Employee was operating a Cat 793D haul truck and was being loaded by a L1400 Letourneau loader. The loader put the third bucket in the truck when a rock rolled to the right side of the truck box and caused the truck to rock which caused the employee pain in the neck area. Employee was seen at the local clinic and released to work. The employee missed their next scheduled shift.

The driver was driving on a raised dirt road when the berm of the road broke and the mixer truck went over the edge. The truck rolled onto its side. The driver was taken to the hospital and received 12 stitches on forehead.

An employee was transferring from a cricket to a mantrip. While the employee was exiting the cricket it rolled pinching the employees leg.

An employee was tramming a scoop loaded with supplies to load a roof bolter. As the employee trammed around the corner of the crosscut, EE grabbed the scoop canopy to pull self up and reposition self. The employee scrapped EE's finger between the scoop canopy and the coal rib, lacerating EE's finger. The employee received 7 sutures.

EE was jostled around in the seat during coal loading, as EE was reaching behind for a load log. EE was evaluated and released to return to duty on that date. On 11/28/2017 EE returned for additional medical evaluation and was held out of service to receive treatment for lower back, making this a reportable event on that date.

Employee was operating a Cat D400E Articulating Truck constructing a stream channel when the employee became stuck. While a dozer was assisting the employee, the tractor tipped over. Employee received a small cut on left arm requiring 2 stitches. Operator was wearing seat belt.

A buggy op was tramming S/C and leaned in to travel around a corner, EE accidentally hit the e-stop bar and stopped the car. it bounced the EE and EE struck EE's arm on the cab. EE was seen at the ER and released with a bruised arm. The EE went to a follow up appointment in the A.M. and EE's arm had swelled over night, the DR. made an incision to reduce swelling.

An EE parked EE's scoop and exited to wait for the shuttle car to tram by the EE was standing between the rib and the scoop bucket. The scoop bucket was sticking out past the corner of the rib, when the shuttle car was tramming through the entry and slid into the scoop bucket shoving it against the EE and pinning EE between the scoop and the rib.

A scoop operator, was dragging a structure tray behind the scoop tramming inby. The injured ee was walking behind the scoop and actually walked into the tray, the tray caught on a piece of rail and flipped the tray in the air causing the ee to go airborne. The ee hit the mine floor and the tray came down on top of EE's lower torso.

Operator said ee hurt back when ee hit a bump in the road. Upon new evidence this has become reportable.

The fuel truck was going up a ramp. The lights of a passing haul truck made it hard for the operator to see. The fuel truck hit the berm on the edge of the haul road and tipped the rear of the fuel truck onto its left side. The operator cab portion stayed upright. The operator was transported via ambulance to South Lincoln Medical Center for injuries he received from the incident.

We were informed late of injury on 05/31/2014. Employee stated he jarred his back in scraper when he ran over a large rock. Due to late reporting we waited for MT ST Fund to review and they accepted the claim as valid on 06/16/2014. Employee missed 2 work days as result of injury

Employee walking from parking lot to change room was struck by a light vehicle that was coming to change room to park. Employee was treated by Mine Rescue personnel and was transferred to County EMS personnel. EMS transported to hospital for evaluation and was admitted for observation. Employee is bruised and sore.

Injured employee lost control of a scraper on a thawing return haul road - laying the scraper onto the operators side of the machine.

Operator let air out of air ride seat and continued to operate with improper adjustment on seat for safe ergonomic operation of the end dump.

Employee operating a scraper and he ran over a frozen chunk of material. The impact "Jarred" him in the seat and he experienced pain and general discomfort in his left testicle. He was examined after shift at the clinic and again before his next scheduled shift and was unable to report for next scheduled shift.

Employee was operating coal hauler when he became distracted and steered machine to the right on haul road and came in contact with guard rail. The incident caused employee to hit his left lower leg on unknown object in the cab causing injury.

A mechanic was operating the shop overhead crane placing a planetary gear on a haul truck. While attempting to lift a tool to be attached to the gears the employee activited the high speed lift button, causing the tool to slip & tip the gear onto the employee's left foot.

Operator hit a bump in the road causing his head to contact the ceiling and coming down hard in seat.

