While adjusting a wire mesh panel, a piece of top rock fell and broke through the previously installed and next panel of mesh and hit the employee's left hand/finger.

Employee was removing rock from the top of the continuous miner when a rock fell from the roof striking the employee on the right arm.

Employee was in the process of positioning the drill station to drill a corner bolt on the outside of the entry when a rock/kettle bottom fell from the roof, sliding down the roof screen and struck EE in EE's back. The strike to EE's back pushed EE forward causing EE to hit mouth against the canopy.

While re-bolting a steep area where the screen was bagged, the employee grabbed the pan rack with right hand to help climb onto bolting platform. At that moment, a rock fell through screen striking the right middle finger about half way up the fingernail, resulting in the fingertip being amputated, and a tuft fracture of the distal phalanx.

Employee was pulling pallets of dust off of a supply trailer using a ratchet strap that was connected to a roof bolt plate by a 1/4 inch clevis. Employee was unhooking the strap when they were struck by a piece of loose rock that fell from the roof.

Employee was installing the bottom rib bolt when a piece of rock fell from the roof striking EE on the left leg right above the knee causing a laceration.

While the employee went to add their second piece of steel together to finish drilling the hole, a piece of loose roof fell and hit EE on the arm.

The employee had just completed pre-op and was handing it to foreman when a piece of rock measuring 3'x4' and from 10' to 2 ½" thick fell from in-between a row of roof bolts. This rock struck the EE on the head pushing EE downward into the walkway of the roof bolt machine. EE's right hand was caught between the rock and the frame of the machine.

Employee was working on a broken face chain at #4 shield on the Longwall. A piece of rock approx 11'x3'x2.5' thick fell between the face and shield tips. The rock fell straight down and rolled into the pan line, pinning employee against the rack bar. Incident resulted in the employee having two broken femurs and lower leg break.

The employee was moving the MRS back to set breaker timbers, when a portion of roof rock fell out from between roof bolts, striking employee on the right ankle and foot.

While positioned at the drilling station of the roof bolter, the employee was going to walk toward the back of the machine. As EE placed hand on the bolter tray to assist with stepping up into the bolter walkway, a rock fell from the roof pinching EE's hand between the rock and the tray. This resulted in a laceration to the left forefinger and two fractures to the left hand.

While roof bolting, EE's drill steel became clogged. As EE reached to remove the steel, shale fell from the roof hitting EE's hand and driving EE's thumb into the drill pot. The impact caused a contusion to EE's right thumbnail. After shift EE went to an urgent care facility where X-rays revealed a fracture to the tip of EE's thumb.

The employee was standing next to the continuous miner in the #4 entry. The employee had placed left hand on the machine's light guard as employee was reaching for a can of oil, when a piece of roof rock, fell from between roof bolts, striking employee's left hand.

Employee was operating a roof bolter and as employee pressurized the drill steel against the mine roof to begin drilling a hole a piece of rock fell and hit the employee on the left hand resulting in a laceration and fracture to the left ring finger and an amputation to the left middle finger at the first joint.

While roof bolting a piece of rock fell from the roof striking the employee on the left forearm. The employee was transported to hospital Philippi, WV where employee was examined and treated for a laceration to the left forearm requiring five sutures. The employee was released to work with no restrictions.

EE was operating a radio remote controlled continuous miner when EE was struck on the top of the head by a piece of roof rock measuring approximately 65 inches long by 21 inches wide by 1 inch thick. Resulted in a fracture to EE's C4 vertebrae.

The injured was struck on the head and shoulders by a piece of roof rock that fell out between roof bolts. The rock measured 4ft. 9 inches long, by 2 feet wide and was up to 8 inches thick.

While roof bolting, reached to remove a plugged drill steel from the roof. As they reached, shale and rock fell around the steel, knocking the injured to the ground. The rock and drill steel struck their right calf and left a laceration that required sutures to close.

While standing beside EE's ride, a piece of rock fell and hit EE on the neck/shoulder. Employee worked until 10/22/19 when EE began missing work. This was recently diagnosed as a degenerative disk disease.

While roof bolting and removing a steel from the roof, employee was lining the steel up with the drill pot when a piece of shale struck employee's hand mashing it against the pot. The impact caused a fracture and laceration to the right index finger.

While preparing to start the roof bolting machine, was struck on the head and left cheek by falling shale. The shale, approximately the size of a dinner platter, struck the edge of the canopy as well, allowing the sharp material to bounce into the operator's left cheek and leaving a laceration that required sutures to close. EE was treated and released to unrestricted duty.

Employee opened a door in order to move supplies with a forklift. When employee was getting ready to close the door, a rock fell out from the roof and hitting employee on the head causing an injury.

While roof bolting in old works, a piece of shale fell and struck employee on the head. Employee felt a sharp pain described as a "Stinger" in the neck and shoulder. Employee did not seek medical attention until the following day. Initially reported as a first aid only on 10/2/2017, became a lost time after surgery to repair herniated disks 2/12/2018.

Employee was installing a spot roof bolt. While drilling the hole, a piece of rock fell around the drill steel, striking the employee's forehead and causing a skull fracture and laceration

A piece of draw rock fell striking employee on the back of the legs and ankles causing a fracture to left heel.

