EE ALLEGES AN INJURY TO HIS BACK WHILE DRIVING A COAL TRUCK. HE STATED TO HIS SUPERVISOR THAT HE HURT HIS BACK WHEN HE HIT A HOLE IN ROADWAY. EXAMINATON FOUND NO HOLE IN THE ROADWAY. EE TOLD OFFICE PERSONNEL THAT HE WAS INJURED IN A MOTORCYCLE ACCIDENT AND THAT THE BOUNCING OF THE TRUCK HURT HIM BECAUSE HE WAS SO SORE. KANAWHA TRUCKING DISPUTES THE ALLEGED INCIDENT.
HE HAD REPAIRED A CABLE ON SHUTTLE CAR & RESTORED THE POWER. HE INSTRUCTED THE OPERATOR TO START THE PUMP MOTOR TO REEL UP THE CABLE, WHEN THE CAR STARTED, A LOOP OF THE CABLE PULLED HIS FEET FROM UNDER HIM, CAUSING HIM TO FALL.
Hit a bump while driving haul truck. Jarred neck and back and got shooting pains down right leg.
EE STATED THAT HE WAS OPERATING A DIESEL LO-TRAC, PLACING A MOTOR IN PLACE FOR A FUTURE BELT HEAD DRIVE INSTALLATION. HE GOT AGAINST THE COAL RIB WITH THE LO-TRAC, CAUSING SOME MATERIAL TO FALL FROM THE RIB. THE RIB ROLL STRUCK HIM ON HIS RIGHT LEG, CAUSING A FRACTURE TO THE TIBIA AND FIBULA IN HIS RIGHT LEG.
HE WAS OPERATING A MOBILE BRIDGE UNIT & WHILE BACK UP, HE RAN OVER SOMETHING IN THE ROAD CAUSING HIM TO BE JARRED. DRS REPORT ON 9/27/05
Haul truck ran out of fuel while going up a ramp. Operator applied brakes but truck rolled backwards hitting spoils and tipping over on right side. Operator was wearing his seat belt but got banged around in cab bruising his shoulder. He was not released to work on his next scheduled shift.
EMPLOYEE STATED THAT HE WAS UNDER A TRUCK ATTEMPTING TH CHECK AN AIR LINE LEAK. HE ASKED THE DRIVER TO START THE TRUCK TO CHECK THE LEAK. DRIVER THEN STARTED THE TRUCK IN GEAR AND IT DRIFTED PINNING EE RIGHT KNEE BETWEEN AIR TANK AND GROUND
Driver stated that the transmission jumped out of 3rd gear, he lost control. Truck turned over onto driver's side.
Driver had 2 previous verbal warnings concerning speeding. Turned over truck was found across tracks at bottom of "A" side lying on its driver's side. Transmission was in neutral, Jake turned off, engine still running, brakes still hot to touch, 1 hr 45 min later. Driver had attempted to shift gears going downhill & got the truck out of gear. Results, runaway truck.
He heard a noise at the #2 north belt drive and was trying to locate the source. The omega coupling located between the drive motor and the speed reducer came apart into four sections. A hard piece of plastic struck the employee on his left hand fracturing two bones in his hand.
EE FELT PAIN/SORENESS IN LUMBAR AREA OF BACK AT END OF SHIFT AFTER DRIVING TRUCK FOR TEN HOUR SHIFT
While attempting to turn off a main haulroad in a service truck, the truck began to slide and eventually turned onto its right side. The employee was thrown across the cab and sustained injuries to his right thigh and knee. He began to loose time from work on January 3, 2005
Lost control of truck, ran in ditch & turned over. Laceration to right ear. Broke fender, air filter, door & mirror & damaged corner of truck bed.
Employee's portal vehicle was struck by a deer while driving on mine property. The air bags deployed causing the driver to enter the ditch and into a sump along the roadway. Employee suffered a fracture to his left ring finger. * Medical records diagnosing and confirming the injury were not available until 12/10/04.
Employee was driving a Dresser water truck. He hit a pothole and experienced back pain the following day.
The window in the haul truck shattered and employee received glass in his eye.
EE WAS OPERATING A BATTERY-POWERED SCOOP. HE WAS SCOOPING THE #4 ENTRY WHEN THE SCOOP BUCKET DROPPED OFF OF A LEDGE APPROX. 10" DEEP, CAUSING EE TO BE JARRED IN THE DECK OF THE SCOOP. (EE CONTINUED TO WORK AND BEGAN MISSING ON 12/28/04.)
Employee was using an endloader to move a large rock from the dig face. A section of the rock broke loose, causing the endloader to jar the employee resulting in a strain to the neck and shoulder.
Employee turned to look rearward while backing his loader down a slight incline and strained his lower back.
