Dozer trap and the grizly at end of its conveyor belt were plugged with boulders 1". While he was removing by hand, they release boulders at chute hit his right hand (finger point 2,3,4) because his fingers were trapped between boulders. Fingers points were bleeding and broken. Surgery was done 7/27/11

Strained Back, Initially viewed as a re-injury of 12/6/10 by Gallager Bassett. Employee now hired legal representation and Third Party Gallager Bassett advised new claim to be opened due to ch enge in legal status. Our first knowledge was on 05/02/11. Thus this filing. MSHA Notified by phone of this and advised this statement be entered.

Miner strained his back while shoveling in the #1 tunnel.

A screen cover (wire cloth) fell into the "Jaw Rock" chute. The victim was removeing the screen cover and a rock approx. 4" x 8" came traveling down the chute and pinched his fingers btwn. the rock and chute wall. He suffered lacerations on his L/H middle and ring fingers. The middle finger was broken behind the fingernail.

Employee was pulling wire inside a bucket and two fingers became numb being in a cramped position for an extended period of time.

Try to move a 12"x12"x6" Rock. The rock slipped and pinch the tip of right hand ring finger and fractrue the tip of finger

Laborer was shoveling material into loader bucket. Material slid off and employee was pinched between bucket and pile. Employee suffered bruising to both thighs. Employee was treated and released at Dr.'s Office. Given pain medication and modified duty.

As the employee was changing screens for the Extec plant he twisted the wrong way and strained his back.

Employee was pulling a piece of steel up under saw belt and hurt his back ? complained of severe pain.

EE was carrying 50lb bucket of agg build from under pit, when he experienced pain in his lower back.

While putting screens in plant screen #1 top deck he was pulling on screen and pulled a muscle in lower right hip.

While lifting a screen, the EE twisted the lower back.

He was lying on his back undoing bolts off of a plate on loader, as last bolt was removed, the plate fell on his right wrist. He received a hairline fracture on his right wrist.

Employee was changing a screen on a screen deck, the screen slipped and caught his left hand between the screen and a column. Laceration of fingers of left hand requiring stitches.

EE's were working together to install a bolt on the dewatering screen, when injured EE put his finger in the bolt hole to check the alignment. Another EE inserted bolt from the other side and struck it twice with a hammer, pinching injured EE's finger tip, causing him to loose the finger nail and fracturing the finger tip.

Twisted back working on #7 scale weight.

While employee was removing material buildup of the dump station and removing a rock from the catwalk, his hand slipped off the rock and was caught against the handrail.

Employee was helping another employee remove a seat from a front end loader. The injured employee was in a ackward position during the removal process and strained his back. Strained Back

While unplugging a feeder the employee struck his elbow against the feeder sustaining an unknown injury to the right elbow.

Employee was in a chute taking measurements and cut his right hand on a piece of sharp metal protruding inside the chute.

Employee was trying to put back a steel cover over a conveyor and the cover got stuck. While trying to push the cover down, he got his right thumb in the way; the cover dropped down and smashed his thumb. Employee fractured his right thumb and also received 15 stitches.

Employee was entering onto forklift in yard. He pulled a muscle in left knee area. Employee was diagnosed with a left knee/leg strain.

EMPLOYEE WAS HELPING SPLICE A BELT WHEN HE SPRAINED HIS RT. WRIST. EE WAS PULLING ON THE BELT WITH BOTH HANDS & JUST SPRAINED HIS WRIST.

Lower lumbar strain due to pulling out a buried guard.

Employee was climbing up onto his loader and when he lifted his leg he felt a pinch and some pain in his lower back.

EMPLOYEE WAS LIFTING A 55 GALLON DRUM WITH A CHAIN AND THE BARREL SHIFTED CAUSING SLACK IN THE CHAIN AND CAUGHT HIS MIDDLE FINGER ON HIS RIGHT HAND.

Employeee was carrying a shaft. As he turned around he smashed his finger between the shaft and a steel rack.

Due to repetative motion of getting in & out of equipment. It is a cumulative strain injury to the left knee

WHILE SHOP MECHANIC WAS WORKING ON MOTOR GRADER ALTERNATOR, HIS ELBOW BUMPED THE HOOD SAFETY LATCH BRINGING THE HOOD DOWN ON TOP OF HIS HEAD. THIS RESULTED IN ABOUT A ONE INCH SUPERFICIAL LACE RATION ON TOP OF HIS HEAD.

Inspection lid swung shut and bruised ee's back.

ee was helping in maintenance to remove a worn out pulley. ee tood the bolts out & when he went to push the pulley out, it slid down & smashed his left index finger.

EE HAD BEEN SETTING CONCRETE FORMS AND WAS LAYING CONCRETE IN THE FORMS WHEN HE FELT SOMETHING PULL ON LEFT LOWER BACK.

AFTER RUNNING THE SHIFT FOR SOME HOURS A TAIL PULLEY REQUIRED SOME SHOVELING/CLEANING. OPERATOR DECIDED TO CLEAN TAIL PULLEY WITH A SHOVEL WHEN WHILE ATTEMPTING, TWISTED HIS BACK AND STRAINED A LOWER BACK MUSCLE.

EE WAS REMOVING A TRANSMISSION FILTER HOUSING THAT WAS WEDGED IN POSITION. AS HE PULLED THE LOWER END TO GET IT LOOSE THE WEIGHT SHIFTED AT THE BOTTOM OF THE FILTER HOUSING CATCHING HIS RIGHT LITTLE FINGER BETWEEN HOUSING.

THE WASHPLANT OPERATOR WAS LOADING SCREENS ONTO THE MAN-LIFT. WHILE DOING SO ONE OF THE SCREENS STARTED TO FALL OFF. THE WASH-PLANT OPERATOR TRIED TO STOP THE SCREEN FROM FALLING. HE GRABBED T HE SCREEN, AND THE SCREEN JERKED AND TWISTED HIS BACK. THE OPERATOR SUSTAINED A SPRAIN TO HIS LOWER BACK.

EE STARTED SHOVELING DIRT IN OUR TUNNEL WITH COWORKER ABOUT 0745 AND ABOUT 10 MINUTES INTO SHOVLING HE HIT STEEL WITH HIS SHOVEL, SO HE REACKED DOWN TO PICK IT UP AND WHEN HE WAS TURNING TO PU T THE CHANNEL OFF TO THE SIDE OUT OF HIS WAY HE FELT SHARP PAIN IN HIS LOWER BACK ON HIS LEFT SIDE.

EE WAS SHOVELING MATERIAL OFF OF P-5 BELT AND FIGURES THAT HE PULLED SOME MUSCLES IN HIS BACK WHILE DOING THAT. HE DIDN'T FEEL ANY PAIN RIGHT AWAY, UNTIL FRIDAY (2-21-03) MORNING.
