EE was cutting electrical tape off the wire connections while wires were in junction box. EE was using a ceramic knife and had on cut level 4 gloves. While cutting the second wire connection the knife blade went across EE's left palm causing a 2 inch laceration.

Use of sledgehammer to remove wear plates inside a mill. Employee missed the wedge, hit wear plate that a small metal piece broke off and punctured employee in right upper leg area.

The employee was tightening bolts on a compressor using a wrench when EE felt pain in EE's right shoulder.

EE was closing up a rail car by prying with a bar. While doing this, EE felt a sharp pain in EE's left side. EE then noticed belly button started to swell twice the size. It is bright red and hard to touch. EE does not have a lot of pain in this area. EE will finish the shift out on the locomotive.

The employee was poking lumps out of a discharge chute with a bar. As employee was poling the material the bar broke through the material, the employee pinched right middle finger between the bar and the edge of the chute.

Employee was to remove the rotary feeder. The rotary feeder is at ground level, so the employees were working from a seated or kneeling position. The crew used prybars to break the seal of the feeder. One side of the feeder came loose allowing the feeder to swing to the opposite side. As the last side broke free, the feeder fell (18") to the ground, landing on the employee's knee.

Employee was cutting cardboard on the floor with a utility knife in the maintenance garage. Their left hand was holding the cardboard, and right hand was cutting. The cardboard slid on the floor and the knife slipped lacerating their left wrist.

While stripping a ProfiBus wire with a safety utility knife, the blade slipped off the cable and sliced their left-hand thumb. Employee was taken to local emergency care and received 5 stitches in left thumb

The employee was tightening the flip on the Finish Mill 3 transport line when the tool EE was using slipped out of the knuckle of the flip causing EE's left finger to get pinched between the tool and transport line.

A maintenance employee was replacing the drive chain of a discharge elevator with another employee. They were joining a section of chain when the other employee struck the pin of the link and a small fragment of metal broke off and struck the maintenance employee in the forearm about 3-4 inches above the cuff of the glove embedding itself in the forearm.

The employee was working on rebuilding a kiln feed pump bearing housing in the mechanical shop and was swinging a 12 pound dead blow sledgehammer to seat the bearing in the housing and felt a pull in their stomach.

Miner was working on cleaning out a plugged air slide on the silo. After they finished cleaning the section of air slide, miner was attempting to put the port back on with a hammer. As they swung, they lost footing and struck a finger causing a break.

Employee was tossing a rake into a pond to clean algae which resulted in a muscle strain or possible tear. The employee is still awaiting MRI and results to have confirmation of type of injury. Employee has not been prescribed any medications but does have restrictions.

Leveling (pushing/pulling) cement for a mud slab pour with a come along rake in a shallow excavation. While moving backward and pulling rake full of cement, arm came into contact with excavation wall. Sudden stop of arm against wall caused a shoulder separation.

Employee was changing silo hammer line valves when EE experienced a sharp pain in EE's Left shoulder. Employee reported the incident but did not seek medical treatment at that time. Employee tried home self-care but the shoulder did not get better. Employee sought medical treatment and was referred to physical therapy, making it a reportable injury.

Injured employee was changing a 10" line blind valve. The change bar slipped out of the knuckle and struck IE over the right eye. IE lost consciousness momentarily. IE was taken for medical evaluation & returned to work with no complications or restrictions

While cutting gaskets for a heat exchanger, the utility knife blade slipped off the gasket, slicing through the glove and causing a laceration across the employee's left ring finger.

An employee was cleaning out the coal mill rotary feeder for #3 kiln. The employee was closing the door, holding it closed with EE's left hand and using EE's right hand to tighten the nut down to hold the door in place with a wrench. As the employee was tightening the bolt, EE felt discomfort in EE's shoulder.

Employee was sweeping material near the sample motor. As the employee stepped forward, the back of their head made contact with the sample port causing discomfort to the neck. Employee was wearing a hard hat and was able to continue their regular shift.

Right Lower Back Strain while swinging a sledge hammer.

A come-a-long was being used to attach a bucket to the structure to prevent movement when it shifted and the hook came off striking the employee in the hand. The impact caused a laceration requiring 5 stitches between the pinky and ring finger on the employee's right hand.

Bolt was stuck in the shell of mill, trying to remove stuck bolt holding a pin to remove while other EE was hitting with a hammer. Hammer missed the pin and hit EE on their right hand injuring EE's right ring finger, both EE were wearing proper PPE.

Employee was on knees on FM1 beating the hatch loose on the secondary end so we could drop and pull this hatch to spin balls out. As employee was swinging down, index finger on right hand was pinched between the sledge hammer handle and a liner bolt that was just to the north of the hatch.

Employee was using a come-a-long and pry bar to get a pin back into place and after pushing and pulling with the pry bar employee felt a pop in lower back.

Removing liner plates inside mill. While using a hammer to free one of the plates a piece of steel hit employee in the forehead. Employee received 4 stitches to the head.

