Miner was appx. 30' away from a capacitor that failed resulting in an ARC flash. Miner denied any injury and refused medical evaluation. On July 8, 2025 miner reported ringing in left ear and shortness of breath from event that occurred on June 12, 2025 and was sent for eval. At no time was a workplace injury reported until July 8,2025. Injury is under compensability investigation.

EE was washing out aggregate bins in screen plant, plugged in a work light and was electrocuted by the 120V outlet. EE was soaked from using the hose to wash out and was not properly grounded when using the plug. Non-GFCI outlet and extension cord were in use.

At about 11:25pm a 2nd shift employee was welding on an arm guard to the primary crusher when EE felt an electric shock in elbow. EE asked the fire watch to turn off the welding machine and found that the leads had worn through grounding to the primary head causing the arc. The employee was brought to the hospital as a precaution and was given prescription medication.

Bearing heater wiring caused short to metal table employee was using to install a bearing. A spark was noted at first use & Ash Grove was notified to repair unit. Unit was repaired by Ash Grove. Job was completed. At cleanup, employee touched table with gloved hand & received shock. Employee was taken to E.R. by EMTs.

Lightening struck conveyor that employee where employee was working causing employee to receive electric shock.

Employee was working on replacing a twenty amp circuit breaker. Employee de-energized panel by turning 100 amp main circuit breaker off. Leads on primary side of main circuit breaker shorted and caused flash.

The person injured was firing a blast, when the connecting wire used to hook in the shot was caught by the wind and hurled into high tension power lines, resulting in electric shock.

EE OBSERVED AN ELECTRICAL TRACKING ON THE LINE SIDE OF THE DISCONNECT, TRIED TO CLEAR BY TAPPING ON IT WITH A SCREWDRIVER HANDLE & IT EXPLODED.

ELECTRIC LACERATION RIGHT HAND RING FINGER.

EMPLOYEE WAS TRYING TO REVERSE THE DISCHARGE CONVEYOR TO GET A ROLLER OUT OF THE TAIL PULLEY. HE PULLED THE BREAKER AND LOCKED IT OUT. HE THEN STUCK HIS SCREWDRIVER INTO THE BOX AND TOUCHED THE ENERGIZER MECHANISM. THE WIRES ON THE TOP OF THE BREAKER WERE STILL HOT BECAUSE THE MAIN WAS NOT PULLED.

EE WAS ATTEMPTING TO CHANGE A FUSE BOX FOR #16 CONVEYOR. EE WENT TO INSERT FUSE. FUSE APPARENTLY CROSS-LEGGED, SHORTING OUT THE FUSE BOX AND CAUSING AN ELECTRICAL FLASH

EMPLOYEE WAS ATTEMPTING TO REPAIR A CIRCUIT BREAKER ON A TRANSFORMER. WHEN HE RECONNECTED THE ELECTRICITY TO THE TRANSFORMER IT BLEW CASUING A BRIGHT FLASH OF LIGHT. EMPLOYEE SUFFERED A MILD B URN TO HIS EYES SIMILAR TO THAT OF A WELDERS BURN.

CHECKING FUSES IN ELECTRICAL BOX. TOUCHED SIDE OF BOX AND GOT ELECTRICAL SHOCK. DID NOT REALIZE EXTENT OF BURN UNTIL OVER WEEKEND AND WHEN WENT TO DOCTORS.

HE WAS TESTING A MOTORSTARTER CONTROL WITH A VOLTAGE METER TESTER. HE CHECKED 2 LIVES & UPON CHECKING THE THIRD, AN ARC EXCAPED & FLASH BURNED HIS FACE, ARMS, CHEST & EYES. THIS MAN HAS 12 YRS EXPERIENCE AS AN ELECTRICIAN, LICENSED & A UNIONMEMBER.

WELDING IN TIGHT QUARTERS, EXTREMELY HOT, SOMEHOW CAUSED ELECTRICAL SHOCK WHICH RESULTED IN EXIT WOUND IN WRIST.

WHILE ADJUSTING THE PLANT FEED AT THE SYNTRON CONTROL BOX THE WORKER CAME IN CONTACT WITH THE REOSTAT INSIDE THE BOX

EMPL MOVING SWITCH ON A 60 AMP FUSED DISCONNECT TO THE OFF POSITION -SWITCH FAILED AND SHORTED BLOWING DOOR OPEN AND BURNING EMPL ON FACE,NECK,AND RIGHT ARM.

THE EMPLOYEE HAD CHANGED A FUSE IN AN ELECTRICAL BOX. HE CLOSED THE DOOR AND THREW THE SWITCH AT WHICH TIME THE BREAKER SHORTED. FIRE ESCAPED PAST. THE EDGES OF THE DOOR BURNING BOTH ARMS AND FACE OF THE EMPLOYEE.

EMPLOYEE WAS TURNING ON SWITCH TO START CRUSHER MOTOR. STARTER EXPLODED BURNING HIS HAND & ARMS. FIRST, SECOND & THIRD DEGREE BURNS.
