Caterpillar 988K wheel loader was maintaining a shovel dig site when the left rear corner of the loader was contacted by loose material that fell from the dig face. The loader operator reported neck and back pain from being jostled in the cab. The left rear loader's fender and boarding ladder were damaged. The loader was taken out of service for repairs.

While an employee was digging for large chunks in a rock bank with a loader, a large chunk came down the rock bank and landed on the loader's full bucket, jarring the loader and the employee. The employee was taken to the local ER/personal doctor due to back pain where it was determined the employee has thoracic compression fractures to T8, T10 and T11 of the spine.

Employee was operating a 240 ton haul truck on the roadway. They noticed smoke from under the deck. They pulled to the side and stopped the truck. Flames engulfed the exits, so they exited over the side rail and dropped to the ground.

While discussing the next shift's work with other contractors, a piece of refractory fell from near the top of the stack, striking employee directly on the top of EE's hard hat, knocking EE unconscious. EE regained consciousness and was transported to hospital. EE has a fractured skull and cheek bone.

A railroad employee was travelling down a section of rail in a piece of equipment. The piece of equipment derailed and the equipment turned onto it's side. The employee suffered a small laceration to the head.

Employee was assigned to operate Caterpillar 777C #648 truck. EE was traveling down a haul road loaded, when EE stated that EE struck a hole in the road, jamming head and neck. EE suffered pain in neck and upper back.

Employee was tramming a scoop when their left pinky finger was struck by a belt rail, causing a laceration to EE's finger. The wound required 3 stitches to close.

While operating a 988K Wheeled Loader (Co # SL22), during normal working tasks, Employee stated was experiencing back pain with pain to the right leg. Employee indicated that the pain had become more intense throughout the shift.

Injured was an employee riding in a track mounted man-trip, when it derailed and struck the coal rib causing the man-trip to stop suddenly. The employee was thrown into the man-trip, and EE suffered a laceration that required one stitch and blunt trauma injury to EE's right eye. EE was wearing safety glasses at the time.

An employee operating a loader was performing normal work duties maintaining a mining floor. During this activity, the employee stated that the loader bucket contacted a toe, jostling the loader resulting in the employee chipping a tooth. Dental treatment was made known on 2/2/2023.

Employee was looking backward in an end loader when the bucket fell to the ground. The jarring action of the falling bucket along with the twisting motion caused a pull muscle in EE's back. Employee began missing on 10/16/2022

A pellet plant utility operator was walking behind a forklift being operated when the forklift inadvertently went in reverse, contacting the utility operator's left leg resulting in a sprained ankle.

Lost control of a loaded coal truck. Driver had a head contusion. Neck and back strain. Lacerations to right hand and a corneal abrasion.

Employee was operating a loader when the tooth on bucket hung up on a solid rock formation causing the loader to stop, causing the loader to lurch, which caused employee to have pain in neck.

Injured employee was operating a track mounted mantrip when EE went through a low area, and EE raised up too quickly. EE knocked off EE's hard hat and EE's head struck the roof. This resulted in a laceration to EE's scalp that required stitches to close.

While backing into spotted shovel bucket, the tail of production truck made contact with shovel bucket. The operator reported discomfort in their neck and was transported to the clinic for evaluation. After evaluation, the operator was prescribed physical therapy for a neck strain.

Trucks were using a back road to access the truck dump due to the tracks being blocked by a train at the crossing. EE operating truck 807 proceeded around a horseshoe curve hauling clean coal from Eckman Plant to Dans Branch Load-out. The driver stated EE misjudged the curve and ran up on the berm due to poor visibility causing the truck to roll over.

EE was assisting a fork truck operator unloading steel plates from a trailer deck. The fork truck operator inadvertently tipped the plate causing it to fall off the trailer deck. The plate struck the EE in the foot resulting in a broken tarsal.

Employee reported jarring their back when the cleanup loader hit a hole in the road during the night. Was seen by an EMT in the morning. Consulted with on-site NP per phone after getting home. Conservative care instructions provided.

Employee was plowing with Skidsteer and when the end of the apron was reached, the blade broke through the ice abruptly stopping the machine. This jarred the employee causing pain in neck and shoulder blade.

Shovel Run truck, 9555, contacted berm on Tower Hill road, truck has some front end damage, pending further inspection. Operator sent to Bell for evaluation of mild neck pain and released. Department to conduct an investigation.

EE was loading T76 when a frost chunk came down and struck the bucket. It forced the bucket to the ground and caused a cap on left hydraulic cylinder to blow off. EE stated EE was fine. Approximately an hour later EE reported stiffness in neck. EE was seen by EMT, and recommended that EE be brought to ER for further evaluation.

