Employee alleges employee was climbing down from a loader and employee's footing got caught on the catwalk and employee twisted right ankle.

Employee alleges they were moving cable wires off of a truck and they heard their left shoulder pop.

Employee alleges the day before they were rolling some belting around but they didn't feel any pain until the next day employee wasn't sure if they injured themselves at work. Later after seeking medical attention they stated they did hurt their lower back at work. This is late reporting do to just receiving Dr. notes.

Employee alleges employee was removing a drive belt guard on a screen and it popped out and struck employee in right arm and smashing employee's middle finger. Employee received stitches in arm. *Employee had surgery on Nov 14, 2022 and is losing time and is still off work.

Employee alleges they were walking along side of the log feed and they stepped on some rocks twisting left knee.

Employee alleges EE was taking a handrail off using a wrench and the wrench slip off a bolt hitting EE in the nose. Late Reporting due to the employee said EE's nose wasn't fractured. The Dr. notes came in on 09/03/21 and the notes stated EE had a nasal fracture.

Employee alleges was using a prybar to remove a large rock from the conveyor, As EE was using the prybar, the rock got caught and cut EE's rt. middle finger. Employee did seek medical attention but was released. EE went back for medical treatment again on 10/15/20. A procedure was performed on EE's finger and EE is losing time as of 10/15/20.

Employee alleges was hooking a chain to a loader bucket and as EE turned to walk away left foot slipped into some mud, wet clay and rocks hurting left knee. Employee just seek medical attention on 4-19-17 and has some restrictions. We can accommodate the restrictions. This employee isn't losing anytime.

EE alleges he was removing bolts from the wash screen with a driver guard and he turned to switch positions and he twisted his right knee.

Employee alleges another employee stepped on a trailer ball and slipped off and ran into him knocking him down. He hurt his left shoulder and his arm. Employee started losing time on 12/07/12. Late reporting due to just received Dr. notes on 01/10/2013.

Employee alleges he was walking and he slipped on some ice hurting his right wrist. We just received Dr. notes stating employee is loosing time as of 02/28/12 and is wearing a splint.

Employee alleges he was driving loader to the crusher to haul rock and the loader jerked causing him to twist his lower back. We were just notified on 11-08-11 that he was going to be receiving his first epidural steroid injection. Depending on how he feels he could receive more. The employee is not loosing any time.

Employee alleges he was placing blowbars in crusher and the rotar got stuck snapping the bar while his arm was still holding it causing a strain to his right elbow.

Employee alleges he was walking down a small incline and he slipped on some rocks and he caught himself with his left hand. Just receive Dr. notes and this is reportable because he is to wear a splint on his left wrist just for protection.

Employee alleges he was loosening bolts on a blowbar with a impact gun and the bolts came loose pinning his hand between the crusher and impact gun bruising his left hand. *Late reporting due to employee was just scheduled for surgery on 12-18-09 and will start loosing time then.

Employee alleges he was trying to roll logs in the washer; prybar slipped and cut his chin. He received 3 stitches.

EMPLOYEE ALLEGES SHE WAS TRIMMING BUSHES AND THE HEDGE CLIPPER SLIPPED AND CUT HER LEFT POINTER FINGER.

EE ALLEGES HE WAS LOWERING WIRE OVER HIGH WALL, THE WIRE FLEW OUT OF CONTROL & HIT HIS WRIST.

EE ALLEGES HE WAS PUTTING STRAPS ON A PIECE OF PIPE AND AS HE LIFTED ONE END OF THE PIPE HE FELT A POP IN HIS LT. SHOULDER.

EMPLOYEE ALLEGES HE WAS CUTTING A PIECE OF THE CONVEYOR BELT WITH A KNIFE AND IT SLIPPED AND CUT HIM ON HIS RIGHT LEG.

EE ALLEGES HE WAS PUSHING AND PULLING TRYING TO ALIGN THE CONVEYOR UP AND HE STARTED FEELING PAIN IN HIS BACK AND RIGHT LEG.

EE ALLEGES SHE BENT OVER TO PICK UP A BOX OF SCALE TICKETS OUT OF THE BOTTOM CABINET AND SHE HEARD HER BACK POPP AND SHE COULDN'T STAND UP.

EE ALLEGES HE WAS PULLING DOWN BINDER TO TIGHTEN CHAINS ON LOAD, BINDER DID NOT SNAP SHUT, IT SPRUNG OPEN AND CUT HIM ABOVE RIGHT EYE.

EE WAS STEPPING DOWN OFF OF MACHINE WHEN HE SLIPPED AND HIT HIS ELBOW.

EE HAD JUST PARKED THE MICH 320 LOADER BY THE BULK FUEL TANK AND HAD JUST CLIMBED DOWN THE LOADER ONTO THE FUELD TANK BERM WHEN HE LOST HIS FOOTING AND SLIPPED DOWN.

EMPLOYEE BENTO OVER & WAS REMOVING A DRILL HOLE PLUG WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.

EMPLOYEE BENT OVER TO PICK UP A LOADER BUCKET EDGE PROTECTOR AND AS HE DID HIS BACK PULLED AND HE FELT A SHARP PAIN IN THE LOWER BACK.

EMPLOYEE WAS REMOVING A 25 TON (15O LBS) JACK FROM UNDER CRUSHER

WHILE MEASURING DRILL STEEL WITH A TAPE MEASURE IT GOT CAUGHT EMPLOYEE PULLED ON IT & IN DOING SO DETACHED A MUSCLE IN HIS ARM

EMPLOYEE GOT OFF ENDLOADER TO UNPLUG PRIMARY CRUSHER. AS HE WAS GETTING OFF, HE FELT PAIN IN BACK.

EE MOVED WIRE ON STARTER SOLENOID, CAUSING A SPARK WHICH IGNITED UNOTICED GASOLINE FUMES FROM PNPONY MOTOR.

DRIVING FUEL TRUCK PICKUP COMING UP HILL THE TRUCK BEGAN TO JERK AND DIE CAUSING BACK PAIN

EMPL STEPPED INTO HOLE AND CAUGHT HIMSELF ON GUARD RAIL AND CUT ARM REQ STITCHES.

EWHILE WALKING DOWN RAMP TO SUMP AREA, EMPLYEE'S FOOTING GAVE WAY AND HE FELL BACKWARDS; AS HE TRIED TO CATCH HIMSELF HE TORE TENDON IN HIS SHOULDER.

EMPLOYEE WAS LIFTING WOODEN BLOCK AND DRILL STEEL AND HAD TO SET TOWER OF THE DRILL ON WOOD RAILROAD TIE WHILE TRYING TO RETRIEVE THE RAILROAD TIE HE SPRAINED OR TWISTED HIS BACK
