Haul truck was struck by a shovel dipper door jostling the truck driver. The truck driver had pre-existing degenerative disc problems which were aggravated by this incident. She sought chiropractic treatment.

Employee was closing an elevator door and slammed her right little finger in the door. Employee received a minor crushing injury to finger and was treated at the local hospital. Employee quit the following day and refused to fill out an accident form. Actual treatment is unknown at this time.

Employee was swinging a sledge hammer to loosen a tooth on a dragline bucket. He changed his swing from right to left and his backed popped. He worked the rest of his rotation, 2 1/2 days and on 1/24/06, his days off, he saw a chiropractor and was manipulated.

While opening a drawer in a tool box mounted on the catwalk, employee stepped backward and put foot under guard rail. He fell against the handrail striking his back and ribs, fracturing a rib.

EMPLOYEE WAS INSTALLING A CUTTING EDGE ON A MOTOR GRADER WHEN THE END OF THE CUTTING EDGE SLIPPED FROM HIS HAND AND HIT HIS FOOT DIRECTLY BEHIND HIS STEEL TOE.

Occupational Illness - Hearing Loss - 1% permanent partial disability.

Carpal Tunnel Syndrome.

A CREW MEMBER WAS USING A RAILROAD JACK TO ALIGN A DRIVE UNIT. THE JACK HANDLE BECAME STUCK. WHEN THE HANDLE WAS FREED UP IT WAS EJECTED FROM THE JACK HANDLE SLEEVE. THE HANDLE TRAVELED ABOUT 5 FEET AND STRUCK THE INJURED EMPLOYEE ABOVE THE RIGHT EYE CAUSING A LACERATION.

Employee was riding on bus at end of shift when it hit a bump jarring his back. He continued to work his remaining rotation and on 11/22 saw a physician who put him on restricted work.

Crew was moving 8x8 oak timbers when one rolled onto the employees (L) thumb causing a laceration. Treatment required stitches and restricted duty.

Employee was attempting to obtain a sample of contaminates to send in for analysis. The plastic container he was using caught fire and sprayed him with material that was flammable. He received burns ranging from 1st to 2nd degree on both hands, face and left arm.

EMPLOYEE ALLEGES AN INJURY AS A RESULT OF A MOTOR VEHICLE ACCIDENT, AFTER THE INCIDENT EE SHOWED NO EMOTIONAL OR PHYSICAL SIGNS OF PAIN OR DISCOMFORT; NOR DID HE REPORT THE INJURY. HE CONTINUED TO WORK FOR (4)HOURS. EE REPORTED AN OFF-THE-JOB INJURY OCCURRING ON OR NEAR 10/25/05, TO HIS BACK & SHOULDER WHEN HE FELL FROM A ROOF. EE WAS ABSENT (4)DAYS DUE TO THIS INJURY.

Occupational hearing loss.

Employee stated that he was moving a 55 gal drum of oil, the drum slipped from metal surface, smashing his hand betwene the drum and a metal pole.

Foreign object entered eye while chipping weld slag or grinding weld.

Employee was raising the hood on truck #107. He lost his footing and fell backward off of the fender and landed on his right hand breaking his hand or finger.

Employee was carrying a cabinet fan upstairs into a shovel on his shoulder when he felt a pain in his lower back. Diagnosed with a lower back strain.

Employee removed hardhat to get between the frame and engine so he could install starter. He cut his head on a hose clamp which required 5 staples to close.

Employee was getting down off his haul truck to use the restroom. He was on the bottom step and slipped and caught himself with his right hand and felt a pop in his right shoulder. The diagnosis was a torn bicep.

Employee went up the engine ladder to turn heater valves on. He slipped on the ladder and hit his left elbow.

EE ALLEGES AN INJURY TO HIS BACK WHILE DRIVING A COAL TRUCK. HE STATED TO HIS SUPERVISOR THAT HE HURT HIS BACK WHEN HE HIT A HOLE IN ROADWAY. EXAMINATON FOUND NO HOLE IN THE ROADWAY. EE TOLD OFFICE PERSONNEL THAT HE WAS INJURED IN A MOTORCYCLE ACCIDENT AND THAT THE BOUNCING OF THE TRUCK HURT HIM BECAUSE HE WAS SO SORE. KANAWHA TRUCKING DISPUTES THE ALLEGED INCIDENT.

HE HAD REPAIRED A CABLE ON SHUTTLE CAR & RESTORED THE POWER. HE INSTRUCTED THE OPERATOR TO START THE PUMP MOTOR TO REEL UP THE CABLE, WHEN THE CAR STARTED, A LOOP OF THE CABLE PULLED HIS FEET FROM UNDER HIM, CAUSING HIM TO FALL.

Hit a bump while driving haul truck. Jarred neck and back and got shooting pains down right leg.

EE STATED THAT HE WAS OPERATING A DIESEL LO-TRAC, PLACING A MOTOR IN PLACE FOR A FUTURE BELT HEAD DRIVE INSTALLATION. HE GOT AGAINST THE COAL RIB WITH THE LO-TRAC, CAUSING SOME MATERIAL TO FALL FROM THE RIB. THE RIB ROLL STRUCK HIM ON HIS RIGHT LEG, CAUSING A FRACTURE TO THE TIBIA AND FIBULA IN HIS RIGHT LEG.

