An employee was cleaning accumulated mud from around a machine guard when their tool contacted the moving conveyor belt. This contact caused the tool to pull forward, pinching the employee's hand against the gearbox. The incident resulted in lacerations that required medical treatment, including stitches.

While employee was cleaning under the wash plant with a mini skid steer EE backed into the H beam which forced EE's arm and wrist into the boom lift lever which injured the employees wrist.

While two employees were working around a tail pulley of a conveyor belt, there was miscommunication between the guys on the ground and the plant operator. This resulted in the belt turning on and grabbing one of the employee's sleeves, pulling them into the tail pulley.

Worker was driving through a ditch and EE's leg became stuck under the chair of EE's vehicle. EE also bumped EE's head on ceiling of vehicle. EMS was called due to severity of leg injury.

Employee was clearing blockage from a closed conveyor belt gate; employee attempted to clear blockage with hands, moving conveyor belt moved the blockage while employee had hand on it and hand became wedged between belt and blockage causing injury to fingers on left hand.

EE was using water truck to wet down area when loader operator who was back dragging roads backed into the water truck EE's chest struck the steering wheel. EE has some strained muscles in chest.

manlift basket was used improperly as a lifting device and the employee was pinched during the lifting process.

Employee was in the process of training the belt conveyor, using a pry bar for cleaning around bearing bolts, pry bar slipped between the belt and the tail pulley "belt was running" this caused the pry bar to push the employee's hand against the conveyor frame resulting in a cut to the right pinky finger, requiring 2 stitches.

EE was traveling on the railcar as the spotter. They decided to stay on the railcar as it traveled through the crusher building. The railcar they were pulling through was too tall for the crusher building, causing the steel beam at the entrance to scrap the metal walkway and rails off of the top of the railcar. Several pieces struck the EE in their shoulder and hand.

Maintenance mechanic was observing an incline pan conveyor for broken wheel attachments and inadvertently put and on rail. Wheel on pan conveyor ran over middle and pinky finger on left hand.

Employee was driving the haul truck back to the pit when EE anticipated a haul truck coming in their direction. EE applied the brakes and slid the haul truck into the dry drainage ditch which caused EE's head to bump the top of the cabin. Initially the driver was unharmed, days later a concussion was diagnosed, months later EE's driving was medically restricted due to the incident.

Two contractors were working on a chute job on one of our sites and the main contractor was in the telehandler and their EE was working on the catwalk when the operator dropped EE's water bottle and when reached to get it, lowered the boom. Causing their EE to be pinched in between the back of the forks and the stationary I-beam

Employee alleges that the tire blew out on equipment EE was operating causing injury to EE's head.

A Haul truck operator was loaded coming out of the pit struck a haul truck going into the pit to be loaded. The haul truck that was hit swerved abruptly to try and avoid being hit. The operator that was hit suffered low back pain, prescribed PT, pain meds and muscle relaxers. Damage to front left of one haul truck and both tires on the other.

Employee operating loader under supervision when EE became stuck in sand stockpile being jarred and bounced causing lower back and tailbone pain, no report of any injury until 12/7/20, sent to clinic, prescribed lodine, terminated 12/7 for non-reporting

The employee was cutting a stockpile with a front end loader when a large chunk of frozen road gravel fell from the top of the stockpile onto the front of the loader and into the cab. The employee suffered minor cuts on the right hand and lower back pain from removing material from their lap in order to exit loader.

Emp claimed inj back on Fri. 7/28 but did not report it until Mon. 7/31. Emp was oper. a Komatsu 400HM haul truck and hit a pothole in the haul rd. causing inj to lower back. This report was originally submitted via paper copy, and this report is being generated to make amendments per MSHA field office. Original was submitted 8/8/17.

Employee was trying to remove a piece of ripped belting that wrapped around the shaft of a tail pulley. Another employee was assisting in bumping the belt to rotate the pulley, and the initial employee removing the belting got EE's arm pulled into the conveyor, causing significant injury to include fractures, and skin lacerations to the right arm.

EE was operating a wheeled loader when EE encountered a bump in the road and states EE bottomed out seat causing pain to lower back and neck.

Employee was operating a John Deere 300D end-dump haul truck while performing dirt stripping activities. Employee was pulling away from dump-site when a hydraulic line supplying pressure to the bed ruptured causing the bed to slam on the frame of the tractor. Employee didn't wish to seek medical treatment and completed tasks for the entire shift.

Employee was handling the suction hose on a vac truck, when the hose sucked against the wall. This caused the hose to coil with vac pressure. Employee lost grip and hands were pulled up and back toward the top of the forearms by the hose. Employee was diagnosed with a sprain to the right hand/wrist.

Ind. Hauler was attempting to leave the quarry site when he lost control of his truck causing it to overturn. The scale house clerk stated that the truck driver was talking on his mobile phone has he left the scale. The driver was distracted and works for Kentucky Concrete.

The employee was parked in a haul truck waiting to dump his load at the crusher. One employee pulled off in his truck and the side of his truck hit the left side mirror and bracket of the other truck causing it strike the side glass and windshield. When the glass broke it landed in the other employees eye.

Employee was operating an articulated off-road truck hauling overburden. Employee attempted to lift himself off the air seat to adjust his position when he felt his back pop.

Employee extended his left foot out of the operator compartment of a Case Skid Steer. At the same time he lowered the bucket catching his foot between the bucket assembly and the frame. (He had received task training in the safe operating of the machine, including the need to keep his head, arms and legs inside the operator's compartment).

