Employee was using a razor knife to notch a hole in the skirt rubber for cone crusher #2 when he cut the tip of his left thumb. The injury required stitches.

The miner was utilizing a utility knife to notch out a piece of skirt board rubber. The miner was first using a dull blade. He changed the blade, began cutting towards his left hand while using his right. The knife jumped and cut his index finger and palm of his left hand needing 4 stitches in said index finger. Gloves were not being worn.

As employee was removing a wear plate from the bucket of the Komatsu WA900 loader with a sledge hammer a piece of metal chipped off and embedded into his left inner thigh.

EE was attempting to dislodge a large cobble plugging a chute using a pry bar. The pry bar slipped and EE fell back on platform. Bar struck him above his left eye brow causing an approx. 3.50 cm laceration. EE received stitches and was released back to full duty.

As the employee was trying to loosen rocks stuck in the crusher with a metal bar, the bar slipped causing a laceration and fracture to his right pinky when it was caught between the bar and the crusher.

EE was cutting a piece of wear rubber. He was cutting towards himself & cut right thigh. Required 3 stitches.

The employee cutting a rubber hose to install a new pump, the knife slipped from the hose and cut the employees left hand. The employee received a 4 CM laceration to the left hand requiring stiches.

Employee cleaning screen top deck chute and employee was using a long pry bar to remove material buildup when the bar slipped through his right hand pushing his hand into the chute. Causing a contusion to his right hand.

Employee was using hammer and punch to punch a hole in 55 gallon drum for a pump, causing injury to hand and wrist.

An altercation between employee and outside contractor. The contractor was removing scrap steel from site. Employee tried to stop them from removing steel that wasn't scrap. The contractor employee became agitated and charged our employee and struck employee on head with adjustable wrench.

The empoyee was cutting through a waterpump hose with a razor knife in a tunnel and cut his left thumb in the process. The employee went to the clinic and was given five stitches in his left thumb.

Employee was using a chain hoist at the crusher area to align two holes so he could put a pin through the holes. The end of chain came loose and hit him in the nose, causing a laceration.

While tightening a bolt on a take up pulley, the employee's wrench slipped and he fell back striking his left shoulder on the man basket cage.

WHILE CHANGING A SCREEN ON OUR PORTABLE PLANT, EE WAS SWINGING A HAMMER TO BREAK FREE THE OLD SCREEN. HE MISSED THE TARGET AREA AND HIT HIS LEFT THUMB. THE CONTRIBUTING CAUSE OF THIS INJURY WA S HIS HAND IN THE PATH OF THE IMPACT AREA.

EE WAS USING A HAMMER TO REMOVE FROZEN BOLT FROM RADIATOR MOUNT. HE WAS USING A SMALL HAMMER THAT WAS BEING USED AS A PUNCH WITH 2 LB SHOP HAMMER. A SMALL PIECE OF THE HAMMER CHIPPED OFF AND S TRUCK HIM IN HIS LOWER ABDOMEN. THE SKIN WAS PENETRATED AND THE METAL CAME TO REST APPROX 1" DEEP.

WHILE REPLACING A MOTOR IN #2 CRUSHER, EE WAS USING A PRY BAR TO MOVE MOTOR SO THAT THE CRANE COULD PICK IT UP. THE MOTOR SPUN, CAUSING THE PRY BAR TO STRIKE THE HANDRAIL, CATCHING EE'S HAND BETWEEN THE PRY BAR AND THE HANDRAIL.
