Employee alleges the day before they were rolling some belting around but they didn't feel any pain until the next day employee wasn't sure if they injured themselves at work. Later after seeking medical attention they stated they did hurt their lower back at work. This is late reporting do to just receiving Dr. notes.
Employee alleges employee was removing a drive belt guard on a screen and it popped out and struck employee in right arm and smashing employee's middle finger. Employee received stitches in arm. *Employee had surgery on Nov 14, 2022 and is losing time and is still off work.

Employee alleges EE was taking a handrail off using a wrench and the wrench slip off a bolt hitting EE in the nose. Late Reporting due to the employee said EE's nose wasn't fractured. The Dr. notes came in on 09/03/21 and the notes stated EE had a nasal fracture.

Employee alleges was using a prybar to remove a large rock from the conveyor, As EE was using the prybar, the rock got caught and cut EE's rt. middle finger. Employee did seek medical attention but was released. EE went back for medical treatment again on 10/15/20. A procedure was performed on EE's finger and EE is losing time as of 10/15/20.

Employee just seek medical attention on 4-19-17 and has some restrictions. We can accommodate the restrictions. This employee isn't losing anytime.

We were just notified on 11-08-11 that he was going to be receiving his first epidural steroid injection. Depending on how he feels he could receive more. The employee is not loosing any time.
Employee alleges he was placing blowbars in crusher and the rotar got stuck snapping the bar while his arm was still holding it causing a strain to his right elbow.
