Employee was grinding sheet metal on bench grinder. Sheet metal kicked back horizontally and cut through glove on left hand. Employee suffered laceration to left palm requiring 9 sutures.

Employee operating power washer when he lost his balance. The water from the washer hit his left foot penetrated the boot and broke through the skin. Surgery was performed to clean out the wound. He was placed in boot and given work restrictions

Air lancing a bin out and a chunk fell and hit the lance causing him to hurt his arm

Employee was assisting in air lancing the coal chute. As he broke through the material in a downward motion, he jammed his left little finger and left ring finger against the top lip of the hood housing. This tore the two fingernails off an fractured the tip of his ring finger.

Employee was drilling for power shot placement in the chalk tank and felt his right shoulder pop and experienced pain. He went to the company doctor and had follow up visits. Employee communicated that his arm has continued to have irritation and pain. Employee has torn rotator cuff and had surgery on 03/04/2014.

On 6/29/11 while closing caps on the Kiln Trunion Rolls employee felt back pain. On 7-19-11 working on the four hole wear plate at the Roller Mill, the pain in his lower back became worse. ON 7/25/11, physician states that EE has damage to his SI joint, recommended restricted duty, physical therapy and medication.

While grinding sheet metal on the Bench Grinder located in Maintenance Shop - the Grinder caught the metal and pulled it in - pulling EE's left hand/thumb into the edge of the grinder - lacerating EE's left thumb

Employee touched his left knee to the side of the finish mill causing a burn. Visited Dr. with restricted duty and prescription. On 5/18/10 employee was admitted into hospital for infection.

Injury suffered a lifting injury to his left arm while attempting to service the sweeping machine he was operating. He attempted to raise and latch the rear door of the sweeper containment compartment in order to dislodge stuck swept material in the compartment.

EE using an air lance to blow material off of kiln Dizr, felt pain in lower back. Place on restricted duty no lifting over 20 pounds.

While ee was using air lance to clean out feed end/seal housing, finger was pinched between air lance and door frame. Left pinkie finger had to have 3 stitches.

Employee was in clinker tunnel jackhammering a large clinker chunk. When the jackhammer broke through the chunk, employee lost his balance causing him to slip and fall on his right leg and twisting his right ankle.

Employee was using a jackhammer and after use he noticed a pain in side and shoulder. Injured did not seek treatment until 10/6/09

EE was drilling holes in concrete for a great deal of the day with a Hiliti hammer drill. He claims he was in a constate bend or kneeling position. After lunch time he noticed back was feeling strained. Said he'd had back problems for 8 to 9 years and this felt different. There has been no lost time and has days restricted duty only.

Employee was utilizing a jack hammer to break up a cement pile, when a piece broke free and landed on the top of his right foot, just behind the steel toe in his boot.

EE was in a manlift attempting to tighten bolts on the roller mill. He felt a strain to his back.

Employee was removing bolts from floor grating in cooler with a cutting torch. The grating and bolts were in an overhead position and one of the "hot bolts" fell into the welding glove. The burn was about two inches below the left thumb on the wrist. The bolt was in a corner and access was difficult to make the cut. Movement was restricted and therefore reaction time was slower.

Miner was investigating why a dust collector's screw conveyor stopped. He turned off conveyor's power but didn't follow procedure to turn off/lockout the co-located rotary feeder's power. He then put his hand into the inspection port to see if there was any material buildup. The still moving rotary feeder struck the index & middle finger of his right hand, & lacerated them.

Employee was jack hammering out barrell bushings from the Kin feed pump. This took the better part of two days. - noticed hands/thumbs were weak and had trouble with his grip and pain in both hands. Note: Please change e-mail to sally.stinevilani@cemex.com (drop usa) thanks

Laceration to right hand while using a water lance to clean Kiln inlet.

Employee caught glove between sandpaper and shaft while sanding a shaft with a lathe, The sudden motion severed the tip of his right thumb.

Employee was attempting to open a dust collector damper while the fan was still operating. This heavy resistance caused employee to sprain his right shoulder.

Employee was using an air lance to blow cold cement down an air slide. Air slide was not blocked but had cold dust laying on the air slide cloth. A puff back occurred and cement dust went over the top of the employee's safety glasses.