HE WAS OPERATING A MOBILE BRIDGE UNIT & WHILE BACK UP, HE RAN OVER SOMETHING IN THE ROAD CAUSING HIM TO BE JARRED. DRS REPORT ON 9/27/05

Employee was carrying an outrigger pad around to set up on the boom truck. While carrying the pad, he stepped in a hole in the ground and felt a sharp pain in the groin area.

Employee was using a chisel to knock the bearing collar off pin on 992 loader. When he struck the chisel with a hammer a metal sliver broke off the chisel & went into employee's forearm.

AS HE WAS HELPING LIFT A RAM JACK FOR A SCOOP INTO THE SCOOP BUCKET, HE GOT HIS RIGHT LITTLE FINGER CAUGHT BETWEEN THE JACK AND SCOOP BUCKET. IT RESULTED INTO A FRACTURE AND LACERATION TO THE END OF THE FINGER

A roof fall was discovered at the 48th crosscut of the 5 west mains panel in the #3 entry. The fall occurred in the belt entry. The fall is approx 30' l, 20' w and 7' h. The area had been bolted with 5' high Teck bolts, and spotted with 12' bolts.

EMPLOYEE WAS LIFTING BOX FROM FLOOR OF MAGAZINE TO LOAD ONTO TRUCK. HE FELT A POPPING IN HIS BACK BELOW SHOULDER BLADES. SAW ER PHYSICIAN & WAS GIVEN LIGHT DUTY (LIFT NO MORE THAN 10 LBS) UNTIL 9-6-05

Haul truck ran out of fuel while going up a ramp. Operator applied brakes but truck rolled backwards hitting spoils and tipping over on right side. Operator was wearing his seat belt but got banged around in cab bruising his shoulder. He was not released to work on his next scheduled shift.

At the time of the incident EE was climbing down the boarding ladder of a water truck. When He reached the bottom of the ladder he extended his right toward the ground and hopped off the ladder.Upon landing his right knee twisted and he fell to the ground.

Employee lost his balance and jumped from D9 dozer walkway resulting in a fracture to his right heel.

He was putting a pin in a rub rail guard on a continuous miner and got his thumb caught between the rail and the frame of the miner.

HE BENT OVER TO PICK UP A ROOF PLATE AND FELT PAIN IN HIS BACK. HE CONTINUED TO WORK & STARTED MISSING WORK ON 01/13/06.

EMPLOYEE STATED THAT HE WAS UNDER A TRUCK ATTEMPTING TH CHECK AN AIR LINE LEAK. HE ASKED THE DRIVER TO START THE TRUCK TO CHECK THE LEAK. DRIVER THEN STARTED THE TRUCK IN GEAR AND IT DRIFTED PINNING EE RIGHT KNEE BETWEEN AIR TANK AND GROUND

He was crossing over a mobile bridge system and tripped and fell. He continued to work and started missing work on 8/30/05. The original doctor's report showed his absence due to rheumatiod arthritis but on 9/30/05 we received a MRI report showing a small rotator cuff tear in his shoulder.

HE WAS WALKING UP AN ENTRY AND STEPPED IN A HOLE AND TURNED HIS ANKLE.

Employee was diagnosed with heat exhaustion.

Occupational hearing loss.

Employee pinched his finger between a sledgehammer handle and a metal bulk prill bin, resulting in a fracture to his left ring finger.

Employee was cleaning weld shop and wearing safety glasses and hardhat. At break, he noticed something in his eye. After trying to remove it himself, he was taken to emergency where they removed a metal particle.

Employee struck steering pin (haul truck) with sledge and two pieces broke loose, penetrating his right thigh. He pulled one piece out himself and the emergency doctor decided to leave the other in place. The wound was sutured.

EMPLOYEE WAS HITTING ON TRACK PIN WITH A 16LB SLEDGE HAMMER WHEN A PIECE OF THE PIN BROKE LOOSE. THE BROKEN PIECE HIT HIM JUST ABOVE LEFT KNEE, LODGING NEAR HIS FEMUR BONE. (NOTE: THIS WAS INITIALLY CHARGED AS A FIRST AID CASE, HOWEVER 4 DAYS LATER HIS DOCTOR DETERMINED THAT METAL NEEDED REMOVED)

At the time of the incident Employee was repairing the final drive on a CAT 793 off-highway truck. When he bent over to pick up an impact wrench he felt a pop in his back and could not stand up straight.

Employee hit a drill chuck with a wrench to insert it into the drill pot. A piece of metal came off the chuck and entered his lower right leg. He was treated in the E.R. and returned for the next two shifts of work. On Aug 3rd he returned to hospital where a Dr located and removed the piece of metal but in process damaged an artery.

Coming down ladder - foot slipped on step - step wet from water - went backward & caught herself with right hand - sprained right wrist.

Slipped on step as walking up.

He felt pain in his shoulder while lifting a 4" water line.

EE WAS TRYING TO LOOSEN A BOLT BY PUSHING ON THE WRENCH WITH HIS FOOT. HE STATED THAT HE FELT SOMETHING POP IN HIS BACK. HE CONTINUED TO WORK AND BEGAN MISSING WORK ON AUGUST 1 2005
