Elevator struck employee on left upper thigh causing contusion.

On 5/30/14 the employee requested medical treatment, was taken to the company clinic and was placed on restricted duty which is being accommodated. During a repair, the employee stepped off a structure and twisted his left knee. Employee was diagnosed with a left knee strain.

The employee went to the company clinic on 12/2/08 and was provided a lumbar support as well as returned to full duty. While driving the VMC haul truck in the plant pit, he hit a bump, causing the air ride seat to bottom out, resulting in a lumbosacral spine strain.

The employee went to the company clinic on 8/2/08 and was diagnosed with a R-hand accessory digit nail plate avulsion and fracture. While the employee repositioned forks on a forklift, a fork moved downward and crushed his second R-thumb accessory digit. The employee was returned to full duty.

The employee went to the Company clinic on 5/28/08 and received ten stitches for a left hand laceration. The employee was placing the radiator of a Peterbuilt yard truck when the spring from the hood came off and hit the employee on the left hand. The employee was released to regular work without restrictions.

The employee went to company clinic on 5/21/08 and received steri strips to keep wound closed. The employee attempted to re-loop drag cable but the cable came out and struck the right side of his face resulting in a right cheek laceration. The employee was returned to full duty on 5/21/08 by company physician.

The employee went to see the company doctor on 4/4/2006 and was placed on restricted duty. While cutting rubber with a utility knife, the knife slipped causing a laceration to R-hand, which required stitches.

The employee went to see the company doctor on 4/4/2006 and was placed on restricted duty. While driving the yard truck at the plant, the steering wheel moved suddenly causing the truck bed to tip over, resulting in a fracture to the employees R-pinky finger due to grabbing the steering wheel to regain control of the vehicle.

The employee went to see the company doctor on 3/7/06 and was placed on restricted duty. While cleaning area with the backhoe, the employee lost footing on loose gravel, slipped and fell, causing a sprain to R-shoulder.

On 2/21/06 the employee went on a personal leave and had surgery around 2/25/2006. On 3/15/2007 Vulcan Materials Company received the DWC Form from the employee informing us it was a work related injury. The employee claimed he injured lower back due to the bouncing of the truck while driving (ALLEGED).

Employee went to see company doctor on 3/29/05 because at the time of accident (2/10/05) he didn't feel necessary, however, his shoulder became irritated and employee wanted to see the company doctor; The employee has been returned to full duty. While lifting a hydraulic pump, the employee strained his R-shoulder.

WHILE RUNNING DRAGLINE DRAGGING BANK, THE CABLE BROKE AND FLEW THROUGH THE OPEN WINDOW STRIKING EMPLOYEE'S LEFT ELBOW AND BOTH HANDS CAUSING LACERATIONS AND A CONTUSION. EMPLOYEE WENT TO SEE C OMPANY DOCTOR, STITCHES WERE APPLIED AND HE WAS PLACED ON RESTRICTED DUTY. (EMPLOYEE QUIT THE NEXT DAY).

EE WENT TO SEE THE COMPANY DOCTOR ON 5-1-03, STITCHES WERE APPLIED AND EE WAS RETURNED TO FULL DUTY. EE WAS WALKING TO HIS SERVICE TRUCK AND STRUCK HIS R-FOREARM ON THE COMPANY SERVICE TRUC K WHICH CAUSED A LACERATION.

EE WENT TO SEE CO. DR. ON 1/23/03 & WAS PLACED ON RESTRICTED DUTY. EE CLAIMS THAT HIS NORMAL JOB DUTIES AGGRAVATED HIS R KNEE WHICH HE INJURED WHILE WORKING FOR ANOTHER COMPANY IN 1981. (ALLEGED)

EMPLOYEE WENT TO SEE COMPANY PHYSICIAN ON 12-4-02 AND WAS PLACED ON RESTRICTED DUTY. EE USING A 9" HAND GRINDER ON A 10" I BEAM, WHILE GRINDING THE BEAM SHIFTED OFF SUPPORT ONTO RIGHT FOOT WHI CH CAUSED A FRACTURE TO RIGHT FOOT.

HAND SLIPPED ON LATCH OF ROLL-UP DOOR WHILE EMPLOYEE WAS CLOSING DOOR. HE WENT TO SEE A DOCTOR ON 8/25/00; STITCHES WERE APPLIED TO HIS LEFT PINKY FINGER.

