While electrician was traveling to shipping, gusty wind conditions caused a dust particle to get into eye. Flushed but did not relieve irritation so went to emergency care and prescribed a prescription ointment.

Workers were finishing up crusher inspections in plant when a worker was stung one to two times in upper right shoulder by hornet. Other workers in the area observed the sting. Worker stated EE was not allergic (that EE knew of) but after 10 minutes EE experienced anaphylaxis.
Windy conditions stirred up dust and entered employee left eye. Left eye irritated that required medicated eye drops.
On 6/11/2018 ee was performing mobile equipment inspection on the cat 262C skidsteer. EE knelt down to check the cab filter when Left knee popped and ee felt a slight pain. EE was seen by a physician who recommended ice and OTC meds for pain. On 9/4/2018 ee noticed pain in the same knee and was seen by a physician who ordered an MRI. Surgery occurred on 10/4/2018.
Snake bite while cutting to recycle conveyor
He was kneeling in the cab of the scaler working, and when he stood up he felt his knee pop followed by intense pain.
EE fainted while at work. He was transported to Columbus Community Hospital via ambulance & treated at the emergency room. The cause of fainting was determined unknown by the ER doctor. EE followed up with his regular doctor the next day & was prescribed anxiety medication. He has not been allowed back to work due to unknown side effects of prescribed medication.
Customer truck driver suffered some sort of health related issue while sitting in his truck with the doors closed. Driver was discovered by plant personnel and CPR was performed until professional medical personnel arrived. He passed away at the Hannibal Regional Hospital
Mine worker found an apparent dead body in pit area. Notified supervisor who notified 911 service and MSHA. Authorities determined deceased was a 17 year old male and not employed by Con Agg of MO, LLC. MSHA inspector completed the investigation for MSHA.
Employee twisted his knee while trying to access bolts to remove drive motor (awkward position). (Did not become reportable until 11/03/11 due to surgery)
