During wear plate replacement; old plate slipped scraping employee's lower legs.

Employee was aligning a pump to insert bolts when he sustained an elbow fracture.

Employee sustained left wrist sprain while changing out drive belts to air blower motor.

Employee was installing overhead rigging when the sling slipped and struck the employee above the left eye. The employee received two stitches to close the wound. The employee was wearing appropriate PPE at the time of incident.

Employee was unloading RR car - when employee tried to open coal car door, it was stuck and the movement caused a pain between right shoulder and neck.

After installing and releasing finish mill access door employee went to raise the safety cage. When reaching for the hand crank winch, twisted his back resulting in a left lower lumbar strain.

Employee sustained a hernia while lifting a guard cover over the 945-14 screw.

Emp was closing a refractory door after clearing buildup. There was a bolt at the top of the door that had to be held up due to improper clearances in the frame work. While closing the door emp used his weight to close the door while holding up the bolt. The employee's thumb was in the pinch point. Thus smashing his thumb.

Employee sustained back strain while removing sprocket off of shaft.

Employee was trying to cut a .003 inch shim stock. He had the shim stock locked in a vise. He could not find a pair of shears but thought he could tear the shim stock by hand. He was not wearing gloves. As he tried to tear the material, his left hand slipped and lacerated the palm of his hand. He received stitches and was referred to a hand specialist.

Replacing return roller on a belt conveyor, roller slipped from carrying pin and smashed employees finger between the roller and the roller.

Pinched finger between a piece of steel and the man basket.

Employee was changing bags in a bag house. Dust had accumulated on his hard hat and on top of his goggles. When he removed his goggles to clear off the fog build-up, dust fell into his eyes. He flushed his eyes with water and went home. The next day he went to the doctor and was prescribed Tobrex and Pred-forte.

On 7/1/2013, while shoveling material under the finish mill belt, employee extended to lift a shovel of material and felt pull in lower back. He did not feel the injury was severe enough to go to the doctor and treated it at home. He felt it "tighten" up on 7/18 while picking up tree branches and visited the doctor on 7/22. He was placed on lifting restrictions and prescribed PT.

As the affected employee was climbing onto the 990 Loader he felt his right knee pop when he transitioned from the first step up to the second step. Employee felt pain and wanted to go to the doctor for evaluation. Doctor did an X-Ray which revealed fluid on the employee's knee.

Employee was lifting an electric motor when he strained his left shoulder.

Employee got right thumb pinched between flange on barge and cement line.

Employee was lifting scaffolding when he felt a pain in his back. He reported the incident but wanted to wait and see if it improved. It did not, he went to the doctor on 9/14/12. Prescribed Naproxen and Felxeril but not placed on any restrictions. On a follow up appointment on 9/21/12 the dr. ordered physical therapy and placed him on lifting/bending restrictions.

Employee accessed the manlift, high heat and humidity, his left hand slipped from top rail of manlift, when his hand caught on the second rail, he had pain in his left thumb. Employee reported to company however did not want medical attention. On 10/23/12 he was seen by Dr whom after an Xray determined his left thumb was dislocated.

Employee was replacing bolts on a hopper when dust fell out of the hopper and on to the employee. The force knocked his goggles off and dust entered his eyes. He was taken to the emergency room where he was prescribed Hydrocodone, Nevanac and Zymaxid and given a contact lens.

EE caught the tip of his left ring finger between the tailgate and grain lock on dump trailer. He was pulling tailgate away from trailer when his ring finger got caught on grain lock severing the tip of his finger.

Employee was on ladder changing light bulb when dust fell from above and got into his eye.

While vacuuming 952 tunnel, employee was lifting pipe in order to clear material, when he strained right shoulder.

Employee was removing a return roller, when it struck his middle finger on his right hand causing a laceration.

While draining oil from 1003 mill, a cover plate struck employee causing him to contact his lower lip and tooth with the framework. The result was a chipped tooth.

An employee was injured while operating a guzzler truck. He re-injured an existing wound (received while off duty) that resulted in modified duty.

Employee was assisting in offloading an I beam from a forklift when his left hand glove was caught between the fork of the forklift and the I beam causing a laceration to his left pointer finger. Employee received sutures. No restricted duty.