DRIVER, WHILE ATTEMPTING TO SHIFT GEARS WHILE IN A SWITCHBACK AND LOADED, ROLLED DOWNHILL. LET ENGINE BRAKE KILL ENGINE, LOSING CONTROL AND TURNING TRUCK OVER ON PASSENGER SIDE.
WHILE HAULING COAL OVER ROUGH ROADS, EE STATED THAT HIS BACK AND NECK STARTED HURTING. EE COULDNOT RECALL ANY SUDDEN INCIDENT OR SINGLE EVENT.
EE WAS HOOKING UP AN AIR COMPRESSOR TO THE PINTLE HOOK ON THE BACK OF A SERVICE TRUCK WHILE THE TRUCK WAS BACKING UP TO THE COMPRESSOR. EE RIGHGINDEX FINGER WAS CAUGHT BETWEEN A SHACKLE ON THE HITCH OF THE AIR COMPRESSOR AND THE PINTLE HOOK.CAUSING LACERATIONS AND A FRACTURE TO THE FINGER
WHILE LEAVING PIT AREA LOADED ROCK TRUCK STOPPED NEAR TOP OF RAMP. EMP REFUSED ASSISTANCE OF 994 LOADER AND DECIDED TO BACK DOWN RAMP ON HIS OWN. AFTER TRUCK WAS PUT INTO REVERSE AND RETARDER APPLIED THE TRUCK FREE-WHEELED DOWN RAMP. WHEN BRAKES WERE APPLIED THE FRONT END OF THE TRUCK RAISED UP AND BACK DOWN TO GROUND.
EMPLOYEE STRUCK HIS RIGHT KNEE ON THE STEERING COLUMN, RESULTING IN A CONTUSION.
OPERATOR WAS TRANSPORTING A CATERPILLAR D-11 DOZER WITH TRACTOR AND LOWBOY TRAILER. AS THE OPERATOR STARTED UP A RAMP HE DOWNSHIFTED THE TRACTOR TO 1ST GEAR. THE FRONT TIRES OF THE TRACTOR CAM E OFF THE GROUND AND THE TRACTOR FLIPPED OVER BACKWARD. THE OPERATOR SUSTAINED A BUMP TO THE RIGHT SIDE OF HIS HEAD AND A LACERATION TO HIS RIGHT SHOULDER REQUIRING SUTURES.
EE STATED THAT HE WAS OPERATING DIESEL SCOOP ALONG 3 NORTH ROADWAY AND RAN OVER A ROCK OR A HOLEIN ROADWAY, CAUSING HIM TO BOUNCE AROUND IN DECK. HE STARTED MISSING WORK ON 4-28-03. HE WAS DIA GNOSED AS DISC DISEASE. WE QUESTION WHETHER AN INJURY OCCURRED.
EE WAS OPERATING A HAULAGE TRUCK & HE STARTED EXPERIENCING BACK PAIN. MRI HAS REVEALED 3 BULGING DISCS & THE DR HAS STATED THAT THIS IS A DIRECT RESULT OF DEGENERATIVE DISC DISEASE. THIS INCIDENT IS BEING CLASSIFIED AS OCCUPATIONAL ILLNESS
DRIVER OF VEHICLE DEPARTED THE PREP PLANT VIA ACCESS ROAD WHEN HE HIT A LARGE HOLE AND BOUNCED VIOLENTLY CAUSING HIM TO HIT THE ACCELERATOR. THE VEHICLE LURCHED FORWARD STRIKING ANOTHER HOLE T HROWING HIM AGAINST THE STEERING WHEEL AND HITTING HIS HEAD. HE WAS UNRESTRAIEND AT THE TIME.
PASSENGER IN VHICLE DEPARTING PREP PLANT OF ACCESS ROAD, BEHICLE HIT A LARGE HOLE IN ROAD CAUSING DRIVER TO BOUNCE & HIT ACCELERATOR WHEREUPON THE VEHICLE LURCHED FORWARD HITTING A SECOND HOLE . THE PASSENGER HIT HIS HEAD ON THE ROOF AND ON DASH. HE WAS UNRESTRAINED.
WHILE DRIVING THE FILLY PERSONNEL CARRIER DOWNT HE SLOPE ENTRANCE, HE LOST POWER ONT HE RIDE. THE FILLY HIT THE RIB, CAUSING HIM TO HIT HIS KNEE AGAINST THE FRAME, RESULTING INTO A FRACTURED K NEE CAP.
EE FELL FROM A 3' LADDER WHEN THE TRUCK HE WAS WORKING ON MOVED FORWARD.
EMPLOYEE WAS JARRED WHILE BEING LOADED AT THE SHOVEL. NOTE: EMPLOYEE DID NOT START MISSING WORK UNTIL 12-2-02.
TRUCK WAS DROVE INTO DITCH AFTER LEAVING SEVEN FEET OF SKID MARKS AND TRAVELING FORTY FT AFTER SKID WITH NO SIGN OF BRAKING INTO DITCH. TRUCK WAS FOUND IN NEUTRAL AND EE BRAKE TURNED OFF. NO DAMAGE WAS DONE TO TRUCK.