Employee was operating a water truck. At 9:30 am, his left eye started irritating him & continued to worsen. He is unsure exactly when and what got into his eye. He was taken to a local ER & treated for a foreign body in his eye. His eye was rinsed and an antibiotic ointment was put on his eye as part of the treatment.

After staging truck at loading face the right rear trailer strut failed as the 1st bucket load of coal was dumped into truck. The resulting severe rocking motion slammed the driver into the operators door of the tractor.

Employee drives a 240 ton coal hauler and while being loaded, when the first bucket of coal was dropped into the truck, the employee experienced pain in neck and back. EE was examined by a Medical Dr. and has begun Physical therapy. Neither time nor wages have been lost.

Employee was operating a coal hauler and drove off the right side of the road, traveled over a small pile of dirt, back up the side of the road and over a large berm. As a result the employee received a laceration and was taken to physician for medical treatment.

EE was operating a Cat 777 end dump. She stated that she was having problems with the operator's seat. She stated that she hit a small bump in the haul road and the seat bottomed out. Later, she felt pain in her neck & shoulders. She was taken to a local ER for treatment & released for light duty.

While returning to pit with empty coal hauler, employee ran into back of a scraper that was waiting for a loaded coal hauler to pass before turning off main haul road. Due to adverse weather conditions (rain, mud) the scraper's tail and can lights were not visible.

Employee was assigned duties operating a tractor trailor for coal hauling. He first reported pain in his lower back on April 19, 2004. His back has since became more aggravated. He went to a physician for treatment and was unable to return to work for his next scheduled shift.

Employee was operating a wheel scraper removing interburden from above the lower coal seam. It was snowing hard and a depression in the roadway was not readily observable. A scraper tire impacted the depression resulting in pain in the employees back. He was unable to return for his next shift.

Employee was hauling coal and he drove the hauler over uneven terrain, causing jarring in his lower back. He continued working, however the pain continued and he sought chiropractic treatment to help relieve the pain. To date neither time nor wages have been lost.

While operating a front end loader, the seat bottomed out when he hit a bump. He experienced pain in his hip, ribs, and lower back. The employee sought chiropractic treatment to alleviate the pain. Neither time nor wages have been lost. Employee did not report incident until 12/20/2006.

Employee was laying topsoil with a scraper and ran over a rock with the front tire. He experienced pain in his neck and upper back. He sought chiropractic treatment to help relieve the pain. He continues to work, and to date no time has been lost.

Employee was operating a scraper and tried to hook up to a scraper in front of him, the push block caught on top of the front machine. When they seperated, his machine dropped to the ground, and he experienced pain in his back. He was diagnosed with a compression fracture of a thoracic vertebrae. He was unable to work his next scheduled shift.

Employee was moving cable with a cable tractor when he hit a rut in the road. He hit his head on the roof causing him pain in his head and neck. He was seen at the clinic and began physical therapy June 20, 2006

Employee, operating a coal hauler, had just left the backhoe after being loaded. She was turning to go up the ramp when the tires caught the mud, causing the hauler to jacknife. When the trailer hit the trailer stops, the force of the impact jarred her from side to side in the cab. She sustained two fractured ribs, bruising to her thumb, leg and ear. She was wearing a seat belt.

Operating an End Dump when the front tire hit a hole, which caused the steering wheel to jerk and spin around, which caught the right hand, causing the fracture.

Employee was operating an end dump, hit a bump in the road and felt pain in his back. He was treated by a doctor who placed restrictions on him for his work schedule of February 2-6, 2006.

Removed wheel chalk from bracket and when it came down, it caused something in arm to pull and felt a sharp pin.

Backed up in to pad to dump the load dozer operator asked me to back up more, so i did when side of pad gave way causing truck to roll over and set up right on haul road.

EE WAS POSITIONING TRAILING CABLE FOR PLANNED DRAGLINE MOVE. PULLING ON CABLE ACROSS A SLOPE OF ABOUT 4 TO 1 GRADE. 1 FRT TIRE OF THE REELER TRAVELING INTO SLIGHT DEPRESSION & LOADER ROLLED ONTO ITS SIDE. THERE WAS ABOUT 1200FT OF 12,500 VOLT CABLE ON REEL. EE WAS USING SEAT BELT,BUT STRUCK HIS MOUTH & CHEECK ON SOME OBJECT & REQ'D SUTURES IN HIS LIP

Employee was moving nine loaded rail cars down a grade and the brakes on the shuttle he was driving failed to activate when applied. The cars contacted a string of nine parked loaded rail cars. Employee was medically evaluated and began Physical Therapy on 09/09/2005. He has continued working. On 2/1/06 he had surgery on his shoulder to relieve the pain.

