On 8/13/24, employee was throwing away a 20 lb scrap pump into metal waste bin when EE felt discomfort in EE's back right side. EE used 1st aid measures to help with the discomfort for 2 weeks. When the pain did not resolve EE sought medical treatment on 8/27, were diagnosed with a back strain and issued restrictions & medications.

Employee was making their way to the back of a locomotive via the walkway and slipped on a step. They landed on their left knee. Later in the morning employee was still feeling discomfort and asked to go to the clinic. Employee was evaluated and referred for further evaluation and given light-duty restrictions.

Employee was swinging a hammer in a downward motion when the head of the hammer flew off, causing the employee to increase momentum. The sudden increase in momentum resulted in pain in the employees left shoulder.

Limited ability to sustain body positions.

EE was changing screen in screen #5. As EE was attempting to exit the area EE set right leg to side and felt pain in knee. The pain immobilized use of right leg and EE needed assistance to vehicle. EE was escorted to ER by spouse.

Return roller fell on finger causing fracture.

Employee tore a ligament in EE's back while lifting a skid steer tire.

EMPLOYEE FILED WORK COMP ON 11/24/2015 FOR CARPAL TUNNEL IN BOTH WRISTS. ON 6/20/2017 HE HAD SURGERY ON LEFT WRIST.

Employee lost his balance while exiting loader and fell to the ground. He sought medical attention and is currently losing time.

Employee was removing a new Hawkbill flip knife from it's plastic packaging when his hand was cut by the blade, requiring two stitches.

Employee experienced a laceration on his finger while assisting another employee with a belt replacement on a conveyor. He sought medical attention, received 4 stitches and was released to full duty.

Employee was performing maintenance on a conveyor when he felt pain in his right shoulder. After seeking medical attention on Feburary 9, 2015 the injury was determined not to be work related. On December 23, 2015 a determination was made that this injury was work related. This is being reported within 10 working days of this notification.

While assembling loader tire bead band became bound. Trying to seat the band the tech struck the top portion of bead band and lower portion sprung back striking tech in the upper abdominal area.

Received results indicating employee reached the MSHA levels of silicosis.

Tech was backed up to a 777C wheel to install wheel studs. The hooks were too small to properly fit in the hole to lock hold. Tech was holding the hook in place and when the load hit the hook, it slipped out and caused nail avulsion on Left Middle finger.

Climbing down ladder from top deck station #3, when knee twisted & popped. Employee was done with work on the top deck Station #3, as he went to the ladder to get down he turned sideways to get into the ladder and popped his left knee. This first aid incident did not reveal to require surgery until May.

DRIVING HAUL TRUCK AND HIT BUMP IN THE ROAD. HIS SEAT CAME BACK DOWN AND HIT HIS ELBOW ON THE FIXED ARM REST ATTACHED TO DOOR.

Employee was changing out worn brake shoes on a company owned railcar. While using a new brake shoe to tap worn brake shoe out, felt a pop in his left shoulder and experienced pain. Immediately reported to supervisor but did not want to go to the clinic. Went to clinic on 11/08/2010 because of continued pain in left shoulder.

Employee was flipping gates to switch from 57 to 467 in Station 6. Went to flip gate over and it didn't shut all the way. Employee pulled it back to shut it the rest of the way and didn't let go of it in time resulting in his middle finger on left hand being smashed.

While dismounting drill rig, stepped off of ladder onto a rock which rolled causing him to roll (sprain) his ankle.

A crew of 4 men were installing a belt, using a forklift and boom truck. Two employees were watching. One stationed at the tail pulley; one between the tail pulley & boom truck. The Forklift was on the opposite side of the conveyor pulling the belt. As the belt went around the pulley the employee reached in. The belt jumped and pulled his hand into the tail pulley.

EE WAS DRILLING. HE TURNED UP SETTINGS ON COMPRESSOR. APPROX 5 MIN AFTER TURNING UP THE PRESSURE, HOSE CARRYING AN AIR/OIL MIXTURE BLEW. HOSE WAS NEXT TO COOLING FAN WHICH BLEW THE MIXTURE INTO THE TURBO ON THE ENGINE & MIXTURE IGNITED. HE LOOSENED HARD HAT & RECEIVED 2ND DEG BURNS ON HIS HEAD. HE THEN PUT THE FIRE OUT W/THE EXTINGUISHER IN THE RIG. FIRE LASTED 3-4 MIN.