EE WENT TO SEE THE DR ON 6/14/00 AND WAS PLACED ON RESTRICTED DUTY. WHILE CLEANING CAT WALK WITH SHOVEL EE STEPPED ON A ROCK, FELL TO THE CATWALK AND STRAINED HIS BACK.

EE WENT TO SEE DR. ON 2-22-00 AND WAS PLACED ON RESTRICTED DUTY. EE SLIPPED WHILE PICKING OUT A CHUTE UNDER SHAKER AND SPRAINED LEFT ANKLE.

INJURY NOT REPORTED UNTIL 2-15-00 WHEN EE WENT TO SEE A DR, WHO PLACED HIM ON RESTRICTED DUTY. WHILE RUNNING TO THE CONVEYOR, EE SLIPPED ON A ROCK AND SPRAINED HIS 1-ANKLE.

WHILE WELDING AND CUTTING , EE SUSTAINED A FLASHBURN TO HIS EYES.

WHILE EE WAS WORKING ON SHAKER 5, COWORKER MOVE THE COMEALONG AND IT SLIPPED.

FALLING MATERIAL CAUSED EE TO LOSE BALANCE AND FALL AGAINST CHUTE.

EMPLOYEE INJURED STOMACH WHILE HE AND ANOTHER MAN WERE LIFTING BATTERYS IN BATTERY BOX ON D10N DOZER. THE OTHER MAN SLIPPED AND EMPLOYEE HELD ALL THE WEIGHT INJURING HIS STOMACH.

WHILE CARRYING A MOTOR MOUNT UP THE STAIRS, EE FELT WHAT WAS THOUGH TO BE A PULLED LOWER STOMACH MUSCLE. AFTER DISCOMFORT CONTINUED SEVERAL DAYS, EE REQUESTED TO SEE A DR. WHE DIAGNOSED A HERN IA. EE WAS IN COMPLIANCE TO RULES, NO MINING EQUIPMENT INVOLVED, REFRESHED TRAINING COMPLETED 2/22/94, NO PROTECTIVE ITEMS INVOLVED.

REPAIRING CRUSHER FELL BELOW BROKEN COLLAR BONE AND 3 CRACKED RIBS

STRAINED BACK WHILE LIFTING BOULDER OUT OF JAW CRUSHER. EMPLOYEE SAID HE HURT HIS BACK ON 3-1490, MISSED WORK ON 3-15 WORKED 3-16 AND 3-17, WENT TO DOCTOR ON 3-19- AND ADVISED TO TAKE A WEEK OFF. EMPLOYEE RETURNED TO WORK ON 3-26-90 WITH A DOCTORS RELEASE.

LOADER OPERATOR WAS ASSISTING MECHANIC ON REMOVING CUTTING EDGE FROM HIS LOADER BUCKET, WHEN CUTTING EDGE FELL, STRIKING LOADER OPERATOR ON LEFT BIG TOE. LOADER OPERATOR WAS OUT OR HIS NORMAL WORK ACTIVITY WHEN ACCIDENT HAPPENED. THE PRELIMINART REPORT IS THAT THE LEFT BIG TOE WAS BROKEN IN TWO AREAS.

EMPLOYEE WAS USING A PRY BAR TO DISLODGE BOULDERS FROM FEEDER PRY BAR SLIPPED STRIKING THUMB ON LEFT HAND.

EMP STRUCK HEAD OF BOLT WITH HAMER, CAUSING BOLT OR HAMMER TO CHIP METAL FRAGMENT SRUCK EMP IN R. EYE. EMP WA> NOT WEARING HIS CO. ISSUED SAFETY GLASSES AT TIME OF ACC.

OPERATOR WAS JARRED WHILE OPERATING TRACK DOZER.

EMPL HAMMERING 2N STEEL WEDGES WHEN PIECE OF WEDGE BROKE OFF STRIKING HIM IN LEFT EYE.

DRIVING WELDERS TRUCK HIT PILE OF DIRT AND STEERING WHEEL JERKED TO LEFT AND PULLED MUSCLE IN RIGHT SHOULDER OF DRIVER.