EE was pushing a wheelbarrow full of cement dust. Was wearing all PPE including safety glasses. Turned wheelbarrow over to dump dust into bobcat, dust came up and entered his left eye. Taken to first aid office where washed eye and placed eye drops into eye, same procedure at 2:30pm on 6/9/11. EE came into off with his eye swelled, red and almost shut, doc appt was made.

When closing one of the belly doors under the spray tower, employee closed the door and was attempting to latch the door when it swung open and smashed his right index finger between the latch strap and the floor grating.

Employee shoveling debris onto a belt. He says that he was in a constant swinging motion for a while. He claims to have not noticed any discomfort until he got home and was taking shower. He noticed a red bump in belly button and then he started to notice the tenderness and ache at naval area. Comp sent him to dr and was confirmed a hernia they are wanting to do surgery.

Employee sitting long time bent over causing pain in back. On 2/7/11 employee attended physician, placed on restricted duty ordered physical therapy.

Lifted large electric motor off pedestal. Planted feet next to pedestal, leaned into motor. Pushed on motor with legs and arms to swing motor away from padestal, to land motor on floor, for removal. While doing so felt pull in right hamstring muscle.

Employee lifting dust collector bags, felt pain in shoulder. Physician placed on restricted duty - no overhead lifting left shoulder.

EE was working on the VSI to remove the table and had the crane attached to the table. The table was broken loose with a hyd jack and when pressure was taken off the jack, the table went down on the shaft. As the crane applied pressure, the table jerked off the shaft and rotated catching EE's foot against an anvil in the VSI.

Employee was holding a CAM spacer when it slipped out of his hands. He was wearing leather gloves, the fine edge of the spacer went through his glove and the skin lacerating his left small finger - this required three sutures.

EE was removing a bearing from a piece of machine when it slipped and cut his left thumb.

Injured stated that he was unrolling a conveyor belt and felt a pain in his lower back. Injured did not recall doing anything to aggravate his back. Injured was diagnosed with lower lumbar sprain

Injured was working at additives tower and stamler additives feeder during this time period. Unknown what caused this injury, but it was related to overexertion during this period. Injury reported on 4/9/2010 but injured did not seek medical treatment until 4/12/2010. Diagnosed with Lumbar strain.

EEs were replacing supporting pipes for head protection inside the "kin riser". Debris came loose, struck EE on the head and shoulder, knocking his hard hat off and causing a contusion on his shoulder. Another piece then fell and struck his head, he was wearing a Tyvek suit hood and a head sock. He was treated and released from the hospital.

Employee lifting metal out of chute when felt pain in right shoulder. Shoulder began hurting again in July sent for MRI placed on restricted duty on 5/11/10.

EE was lifting a 5 to 10 trash can over into dumpster when he felt pain in his right shoulder radiating down to elbow. EE reported, however after continued pain went to the physician on 1/26/10 at which time he was placed on restricted duty for three weeks.

Employee was removing cribbing from under dozer and machine shifted. Cribbing struck right hand.

Employee was washing a beaker and did not know the lid had a break in it and the employee cut his right hand between the pinky and ring fingers.

Two emp. were disassembling bricking ring on kiln bricking platform. One emp. lifted height adjuster too high on one side. The resulting pressure on other side of ring broke a guide, causing ring to fall. Emp. near broken guide jumped off bricking platform to avoid falling ring. This caused a re-separating shoulder previously injured in non-work related incident.

Injured was moving some water pump hoses that had become twisted. The hoses were 8" diameter. When injured was moving these hoses he felt a pull in his groin area. Injured did not seek medical treatment until 9/29/09.

Employee was disassembling a sand blaster unit hose. The employee did not completly bleed off all air pressure and upon removal of the hose, the coupling came back, striking him in the chin. The employee received stitches to his chin.

Employees were putting sheets of tin on a building. Employee was handing a sheet of tin to another employee, who was unable to grip the tin and it slipped out of his hand striking injured employee in the left arm.

Injured was trying to retrieve an impact wrench from golf cart bed. In the bed was a broken shaft assembly that he had helped remove from reclaimer. While retrieving impact wrench, the shaft fell out of cart bed and the coupling end fell onto the floor and the other end fell on injured's left foot.
