On Tuesday 1/24/2023 the EE experienced a first aid injury to EE's right shoulder while lifting channel iron. On 3/23/2023 EE visited the clinic and was diagnosed with a tear in EE's shoulder area and given work restrictions.

A water pump in the pond was being replaced by 6 employees. One employee was holding a 48" pipe wrench holding the shaft in place. Another employee was using a 36" pipe wrench to turn the shaft to break it free. As the employee turned the shaft, the employee holding the pipe wrench lost grip and the tool swung around coming off striking an employee in the head above left ear.
Employee exited water wagon which was parked by water stand to turn off water valve which is located 20' from water wagon. Employee stated they stepped on soft spot and twisted ankle.

On Thursday, November 14th, the employee reported to the site supervision that they were experiencing pain in abdomin due to overexertion from a task performed October 28, 2019. Employee alleges they felt a small (nickel sized) ball behind belly button they felt on Tuesday, November 12th at home.
Employees were replacing troughing idler on conveyor. Employees left removed idler leaning on conveyor. When employee began to re-install guards, they bumped and shook idler leaning on conveyor, it fell over striking them on the left metatarsal causing bruising

EE was performing maintenance with a pry bar and allegedly felt a pull in the stomach area. Monday, EE stated EE was lifting grease cover and felt a pop in the stomach area.
Employee had a periodic screening chest x-ray read by two B-readers using the ILO procedures. The X-ray was characterized as consistent with pneumoconiosis with readings of <=1.5mm and <=1.5mm. Damage or impairment cannot be determined solely by reading chest x-ray films. This report is being submitted to satisfy MSHA program policy letter P92-III-2.

The employee alleged that while operating a dozer pushing material, he backed to the top of the slope and when the dozer broke over to even ground, he felt pain in his lower back and leg. He was diagnosed with a lumbar strain and disc degeneration.

EE WAS PUTTING PIN AND SPACERS ON PIECE OF EQUIPMENT WITH THE HELP OF 2ND EE. THE 2ND EE PINCHED THE HAND OF THE 1ST EE WITH THE RIPPERS, CAUSING A LACERATION OF HIS FINGER WHICH WAS STITCHED THAT SAME DAY.
EQUIP. OPERATOR CHOSE TO CHANGE THE NORMAL ROUTE OF HAUL. HE CROSSED THE PATH WHICH THE SCRAPERS HAD BEEN HAULING AND HIT A ROUGH SPOT, CAUSING A JOLT AND INJURING HIS BACK. IT IS UNKNOWN WHY THE OPERATOR CHANGED THE HAUL ROUTE FROM WHAT HE WAS INSTRUCTED TO DO.
