During preparation for dismantling of motor and gear box on slide gate. Employee didn't follow given instructions and removed all bolts from gearbox. That cause released of stored energy on slide gate and motor with gear box start spinning. Employee got caught by the equipment and started spinning with it. During spinning their head hit the handrail which caused a fatal injury.

Operator was backing up close to the edge at a berm and was facing the opposite way. Operator was pulled backwards by the counter weight, operator attempted to turn the machine to stop it from rolling into the ditch.
It rained and the mining pit needed to be dewatered. EE was at the mine site alone. It was reported that EE's spouse, was in the truck on the mine site while EE went into the pit. EE moved the telehandler into the pit, parked it on the bottom of the pit parallel on the ramp. EE exited the tele. While standing outside of the tele facing east, the tele fell over onto EE, crushing EE.
Employee was operating a DM45 Highwall Drill on a drill bench. Employee was traveling parallel to the edge and the outside edge gave way. The drill slid off the bench and landed 25 feet below.
Miner did not follow protocol. Therefore causing the death of the same.
Employee died when they became entangled in the log washer.
Employee was standing on 172 shield and advancing 173 shield towards the face. The employee was found with an injury to the back of the head at 171 shield.
Employee was operating an Excavator assisting Dozer operator in removing a big rock on the DC8A pit area. Excavator was between the rock and the top of highwall. Excavator went over the highwall ejecting the employee.
During a pipe breaking operation by the float crew on the #5 Matrix line near the #8 Lift Pump, the Leaderman was struck by a pipe and suffered fatal injuries. The investigation of the accident by MSHA and the company is ongoing. Autopsy results are also pending.
EE STARTED THE DAY AND HAD WORKED AT THE SHOP LOCATED IN LEHI, UTAH FOR THE MAJORITY OF EE'S WHOLE SHIFT. THERE WERE MATERIALS THAT NEEDED TO BE TAKEN TO OUR CREW TO CONTINUE WORKING. EE DROVE TO THE WORKSITE. WHILE EE WAS THERE, EE WAS WALKING AND WHILE WALKING EE WAS STRUCK BY THE FORKS OF A PEICE OF EQUIPMENT THAT FELL OVER, WHICH RESULTED IN EE'S LIFE ENDING.
Miner was moving a rinse bucket to access a piece of metal that needed to be repaired and welded. As EE pulled the pin from the rinse bucket somehow the hydraulic cylinder gave way pinning the miner in between the bucket and a handrail. Resulting in the death of the miner.

While attempting to lower feed box on wash plant employee was trapped between feed box and hand railing causing fatal injuries to employee.
While lifting a VSI lid a strap broke. The lid struck the victim causing severe head trauma.
EE's working to clean the mainframe arm on primary crusher. During the lift, the rigging assembly appears to have failed, striking the EE who was located in the line of fire.
Machine operator was operating a CAT D9T dozer clearing overburden material from a bench. Operator reversed the machine backwards an estimated 200-300 ft. at a high speed without looking back and without responding to honking from another machine, per witness testimony. Machine fell off an edge. Operator was pronounced dead at the scene. Machine is total loss.
While assisting in the offloading of a tank from the RGN trailer, the equipment being used to crane the tank struck JD's Custom Transport employee, causing EE's death.
An outside truck driver came onto this site to get loaded where the driver then parked their truck and was out of view from out scale house. Driver began to work on the back of the truck where the air bag blew out from under the truck and hit the driver in the head. Driver was not our employee, nor was the driver supposed to be working on their truck.
EE was working outside prep plant on disassembly of vibrating screen. Removed screen from prep plant was on ground, while foreman was checking area, sideboard of screen began to fall. EE tried to avoid injury by moving out of the way but was pinned by sideboard against smaller screen which had been removed from prep plant. EE suffered fatal injuries. Under investigation by MSHA.
The victim was mounting a haul truck tire on a rim when a rim locking ring dislodged and struck employee, causing fatal injuries.
An employee was operating a CAT 390F excavator in the South Quarry Pit. The machine went into the water and sank to a depth of approximately 25 feet. Consistent with 103K order modifications approved by MSHA, the operator was recovered the following day.
OUR EMPLOYEE WORKING AT MOUNTAIN AGGREGATES QUARRY MINE SITE WAS FATALLY INJURED WHILE OPERATING A DOZER ON AN ACCESS RD IN WHICH THE EQUIPMENT LEFT THE ACCESS RD AND ROLLED DOWN AN EMBANKMENT.
