IW left pinky finger got caught between the drive belt and sheave resulting in a laceration.

EE strained back while lifting 14" flange with backing ring.

Employee was completing a final drive rebuild for a 745 axle. The final drive had already been disassembled and laid out on table for cleaning. Employee went to stand spindle up on table when the spindle slide and caught employee's left index finger between the spindle and table.

While working inside C-1 chute employee alleges ee was hit on the hard hat by loose material causing ee to fall backwards and hit ee head on the chute.

Employee slipped and fell to the ground injuring right shoulder.

Employee was working on changing a hydraulic line on the blade of the dozer. When climbing out to get parts, right foot slipped on the arm of the blade and went between the arm and the tracks as the rest of their body fell towards the ground resulting in ankle and knee pain. Employee did not seek medical treatment until 10/23/2017.

Employee stepped from conveyor concrete platform and hyper extended left knee.

Employee was walking toward the steps leading to the log washer. The first step was under water. As the employee was ascending the stairs, employee's right foot slipped off the bottom step, and bent employee's toe back.

Three juveniles were trespassing at the quarry. One juvenile slipped into a pond and drown.

An employee performing was drilling a hole in the back of an electrical box bracket. In the process of wallowing out the hole to make it larger, the drill bit slid out of the hole and contacted the employee's palm of his left hand. This resulted in a laceration that required stitches.

EE was using a hammer to dislodge the roller. When he hit the roller it broke free and fell back towards him, falling on his finger.

EE was replacing a pipe under a screen tank. He noticed after cutting,couple sparks falling on a rubber hose. He poured a bucket of water into the hose then covered the hose to prevent further problems. He noticed smoke again coming from the pipe. With assistance & fire extin, he determined the fire out of control and contacted

EE received concussion with no loss of consciousness after hitting his forehead on a cab jack while underneath a D400E haul truck while working on a retarder line.

Employee was using a loader and was pushing brush up into a pile, the loader was an open cab. While doing this the employee was struck by a stick that had come up through the cab and struck him in the head, rendering him unconscious for approximately one to two minutes

EE right hand was contused after hitting it on the haul truck hand hold after slipping and falling off the cab.

CONTRACTOR EMPLOYEE WAS BITTEN BY A SNAKE WHILE SERVICING FIRE EXTINGUISHERS AT VULCAN. HIS LEFT LEG WAS BITTEN.

EE WAS HELPING COWORKER PLACE GEASE DRUM ONTO BACK OF FUEL TRUCK. HIS HAND WAS CAUGHT BETWEEN THE GREASE DRUM AND TRUCK BED. HE WAS NOT WEARING WORK GLOVES.

EMPLOYEE WAS HELPING CO-WORKER REMOVE ROCK SCREENS FROM DECK AREA SO THEY COULD RETREIVE A WATERHOSE LAYING UNDERNEATH THE SCREENS. AS THE CO-WORKER LET GO OF THE SCREENS TO PICK UP THE HOSE, THE INJURED EMPLOYEE LOST HIS BALANCE AND THE SCREENS FELL ONTO HIM.

EE SLIPPED OFF BACKHOE TRACKS AND FELL BETWEEN TIRES OF MOVING FRONT END LOADER. REAR TIER OF LOADER STRUCK HIS HAND.

EE FELL FROM CRUSHER PLATFORM.

EMPLOYEE WAS USING OXY ACETYLENCE TORCH TO REMOVE ROCK SCREEN & WAS BURNED BY HOT SLAG

EMPLOYEE WAS STEPPING DOWN FROM BARGE WINCH AND TWISTED HIS ANKLE/FOOT.

A GUARD REMOVED FROM RINSING SCREEN TO CHANGE BEARINGS EE WALKED BY GUARD AND A SMALL BURR ON GUARD LACERATED HIS RIGHT FOREMAN.

EE WAS PERFORMING ROUTINE MAINTENANCE ON HAUL TRUCK HE WAS STOPPED OVER UNDERNEATH TRUCK AND TWISTED HIS KNEE.