THREE MAINTENANCE REPAIRPERSONS WERE working behind LINE 8 PRIMARY MILL. AS THE FORKLIFT OPERATOR WAS BACKING UP, THE REAR OF FORKLIFT STUCK AN EMPLOYEE IN RIGHT foot and ultimately backing over right leg. Employee sustained bruising and skin abrasions to right leg calf and thigh. EMPLOYEE WAS TAKEN TO UP HEALTH SYSTEM- BELL FOR EVALUATION, X-RAYS WERE NEGATIVE.

Employee was operating a pick-up truck in load-out area at night while it was dark, drove over two, one foot high mounds of pellets resulting in being jostled in cab, chipping a tooth. Tooth repaired on 5-10-19.

EE reported hitting the headliner of a pickup truck with hardhat causing pain in neck and back. Employee was front seat passenger when the pickup made contact with the end portion of a dirt berm located along tie down area. Employee was assessed by on-site medical responders, and then brought to hospital for further evaluation where medical treatment was provided.

EE became distracted when their gloves fell off the seat of the pickup while driving. EE swerved off the road while retrieving the gloves from the cab floor hitting two used tails basin pipes. EE suffered a laceration to their knee.

EE was operating a loader in the chunk tunnel when the loader bucket contacted something causing the loader to stop suddenly. EE reports pain in the chest area. Seen by on site nurse and transported to UP Health System Bell.

EE was driving a production truck, and reported a sore back from being shook while in the cab.

**Alleged* * On 10-9-17 at end of shift, ee reported an injury occurred on 10-3-17. EE claims striking left shin on piece of steel pipe causing an abrasion. EE worked remainder of shift, two more shifts, and had long weekend off. At the end of first shift back (10-9-17) ee reported injury. EE advised management the following morning (10-10-17) EE received treatment in the ER.

Dark rainy conditions with poor visibility road way was bumpy due to recent rain events. Employee was in haul truck when EE hit a bump in the road the bump bounced EE in the seat which EE had aired up quite high. EE is a tall person and the bump caused EE to strike head on the roof of the truck causing EE pain in the neck and the right arm.

Mine RR ops tech was unloading scrap in the yard off of a grapple truck from the elevated loader operators seat. While picking a piece of RR track up with the grapple, EE inadvertently swung the wrong direction. The rail hit the guard and than raised up striking the employees right arm and right knee, causing a laceration requiring stitches to employees right arm and right knee.

Employee was riding on top the #31 mantrip coming off of the 9L section at the end of the shift. The trolley pole came off of the wire, struck the mine roof, then broke into. One of the jagged edges of the broken pole struck the employee on the right hand between the middle and ring fingers causing a laceration that required 5 stitches.

Injured was tramming scope when he hit a hole in the bottom throwing him into the canopy of the scoop. Causing pain in his neck. He finished the shift and saw the PA on site.

Inattentive Driving: EE was a passenger in company vehicle driving on plant roadway. Driver reported sun glare caused him to veer and miss a turn in roadway. Continued down 5' steep grade into drainage area. The abrupt stop caused a fractured fibula bone to driver and passenger re-injured a previous non-work related injury to his wrist. Very little damage to company vehicle.

Continued down 5' steep grade into drainage area. The abrupt stop caused a fractured fibula bone to driver and passenger re-injured a previous non-work related injury to his wrist. Injured employee was a passenger on #25 bus and spurred into 9G to allow supply motors to pass, when the supply motor hit the 9G switch, the motor veered into 9G, striking the #25 bus and injuring a miner.

On 10/17/15 employees reported soreness from driving on rough roads but did not want to seek medical attention. On 3-26-15 the employee went to a physician and was placed on restrictions for a lumbar strain.

Injured was operating a battery ram car, hauling coal from the miner to the feeder, when the car hit a pot hole, causing the injured to strike hardhat against the canopy of the ram car. This resulted in a cervical strain. The injured was seen by a doctor on 12/16/14 and put on restrictions.

Employee was operating a scoop with an interlocking double door. The safety pin was not installed correctly into the hasp and the doors opened while the scoop was in motion and struck the employee in the lower back.

Employee was transporting supplies to section on a motor trip, when he struck his head in low top and felt pain in his neck.

While operating ramcar the ramcar ran over some crib blocks on the side of the rail causing the employee to bounce up and down. Employee struck head on canopy.

Employee alleges that he was walking through flypads and was struck by a shuttle car. Cliffs Logan County Coal disputes that EE sustained an accident, as that term is defined in 30 CFR 50.2(h), or that he sustained an occupational injury as that term is defined in 30 CFR 50.2(e) or an occupational illness as that term is defined in 30 CFR 50.2(g).

Employee was making his normal fireboss run on a ride and was coming through the outby airlock doors. He got off the rail ride to close the doors. As he went to close the doors, he noticed the ride to start moving and he ran and jumped onto the ride when his foot was caught underneath the ride which resulted in a fracture.

Employee was traveling as a passenger in track ride facing the direction of travel on West Mains track. At #142 break the employee struck a roof bolt causing the employee's head and neck to recoil. The employee was transported to the local hospital.
