BEING INVESTIGATED BY TGI'S RISK MGNT AND SAFETY DEPARTMENTS.
THE OPERATOR WAS CHECKING AN ALTERNATOR BELT ANDDID NTO REALIZE THE MECHANIC WAS ABOUT TO START HIS TRUCK. THE TIP OF HIS INDEX FINGER WAS PULLED THRU THE ALTERNATOR BELT.

EE WAS HELPING TWO OTHERS LOAD A 250 LB TRANSFORMER INTO A PICKUP. EE WAS USING THE CORRECT LIFTING METHOD UNTIL THEY REACHED THE TRUCK AND HE EXTEND THE WEIGHT LOAD OUT TO PLACE IN THE TRUCK BED. EE POSSIBLY TWISTED UPPER BODY RESULTING INA PULLED BACK MUSCLE.
THE EMPLOYEE WAS FOUND UNCOUSCIOUS, SLUMPED AT THE PASSENGER DOOR OF INTERNATIONAL FUEL TRUCK #5204. CPR WAS PERFORMED, AMBULANCE ARRIVED, CORONER PRONOUNCED EMPLOYEE DEAD AT THE SCENE. PROBABLE CAUSE OF DEATH IS HEART ATTACK.
