On 9/19/17 employee sustained 1st and 2nd degree burns to both hands and right wrist and was placed on restricted duty which was accommodated. The employee was involved in an electrical arc flash incident.

On 5/30/14 the employee requested medical treatment, was taken to the company clinic and was placed on restricted duty which is being accommodated. During a repair, the employee stepped off a structure and twisted his left knee. Employee was diagnosed with a left knee strain.

The employee went to the company clinic on 12/2/08 and was provided a lumbar support as well as returned to full duty. While driving the VMC haul truck in the plant pit, he hit a bump, causing the air ride seat to bottom out, resulting in a lumbosacral spine strain.
The employee went to the company clinic on 8/2/08 and was diagnosed with a R-hand accessory digit nail plate avulsion and fracture. While the employee repositioned forks on a forklift, a fork moved downward and crushed his second R-thumb accessory digit. The employee was returned to full duty.
The employee went to the Company clinic on 5/28/08 and received ten stitches for a left hand laceration. The employee was placing the radiator of a Peterbuilt yard truck when the spring from the hood came off and hit the employee on the left hand. The employee was released to regular work without restrictions.
The employee went to company clinic on 5/21/08 and received steri strips to keep wound closed. The employee attempted to re-loop drag cable but the cable came out and struck the right side of his face resulting in a right cheek laceration. The employee was returned to full duty on 5/21/08 by company physician.

The employee went to see the company doctor on 4/4/2006 and was placed on restricted duty. While cutting rubber with a utility knife, the knife slipped causing a laceration to R-hand, which required stitches.
The employee went to see the company doctor on 4/4/2006 and was placed on restricted duty. While driving the yard truck at the plant, the steering wheel moved suddenly causing the truck bed to tip over, resulting in a fracture to the employees R-pinky finger due to grabbing the steering wheel to regain control of the vehicle.
The employee went to see the company doctor on 3/7/06 and was placed on restricted duty. While cleaning area with the backhoe, the employee lost footing on loose gravel, slipped and fell, causing a sprain to R-shoulder.
On 2/21/06 the employee went on a personal leave and had surgery around 2/25/2006. On 3/15/2007 Vulcan Materials Company received the DWC Form from the employee informing us it was a work related injury. The employee claimed he injured lower back due to the bouncing of the truck while driving (ALLEGED).
