EE was using a cordless impact to undo bolts on the CR1 flywheel. EE positioned a hand behind the sheave, between the crusher frame and the nut to hold the nut when it broke free. When the bolt broke loose, the nut followed the threads, moving backward and pinching the employees finger between the nut and the frame. EE was brought to get X-rays where a fracture was discovered.

Hilltop Energy Inc had a Flyrock incident that struck a parked vehicle when 4 rocks where collected from a local Park area. No injuries occurred and MSHA was called and on scene.

EE was washing out aggregate bins in screen plant, plugged in a work light and was electrocuted by the 120V outlet. EE was soaked from using the hose to wash out and was not properly grounded when using the plug. Non-GFCI outlet and extension cord were in use.
EE was unplugging transfer hopper when rocks broke free hitting the bar that EE was using to free up material. The bar pushed back and caused EE's left hand to impact the conveyer frame still holding the bar. EE had MRC predator Safety gloves ANSI IMPACT 1, PUN 2, ABR 3. EE was sent to ER for X-ray. EE got 8 stiches w/ antibiotics. No ligament or bone damage.
EE was working on rock breaker at primary crusher with supervisor - after the shift EE felt soreness and pain in shoulder which was later found to be a rotator cuff injury

INJURED WAS BENT OVER WELDING WHEN HE STOOD UP HE STRAINED HIS BACK WHILE SLIPPING ON WELDING CABLE.IMPORTANT HE KEPT ON WORKING UNTIL 03-15-95 AND WENT OFF WORK ON 03-15-95.