EE was using high pressure washer and felt soreness/discomfort in right shoulder from recoil of spray wand, when applying the trigger. EE was sharing time with 2 other EE's. EE did not receive medical treatment immediately, is now receiving therapy.

A piece of refrafractory fell off a wall and hit employee while he was cleaning up debris. He was knocked into the wall and injured his right shoulder and was cut about his eye.

Employee was cleaning out a plug in the nextor hopper (massary additive) by standing on a platform and digging into it with a shovel. When ee returned to the lab, she compalin of back pain when she breathed. The dr prescribed pankillers and muscle relaxers for her muscle sprain and gave her a release to came to work.

The employee was using a water blaster to dislodge material from the Preheater. He later complained of pain in his right arm and shoulder.

While inspectingcooler, dust fell in right eye. Doctor removed particle from eye. EE returned to work.

Employee was using hand-power tool, drilling into wood; drilling bit unexpectedly became stuck into wood, then "freed-up", causing a twisting motion to hand/wrist area causing small bone fracture located on topside of right hand, near right ring finger.

Employee was grinding a pipe nipple when the pipe slipped and hand struck the grinding wheel, cutting left index finger.

While cutting a repair patch of metal, part dropped and bounce on the ground. Thus, bouncing into the leg of injured person.

David was clearing a bag jam at the flapper. This is at the transition of the packer discharge conveyor and the incline spring conveyor. He pulled the flapper up by grasping the lower edge and cut his left middle finger.

Broke Middle finger on right hand on welding mach.

Try to empty silo, opened port to remove lumps and material flushed out into face. Jack hammering lumps materal disoldged and flushed into face.

While jogging the blower on the 08-04 FK pump, dust flew into the employee left eye.

Employee using hand tool, small handheld grinder. He was grinding on part of a metal handrail. Grinding wheel got pinched/caught in between piping, then "popped" out of piping which then it knocked into the bottom of employees faceshield causing faceshield to partially flip up. The grinder hit employee on lip causing a slight laceration.

Employee was jack hammering concrete, jack hammer slipped and hit right foot and bent the steel toe boot down.

Employee was making repair to water pump housing on plant truck. Drill bit was too large and as he was changing bit, he accidentaly engaged drill. Employee's left thumb was cut and required stitches. Employee failed to disconnect power before changing bit.

The injured employee was working with the vacuum truck when he noticed a damaged section of hose. The employee broke off the damaged section while the truck was in operation and his left arm was pulled into the hose.

Employee was holding a bolt underneath a grate while another employee was using an impact wrench to screw the bolt in. The impact wrench started and caught the employees thumb lacerating his thumb.

The injured employee was assisting an employee that was welding. They were working in a fairly confined area. The injured employee received a flash burn to his left eye.

Poking buildup of pre-heater tower, poke port went positive blowing material into face and right eye of employee.

Employee was working on preheat tower with inspection door open. Hot air blew into employee's face, resulting in minor burns to face.

While cutting rusty metal with a cutting torch, a piece of slag flew into the employee's left eye.

Miner was affected by smoke while removing liners from Finish Mill. Miner was taken under his own power to the hospital where he was treated with fluids (I.V.)

The employee reported that during the previous week he had strained his back while packing cement bags. As he was packing, two bags began to pile up and he reached out and grabbed one of the bags.

Grinding on a hole in a shroud in the raw mill, he got dust or dirt in his right eye. Flushed eye with saline solution. Eye didn't improve in two days so he went to urgent care. Doctor applied prescription eye drops. Employee was wearing safety glasses and grinding shield.

Employee was giving signal to crane operator and operator stepped on boom pedal instead of brake causing boom to strike employee on front side of body. Employee continued to work full duty and asked to see doctor on 11/4/05 and was released with restrictions.

CLEANING PREHEATER WITH 12' WATER BLASTER. EE WAS BENT OVER WITH HEAD COCKED TO ONE SIDE WHEN HOT MATERIAL STRUCK FACE SHIELD AND KNOCKED IT OFF, ALONG WITH HIS HARD HAT. HOT DUST ENTERED LEFT EYE. EE WAS TREATED AT URGENT CARE, WITH ADDITIONAL CARE BY OPTHAMOLOGIST.

Steel erection. Employee was reaching around a beam to bolt up to the shear clip and strained shoulder and bicep.
