IW left pinky finger got caught between the drive belt and sheave resulting in a laceration.
EE strained back while lifting 14" flange with backing ring.
Employee was completing a final drive rebuild for a 745 axle. The final drive had already been disassembled and laid out on table for cleaning. Employee went to stand spindle up on table when the spindle slide and caught employee's left index finger between the spindle and table.

While working inside C-1 chute employee alleges ee was hit on the hard hat by loose material causing ee to fall backwards and hit ee head on the chute.
Employee slipped and fell to the ground injuring right shoulder.

Employee was working on changing a hydraulic line on the blade of the dozer. When climbing out to get parts, right foot slipped on the arm of the blade and went between the arm and the tracks as the rest of their body fell towards the ground resulting in ankle and knee pain. Employee did not seek medical treatment until 10/23/2017.
Employee stepped from conveyor concrete platform and hyper extended left knee.

Employee was walking toward the steps leading to the log washer. The first step was under water. As the employee was ascending the stairs, employee's right foot slipped off the bottom step, and bent employee's toe back.

Three juveniles were trespassing at the quarry. One juvenile slipped into a pond and drown.
An employee performing was drilling a hole in the back of an electrical box bracket. In the process of wallowing out the hole to make it larger, the drill bit slid out of the hole and contacted the employee's palm of his left hand. This resulted in a laceration that required stitches.