While moving cut ends from timbers, the employee picked up a timber end and a piece of draw rock fell striking EE on the right forearm and wrist causing a laceration on the arm requiring sutures.

Continuous miner operator was struck by a rock that fell from the roof that broke the operator's right fibula.

While operating roof bolter employee was struck by a glancing blow from a piece of drawrock. Employee finished regular shift, continued to work and did not go to the doctor until 6-14-17. Employee was released full duty. During a follow-up visit on 7-24-17 employee was taken off work pending an MRI. It is at this time the incident became a reportable injury.

While worker was repairing a water valve, a piece of draw rock fell striking EE on the right shoulder and head. EE continued working and began missing work on 3/27/2017 when taken off work by a physician.

EE was operating shearer when a piece of coal fell into the bretby trough. EE stopped the shearer from tramming & removed the piece of coal. When returning the shearer, another piece of coal fell from longwall face, striking EE, pushing EE into the shield base lift cylinder. Reportable medical treatment was not provided until March 8, 2017. Became lost time 4/26/17.

The employee states that after swinging in from installing outside bolt, ee set stab jack to begin drilling the inside bolt. A piece of rock measuring; 54" long, by 24" wide, and 1 1/2", to 4 1/2" thick fell and struck leg while on knees drilling.

While moving the roof bolter, a piece of roof rock measuring 13" x 15" by 2" in thickness fell striking employee's hard hat causing the hat to slide down employee's face. Employee sought medical attention on a later date and began missing work on 2/9/2016 when employee had surgery on their neck.

While employee was standing on the off-side of roof bolter #13, employee was struck in the right side of head and face by a rock that fell from the top which was 9' high. Injured went up to the bolter to ask the bolt crew if they needed any supplies when the rock fell from the top. The rock measured 42"long x 18"wide x 3".

After the employee drilled part of the hole, EE put the two pieces of steel together to insert back in the hole to complete the hole depth. When EE reached out to push the steel into the hole a piece of rock measuring 23" long, 10" to 1" in wide, and 1 1/2" thick fell and hit ankle.

While employee was hanging the miner cable a piece of draw rock fell striking right hand which was still on the cable. The employee received a laceration on right middle finger which required sutures. EE was wearing gloves at the time of the incident and returned to work next scheduled shift.

While employee was setting bits on the longwall shearer, a rock approximately 4" thick x 3' wide x 7' long rolled out of the top striking the employee.

Operator was standing between the second and third row of pins when shale fell and bounced off the miner and slid, striking the operator in the right shin and foot. EE did not seek medical treatment. Swelling and pain prompted EE to seek treatment June 22, 2016 when X-rays revealed fractures to right foot. EE returned to work with a sedentary work only restriction.

While roof bolting shale fell from the roof and struck EE on top the head driving hard hat into EE's glasses and causing lacerations to nose and forehead. 6/13/2016 Orthopedic Specialist diagnosed 2 herniated discs allegedly associated with this occurrence that were previously determined to be degenerative in nature as related to this incident.

While roof bolting and drilling to install outside pin the operator's steel plugged. After clearing the steel and resuming drilling ee was struck in the face by a piece of falling shale leaving a laceration between lip and nose that required sutures to close. Treated and released to unrestricted duty.

While roof bolting the injured was drilling a hole into the mine roof when a portion of roof broke around the drill steel. The rotation of the drill caused the rock to spin around the steel. EE stopped the rotation and the rock broke and a portion of it struck EE on the forehead above the left eye. The rock caused a laceration and small portions had to be surgically removed.

The employee was hit in arm by a piece of rock while drilling the hole for a roof bolt.

While roof bolting and having already installed the first two bolts of a row, had drilled the inside hole, inserted the glue and turned to get a bolt when the operator was struck by shale on the back of his left leg. His leg was splinted and he was transported to the surface. Diagnosis revealed fractures to the left Fibula and Tibia that will require surgery to correct.

While roof bolting and adding a 27" extension was struck on the left hand by falling shale leaving a fracture to the left little finger and a laceration to the left index finger that required sutures to close. He was treated and returned to work with a "no use of left hand" restriction.

He was installing a roof bolt when a piece of rock approximately 22'L X 14'W X 2' thick fell striking him on the top and side of the right foot.

After the ATRS had been set worker was walking up the drill station when a piece of rock measuring 48" by 26" by 1 1/2" fell and slid down the T-bar and struck him on the head and back. He suffered a laceration on his head that was 2 centimeters in length that required three staples.

Employee was bolting the face of the #2 entry on the #3 section and when he put his drill steel against the roof a piece of rock came loose and hit the ATRS which deflected the rock toward him striking him on his right arm above the elbow resulting in a laceration.

The injured was roof bolting when he was struck on the left hand by head coal causing a laceration to the little finger of this hand requiring 3 sutures to close. He was wearing protective metacarpal gloves at the time.

While bolting was struck by a roof bolt. The bolt was assembled and laid across his drill mast. While inserting his wrench a piece of shale struck the end of the bolt causing it to flip and strike his safety glasses. Two parallel lacerations in and below the left eyebrow required sutures to close. He was released to unrestricted duty reporting to work his next scheduled shift.