Employee was operating a water wagon on a haul road with a 70' top when a hydraulic line failed causing loss of steering. The water wagin went through the barrow ditch and onto natural terrain. Employee experienced pain in his back going through ditch. He began chiropractic treatment on 08/09/05. No time has been lost.

Employee was hauling topsoil with a 657-E Caterpillar scraper. He hit a bump and experienced pain in his low back when the cushion hitch malfunctioned. On 06/05/05 he received an injection for pain, and on 06/14/2005 he began physical therapy.

On 12/30/2004 the employee reported ever increasing pain in his shoulders and attributes it to his duties as an over the road truck driver(rolling tarps, shifting, etc). On March 30, 2005 he reported that he began receiving physical therapy to relieve the pain. Neither time nor wages have been lost

As ee was driving a loaded hauler away from a backhoe, the right rear trailer tires of hauler fell into a large hole, stopping hauler suddenly. His lt arm struck the steering wheel, rt elbow hit shift tower. He had seatbelt on. He went to a local ER, was treated & released to return to work w/restrictions for 5 days. On Fri 10/29/04 he started losing time from work.

EE STATES HE HIT A CHUCK HOLE AND THIS HURT HIS BACK A LITTLE (8/31/04). THE PAIN LEFT AND EE DID NOT REPORT IT UNTIL THE AFTERNOON OF 9/2/04 (EE DID NOT SPEAK TO SUPERVISOR UNTIL 9/3/04). EE CALLED 9/3/04 AND STATED THAT HE HAD LOWER BACK PAIN & WAS GOING TO A DOCTOR. DOCTOR EXAMINED EE AND PRESCRIBED 2 MEDS. DOCTOR GAVE HIM WORK RELEASE FOR 9/7/04.

Employee was a passenger in a Euclid Ch-120 haul truck riding in the jump seat. The loaded truck stopped to dump and when the brake was set the tractor frame failed breaking nearly completely through on both sides. The cab fell forward contacting the ground with significant impact. Employee struck head on air conditioner and his lower leg on the passenger door.

EMPLOYEE CALLED IN SICK ON 7/20, CAME TO WORK ON 7/21 TOLD SUPERVISOR HE WAS URINATING BLOOD & WAS GOING HOME. CAME TO PICK UP CHECK ON 7/22/04 & TOLD HIS SUPERVISOR HE HAD A BAD VERTEBREA & HAD TO BE OFF UNTIL 7/26/04. EE SAID THE BOUNCING ON THE SCRAPER DID NOT CAUSE IT BUT HE WAS NOT TO RUN THE EQUIPMENT UNTIL HE WAS WELL. ON 7/28 EE REPORTED HE HURT HIS BACK ON SCRAPER.

Driving into pit, the employee's truck jack-knifed on wet road and came to a sudden stop, jarring his neck and back. Examined that day, returned to work with no restrictions. On November 17, 2004 employee notified company that he was receiving physical therapy for pain management. To date has missed no time and performed no restricted work.

Employee had completed his shift and was leaving site when a pickup driven by a NWR electrician ran into him as he was walking to his personal vehicle. he was hit on the right arm and sustained injury to wrist and elbow. he was transported to hospital. Both wrist and elbow were broken.

While operating a water wagon the employee drove over a bump in the road, the air seat bottomed out and did not return to the original position. He reported pain in his lower back from the incident. He is currently receiving treatment from a chiropractor, but is working his regular job title without restrictions. Neither time nor wages have been lost.

EE WAS TRANSPORTING A BACKHOE ON THE LOWBOY TO AREA D. THE TRAILER HIGH CENTERED AND THE TRACTOR REARED UP. WHEN THE TRACTOR CAME DOWN, HE FELT PAIN IN HIS NECK AND LOWER BACK. (HE WAS WEAR ING HIS SEAT BELT.) HE WENT TO THE DOCTOR ON 02-17-04 AND IS ON LIGHT DUTY.