Employee was mounting a cylinder on a haul truck and caught his finger between the cylinder and the tank. Employee pulled the cylinder and jerked his hand out causing injury to the left index finger. Employee was transported to a local Dr. Michael Carter. The employee received stitches, a mandatory company drug test and returned to work approximately 1.5 hours later.

EE was climbing down ladder on a Euclid R-50 size Haul truck. When got to the next to last step he stated that he thought that he was on the last step and stepped to the ground which was further than he thought causing him to lose his balance and fall. He fell 3' landing on his left side. X-rays revealed no injury, but family Dr. put ee off until 2/8/10.

Using skid steer to move a conveyor belt, the operator had the bucket over the belt moving it. The employee was trying to help move the belt. Operator raised the bucket and the belt started to roll. He sat the bucket back on the belt. Operator couldn't see the employee's hand on the belt trying to stop it from rolling.

Operating load in the pit. A rock rolled out at shot pile, hit loader bucket, made hand slip and jammed thumb which broke a bone above the thumb.

OPERATOR WAS DRIVING DOWN THE HILL LOADED. HE SAID THE TRUCK STARTED GOING FAST AND HE DROVE IT INTO THE DITCH TO STOP.

SPLICED CONV.BELT, AFTER COMPLETING SPLICE EE STARTED BELT & THOUGHT IT WAS RUBBING SOMEWHERE SO HE WAS LOOKING TO SEE WHERE WHEN A ROCK FELL OFF & LANDED ON HIS FOOT. EE BROKE A BONE IN HIS FOOT, EE HAD STEEL TOES ON.

OPERATOR TRAVELED DOWN HILL TO TURN AROUND SITE PUTTING MACHINE IN REVERSE AN BEGAN TO BACK UP FELT A JERK AND CAB OF TRUCK TURNED OVER AND CAME TO REST ON ITS RIGHT SIDE.

EE WAS OPERATING A CLEAN UP TRACTOR IN THE SECONDARY PLANT. HE WAS CLEANING FROM UNDER THE CONVEYORS WHEN A 2" TO 2 1/2" ROCK FELL AND HIT HIM ON THE LEFT KNEE.

DRIVING STOCKPILE TRUCK, DUMPING LOAD HEARD POP LOOKED OVER HIS SHOULDER. BACK GLASS IN TRUCK HE WAS DRIVING SHATTERED AND A PIECE OF GLASS HIT HIM IN EYE. CUTTING HIS EYE.

WAS REAR ENDED AT STOPLIGHT

ATTEMPTED TO CLIMB UP THE SIDE OF CONVEYOR WHEN A ROCK FELL FROM CONVEYOR FRAME. WHEN HE STEPPED BACK DOWN TO GET AWAY FROM ROCK, IT FELL ON HIS FOOT.

AGGREVATION OF BACK INJURY SUSTAINED ON 4-3-93, THIS WAS NOT A LOST TIME INJURY AT THE TIME OF INITIAL ALLEGATION. OPERATOR ALLEDGES THAT LOADER STRUCK A POT HOLE IN HAUL ROAD AND JERKED HIS B ACK, EMPLOYEE CONTINUED WORKING UNTIL TODAY.

THE OPERATOR HA8F BACKED HIS TRUCK UP TO THE BIRM AND STARTED TO DUMP WHEN THE ROCK GAVE UNDERNEATH HIM. THE EUCLID WENT BACKWARDS DOWN THE FACE AND FLIPPED BACKWARDS. THE OPERATOR SUSTAINED 2 SMALL CUTS O3E ON HIS NOSE AND ONE ON HIS UPPER LIP WHICH REQUIRED 6 STITCHES. HE WAS ALSO BRUISED ON NUMEROUS PARTS OF HIS BODY.

AT 11:15 I NAME3WAS SITTING IN THE TRUCK BEING LOADED AT THE LOADING SITE AT MUCK PILE AND A LARGE ROCK CAME OFF THE MUCK PILE HITTING THE TRUCK ON THE REAR END OF THE BED CAUSING IT TO JERK HIS NECK AND S2OULDERS SENT HIM TO HUMAN EMERGENCY RM. RESULT STRAINED MUSCLES IN HIS NECK OFF 2 DAYS.

EMPLOYEE WAS AP1LYING STICK TYPE BELT DRESSING TO HEAD PULLEY OF FEED BELT ON CONVEYOR LEFT ARM BECAME ENTANGLED IN PULLEY AND WAS PULLED AROUND, BREAKING ARM AND CAUSING SEVERE LACERATIONS OF UPPER ARM AND 9LBOW.

VICTIM WAS INST5LLING A MOUNTED TIRE 1800X33 ON AN END DUMP WITH A LOADER WHILE HIS FELLOW WORKER WAS OPERATING THE LOADER THE VICTIM WAS TRYING TO LINE UP THE BOLT HOLES TO START A BOLT WHE N THE VICTIM SH3FTED TEH TIRE IT FELL FROM THE HUB PINNING THE VICTIMS RT SIDE

EMPLOYEE WAS CHECKING CONVEYOR BELT TO SEE IF IT WAS FREE. AS THE EMPLOYEE WAS TURNING SHEAVE ON HEAD PULLEY DRIVE THE CONVEYOR BELT ROLLED BACK, PINNING HIS LEFT HAND BETWEEN SHEAVE AND GEAR REDUCER, CAUSING A LACERATION OF THE INDEX FINGER.