55061S REMOVING THE HITCH PIN ON AN ARTICULATING REAR DUMP HAUL TRUCK BY STRIKING A CHISEL WITH A MECHANICS HAMMER THE CHISEL FLEW BACK AND STRUCK EE ON THE LEFT SIDE OF NOSE.

EMPL PERFORMING PRESTART UP INSPECTION OF HAUL TRUCK UNIT 8. TRYING TO DETERMINE LOCATION OF AIR LEAK WHILE STANDING ON EQUIPMENT STEP. THE STEP SUPPORT CHAIN BROKE AND EMPL FELL BACK ON HEEL AND JARRED BACK.

EMPL SPLICING BELT CONVEYOR AND LOST FOOTING,FALLING BACKWARD AND STRIKING HIS BACK ON TROUGHING IDLER.

EMPL USING T-HANDLE PRY BAR TO DISLODGE ROCKS FROM CONE CRUSHER AND CAUGHT FINGER BETWEEN HANDLE AND CRUSHER AS ROCK BROKE FREE.WAS WEARING WORK GLOVES.

CHANGING SCREEN CLOTH CUT FINGER ON EDGE OF SCREEN

EMPLOYEE WAS USING A HIGH PRESSURE WATER HOSE TO WASH OUT LADDER CHUT E AT END OF ROTATING TROMMEL SCREEN HE WAS STRADDLING THE WATER HOSE WHEN IT BECAME CAUGHT ON THE SCREEN & HE WAS PULLED OVER THE SCREEN & FELL ONTO DECK ON THE OPPOSITESIDE

EMPLOYEE WAS CHANGING WATER PIPES (10' X 1 1/4") ON THE DENSE MEDIA SEPARATOR. HE PULLED THE PIPE FROM OUTSIDE OF TANK AND AS THE OTHER END FELL DOWN INSIDE OF TANK, HIS FINGER WAS CAUGHT BETW EEN PIPE AND TOP OF TANK. HE WAS WEARING WORK GLOVES.

EMPL WAS WALKING DRAGLINE CRANE DOWN AN INCLINE HAULROAD INTO PIT THE CRANE WAS TRAVELING WITH THE BOOM IN FRONT AND BY EITHER OPERATOR ERROR OR MECHANICAL FAILURE THE CRANE BEGAN TO FREEWHEEL AND THE EMPL JUMPED FROM THE CRANE CAB WALKWAY AND BRUISED HIS R HEEL AS HE STRUCK THE GROUND

EMPLOYEE WAS HELPING TO HOIST BRAKE ON DRAGLINE CRANE.AS HE WAS HOLDING ONTO BRAKE BAND,HE SLIPED AND FELL AGAINST THE BAND PINCHING HIS FINGERS BETWEEN THE BAND AND BRAKE DRUM.HE WAS NOT WEAR ING WORK GLOVES.

EMPLOYEE WAS HE5PING TO CHANGE DRAG CABLE ON A CRANE. HE AND A CO-WORKER HAD ALREADY REMOVED APPROX. 10 FEET OF CABLE FROM THE CABLE SPOOL, WHEN THE OTHER END OF CABLE CAME LOOSE FROM THE SPO OL AND STRUCK H3M IN THE MOUTH.

EMPLOYEE WAS HE5PING TO CLEAN GRIZZLY.HE WAS BREAKING UP LARGE ROCKS WITH A SLEDGE HAMMER AND A PIECE OF ROCK BROKE OFF AND STRUCK HIS SHIN.

INJURED EMPLOYEE AND THE CRANE OPERATOR WERE REMOVING A TRACK PIN ON DRAGLINE CRANE IN ORDER TO CHANGE TRACK LINK HE WAS HOLDING A SLEDGE HAMMER ON PIN WHILE THE OPERATOR STRUCK IT WITH ANOTHE R SLEDGE HAMMER THE HAMMER HEAD SPLINTERED AND A PIECE OF METAL STRUCK EMPLOYEE IN THE FOREHEAD