HE WAS TRAMMING A SCOOP LOADED WITH ROCK DUST & THE CENTER SECTION OF THE SCOOP BROKE, BOUNCING HIM INTO THE CANOPY.
DUE TO RATE OF SPEED, A LACK OF STOPPING POWER CAUSED EXCESSIVE SPEED BEYOND HANDLING CAPABILITIES THEREFORE RESULTING COAL TRUCK TURNING OVER INTO DITCHLINE & SLIDING ON ITS SIDE APPROX. 20 FT. ON ITS SIDE BEFORE COMING TO A STOP.
BANGED KNEE ON TRUCK LADDER.
THE TRUCK THE EE WAS OPERATING CAME INTO CONTACT WITH A SHOVEL BUCKET, RESULTING IN A JAR TO THE TRUCK OPERATOR. PRIOR TO THIS EE SEEKING MEDICAL ATTENTION SHE WAS ASSIGNED THE TASK OF DISPATC HER. THE DR. GAVE HER A RESTRICTED DUTY RETURN TO WORK, BUT HER REGULAR JOB FELL INTO THIS CATEGORY THEREFORE THIS REPORT IS FOR FILE ONLY.
EE WAS SHOVELING COAL ONTO THE BELT LINE WHEN HIS SAFETY GLOVE ON HIS RT. HAND WAS CAUGHT AND HIS THUMB WAS SMASHED BETWEEN THE ROLLER AND BELT LINE. THE EE HAD 3 STITCHES & LOSS OF THUMBNAIL.
EE WAS BEING TRAINED ON LOADER SHE WAS SITTING NEXT TO THE TRAINER. WHILE LOADING A TRUCK THE TRAINER BUMPED THE HOIST CONTROLLER JARRING THE MACHINE. THE EE HIT THE SIDE DOOR LATCH WITH HER L EFT SIDE. SHE FINISHED THE SHIFT AND WENT ON DAYOFF. ON 6-19-02 SHE WAS DIAGNOSED AS HAVING A FRACTURED RIB. SHE WAS GIVEN WORK RESTRICTIONS, WHICH TOOK EFFECT ON HER NEXT SHIFT ON 6-21-02.
LOADER BOOM HIT HAULER BED WHILE LOAIDNG HAULER. THIS CAUSED THE HAULER OPERATOR TO BE JERKED SIDEWAYS WHILE INT HE OPERATORS SEAT HE COMPLAINED OF BACK PAIN AND WAS SEEN BY A DR. DR SAID POSS IBLE PULLED MUSCLE AND RETURNED HIM TO WORK. ON JUNE 13TH HE RETURNED TO THE DR MRI BEEN OF WORK SINCE JUNE 14TH.
EE HIT A BUMP WHILE OPERATING A SCRAPER. SAID HE FELT HIS SHOULDER POP & BEGAN TO EXPERIENCE PAIN.
EE JAMMED WRIST AND THUMB DOWN SHIFTING TRUCK.
EE WAS ATTEMPTING TO RAISE A LARGE ROCK WITH THE ENDLOADER BUCKET. THE ROCK BROKE WHICH RESULTED IN A JARRING ACTION TO MACHINE. STARTED MISSING WORK 3/19/02.
EMPLOYEE WAS OPERATING AN ENDLOADER ALONG THE TOE OF THE HIGHWALL WHEN THE CORNER OF THE BUCKET CAUGHT ON A SOLID PIECE OF THE WALL, RESULTING IN A SUDDEN STOP.
HE OPERATED A LOADER ON 2-6-02, LOADER TIRE HIT A POTHOLE IN THE ROAD CAUSING HIS BACK TO BECOME MILDLY PAINFUL. HE OPERATED THE LOADER ON 2-8 AND 2-9. ON 2-9 HE WENT TO A LOCAL EMERBENCY ROOM WHERE HE WAS DIAGNOSED WITH A HERNIA OF A VERTEBRAE DISK.
HE WAS OPERATING A BATTERY POWERED SCOOP AND RAN OVER A WOOD BLOCK IN THE ROAD, CAUSING HIM TO BE BOUNCED INTO THE CANOPY.
EE WAS DUMPING A LOAD OF COAL ON A STOCKPILE & THE TAILGATE MALFUNCTIONED CAUSING THE TRUCKS FRONT END TO SUDDENLY RAISE & JUST AS SUDDENLY, DROPPED DOWN.
EMPLOYEE WAS PREPPING COAL NEAR QUIT TIME. AS HE WAS COMING DOWN OFF COAL RIB HE MISSED THE RAMPHE WAS JARRED AROUND IN THE LOADER ALLOWING THE RIGHT SIDE OF HIS HEAD TO STRIKE A SUPPORT INSID E THE CAB.