Employee was removing a taper lock bushing from a drive sprocket. Employee was using a pry bar to remove the taper lock, when the bar slipped, striking the tip of EE's right ring finger between the bar and an I-beam. Employee suffered a fracture to finger tip. Employee taken to medical professional, finger was splinted & employee placed on restricted (one-handed duty)

Employee was un-choking an airslide. When material started to flow suddenly, cement got behind EE's glasses & in R eye. Eyes were flushed, & EE was taken to a medical professional, given a prescription antibiotic. EE worked next scheduled day on restricted duty (clean environment) & was released to full duty at follow-up appointment the next day. (1 day restricted)

Employee was cutting off a bolt on a hatch on the inlet chute of the raw mill. There were 2 bolts in the door holding it in place. The one bolt that was holding it from rotating did not have a nut on it. As the employee began the work, the bolt without the nut somehow moved allowing the door to swing down and impact employee's leg, cutting it.

employee was removing end cap bolts on a kiln trunnion w/ a 2-1/4" box end wrench. As EE tugged on wrench, it came off the bolt head, pinching employee's R hand between the wrench and a pipe. The wound was cared for onsite, employee taken to ER & received 10 stitches. RTW on restricted duty. IE was wearing heavy leather gloves, wrench & bolt head were in good condition.

Employee was using a prybar to lift the tail pulley on finish mill 4 when the prybar slipped and dropped roughly 6 inches causing employee to injure lower right part of back.

EE received laceration and bruise from jack resulting in NO LOST Days.

The Serviceman IV was changing the teeth on a surface mining machine and was re-installing a bushing when EE struck EE's left hand with a sledge hammer.

Employee was adjusting a jack bolt on #1 mill motor for alignment when the wrench slipped smashing right ring finger between the wrench and the floor. The contact caused a laceration to the finger

Employee was removing wrapping tape from the seat of new conference chairs. Employee gathered paper to cut and remove, and cut the end of left middle finger with scissors. Skin was lacerated on three sides of the tip, remaining intact on the side closest to thumb. Employee received 6 stitches to aid in healing of the lacerated skin.

The employee was in a awkward body position lying down to place a wrench on a bolt of a piston shaft and when EE got up EE felt a twinge in EE's back.

Employee was struck by a piece of metal that broke off of a hand tool while trying to line up a hole on a liner.

Injured employee was removing a 2" pipe plug from a line. The wrench slipped, and the employee fell backwards. EE threw EE's hand out, striking the backside of EE's left ring finger on the corner of an piece of angle iron. I.E. was seen by physician, treated for a broken finger and placed on restricted duty.

Employee was using a box cutter knife and placed it on a pallet of sand knee high with the blade exposed. The employee walked away and as ee walked back toward the pallet ee ran knee into the knife causing a laceration of the left knee.

The employee was using a scraper to clean a chute when EE struck something hard suddenly stopping EE's movement and causing pain to EE's shoulder.

The employee was using a rivot buster to clean up around a tail pulley when the employees left finger came into contact with the shaft as it was recoiling causing it to get pinched between the shaft and the collar.

Employees used shotgun to breakup material buildup in 3rd stage preheater tower. EE held the stock in right hand which became sore and blistered the next day. EE requested medical treatment & was given prescription 9/28, making incident reportable.

The injured miner was replacing a door on the side of a screw and was replacing the bar that held the door in place with a hammer when the miner hit left index finger causing a laceration.

Employee was removing a nut holding a crossbar for a ball mill hatch. When EE loosened the nut EE's finger got pinched between the bar and the nut. Employee was taken to physician, received 4 stitches and placed on restricted duty.(No forcible grip with right hand, no use of vibrating tools with right hand)

The employee was threading a bolt through a coupling when employee's right hand slipped off ratchet. Momentum caused right forearm to come into contact with the north coupling cutting right forearm just past glove.

Employee was breaking up material clogging a chute with a bar. Employee felt a pop and pain in employee's left shoulder. Employee denied medical care at the time, but requested to be seen on 1-23-2017.

Employee was working in the raw mill, hammering out mill liner bolts. Employee was driving out bolts with a sledge hammer and bolt ricocheted back and struck employee in the mouth. Laceration to lower lip required two stitches. Chip on upper left incisor tooth.

Employee was clearing the D-16 to D-11 chute from a flat rock that became lodged in the chute. Employee pulled the e-stop lanyard on D-11 and got on the 3 step platform from the south side of D-11. Employee used a bar to pry the rock from the chute. The rock came loose allowing the bar to drop suddenly and employee's pinky finger got between the bar and the metal skirting.

The employee was swinging a sledge hammer to drive in a wedge on a Symons crusher. The employee partially struck the pin and the sledge hammer continued around. Both hands were on the handle of the sledge hammer. EE right hand middle finger struck an adjacent support causing a laceration requiring sutures.

The employee was using a ratchet to remove a bolt while trying to uncouple a motor. The ratchet being used was a 1/4" drive with a 3/8" allen socket inserted. While using left arm/hand to loosen the bolt the ratchet slipped. When the wrench slipped the employee felt pain in left shoulder/neck area. The employee reported continued pain later and was sent for evaluation.

Employee was cutting a piece of vacuum hose while holding it between legs. The box cutter EE was using cut the hose then the right leg.