Employee was mobilizing dozer to work site for following day via haul road. Cause of accident unknown. Employee went over embankment and was found approx. halfway down mountain. Details unknown at this time. Employee found deceased. No eye witnesses.
A utility crew was assigned to complete rehab work at the 1600 haulage corridor, which entailed removing utility lines. A team of three employees were utilizing a scissor deck with a pipe handler to remove 10" steel pipe. They had removed one pipe section and were starting to disconnect the second pipe when they sustained injuries.
Employee was assisting repair/replace toggle plate and toggle plate bearings on the jaw crusher when they suffered a pinning type injury.
Employee was working with a 3 man crew dismantling a Caterpillar 390F Excavator. During dismantling, EE was impacted by the counterweight. The other 2 crew members did not witness the accident.
A Grove RT765E-2 crane was in use to install a motor into a Caterpillar 773B haul truck on level 3 in the quarry, when the auxiliary line broke, dropping the ball/hook onto a miner below, resulting in a fatal injury
Contractor suffered fatal injuries while performing maintenance on "Dutchman" of electric shovel bucket. Fatal injuries resulted from being pinned between shovel bucket door and bucket.
Employee was servicing a dozer alone with the machine running & it was somehow put in gear & started moving. Before the employee could get out of the way the machine trapped employee's foot & started pulling employee in between the track & the cab, which led to employee being run over by the machine. This is our best guess as there were no actual witnesses to the event.
The employee was near a screw conveyor on the 3rd floor of the finish mill 2 building. At some point their hand and arm became entangled in the screw conveyor. Emergency response was called immediately and the employee was transported to a hospital. The hospital later reported that the employee passed away.
Fatal injury to a temporary employee with Gillman Services, Inc was experienced when they were engulfed by a flow of lime kiln dust while operating a CAT 336 Excavator.
A contract miner died when the compactor EE was operating overturned, pinning EE beneath the cab. As the compactor was traveling in reverse, the compactor's left tire went off a four-foot embankment causing the compactor to overturn.
Dozer was supposed to be pushing into slot to push out to loader on top Bench, EE slid off top bench onto a shelf that was bermed off, EE then pushed a large rock over the edge and stopped. EE moved forward and slid off shelf blade first and landed on blade, EE rolled to the right and off shelf then flipped 3 times and landed on top of cab on the ground.
Employee was attempting to move the face drill and got pinched between the machine and the rib causing injury.
Injured was found in entry 11, caught between the left rib and the horn of the miner.
Utility operator was found non-responsive with EE's clothing entangled in the drill steel of the roof bolter.
Employee placed self on the bumper of the miner below the unsupported conveyor tail boom with the machine still energized. EE apparently was trying to trouble shoot issues with the stab jack by utilizing the remote control functions and in the process inadvertently lowered the tail onto self resulting in fatal crushing injuries.
An employee was fatally injured during maintenance work on the hopper. EE was operating a CAT forklift in close proximity to a Volvo excavator. EE exited the vehicle without notifying the other employees working in the area. Other employees stated they heard a "pop" and found the forklift operator unconscious next to the fork of the forklift.
A Millwright drove the pins out of a 200-ton crane boom to change out a section and the boom fell on employee.
A group solely comprised of RWS contractors [MSHA Contractor ID B5045] was working in the mine. A pump sitting on a trailer was moved by RWS contractors by a telehandler, without notification to Nyrstar. RWS hitched the trailer to their telehandler. RWS pulled the trailer up a ramp. The trailer hitch broke. An individual RWS EE traveling on foot behind the trailer was struck
Employee was trying to insert a spud pin into a spud before the spud stopped moving. Pin struck victim in the head.
Victim was clearing brush with the excavator near pond and the bank appears to have given away causing the excavator to fall into the water and become totally submerged. It is not known if victim drowned or was fatally injured in the accident. We are uncertain of the exact time of the accident. The accident occurred between 10 a.m. and 1 p.m.
A miner was fatally injured while using a tool to remove a down hole hammer. The drill motor turned unexpectedly, pinning the driller's leg between the tool and the drill mast.
Changing the rear tire on a 992D Loader. Jack kicked out and and loader fell down crushing employee.
D-10T Dozer was slot dozing un-shot muck to assist a loader in Phase W-A. After working parallel to the pit crest for approximately an hour, the dozer reversed abruptly to the right and backed over the pit edge berm. The dozer fell approximately 300' landing on a catch bench below.
Task was to remove screen flare. Employee was loosening bolts, changed work location, dangerously close to excavator, without warning or eye contact with excavator operator. Excavator rotated, bucket of excavator struck employee in the upper chest resulting in death.
