Employee was riding in the basket of the manlift through the yard and hit a small bump. The employee struck EEs left elbow on the manlift basket cage. The injury started as a first aid incident and was being treated by Ortholive with conservative care. The incident became a reportable incident on 5/20/25 when the employee went to an urgent care facility and was given restrictions.

Truck Driver became distracted for a moment while putting trash in their lunch box when hauling 1" DOT basecourse from the Blackhawk Quarry. Before they knew it the left steer tire was already over the left berm embankment as they were exiting the pit just before the scale. Resulting in the truck and pup to roll onto its left side.

While driving loader, operator ran over a big stone that caused operator to be jolted inside the cab. While steadying self, operator placed left hand on the frame of the cab (where door hinges are). The door swung closed and pinned operator's thumb. Thumb was fractured, received 2 stitches, and given prescription antibiotics.

Haul truck hit a bump and the right glass window broke. Employee grabbed right armrest leaned to the left to avoid the glass, hand slipped off causing a small laceration to EE's right arm. Employee was taken to the hospital for evaluation. The wounds were cleaned & sutured. The employee was released from ER with No Work Restrictions.

while driving the haul truck over a recently constructed haul road the tires slipped off edge and went into a slight water puddle causing the truck to rock and the employee struck EE's head on an AC unit that was hanging from the ceiling. employee was taken out of work for one day and is now on restrictions due to lingering symptoms.

While operating an articulating haul truck, the employee drove through a rut caused by water drainage from recent rains. When the rut was struck, the cab was shaken around. After these events, the employee complained of pain in neck, back, shoulder and knee on the left side of the body. Events did not become occupational injury until medical treatment on 3/2/2023

The miner was backing the haul truck into the shop to be repaired and caught their hand between the truck's catwalk handrail system and the garage door frame. The handrail system was bent, and the employee reached through the cab window to pull it in while backing in. The impact caused a contusion to EE's left hand.

The miner was driving a Bell B45E articulated dump truck up a ramp in the mine and hit a rut in the road causing EE to bounce up off the seat and hit EE's head on the safety handle located on the roof of the truck. The miner went to the local ER where EE was placed on light duty due to having concussion symptoms

While riding in a man lift, a 3 foot piece of I-beam fell over and cut lower leg of Employee.

EE was driving personal vehicle on way to leave for the day. Snowfall caused the ee to become disoriented and ee struck a stockpile. EE's head struck the steering wheel causing an abrasion to ee's lip. Injury required stitches.

A driver from the trucking Div. of Branscome Inc. was coming into the mine site on the main haul road to dump off millings. Driver was reaching for a bottle of water, they failed to maintain control of the truck and drove off into the ditch on the right side, causing the truck to lay over on its side. The weather was sunny and dry .

Used a section of conveyor belting as a floor mat.

Against procedure an employee attempted to track a conveyer while it was in motion buy accessing a ladder to reach the adjusters, While doing so he came in contact with the moving belt causing his left arm to be caught briefly. He received first aid for abrasions and contusions and has no restrictions. The employee was given a prescription for pain, making this reportable

Employee was loading a truck with a 980K loader and turned to look back, tweaking his back. This Employee has a pre-existing back injury which is non-work related.

Injured party is not an employee of the mine. Mine personnel were assisting a vendor unload metal culvert pipe that he was delivering to the mine. One pipe had been removed with a forklift and placed on the ground. He then walked around the trailer to the side opposite the forklift to secure a strap when a second pipe rolled off the struck him in the leg fracturing the leg.

The worker was reinstalling a guard on the rock screw. As the loader let the guard down. The guard came in contact with the frame of the rock screw. Causing the chain to release resulting with the guard coming down on the right foot just above the steel toe of the boot. The resulting injury is a broken little toe on the right foot.

Employee while working from a manlift 20ft above the ground he injured his right shoulder during the lowering of a counter weight pulley using a chain jack. The manlift basket suddenly dropped 2ft from the original working position causing an over extension of his right arm.

Employee was attempting to clean a conveyor pulley while it was in operation with a pry bar agaist company policy and without any directive to do so. Pry bar was caught by conveyor and smashed employee hands against conveyor frame causing laceration to digits of the right hand. Operation was stopped and re-training was completed.

Employee was connecting a chain from the stacking conveyor to the front end loader to reposition stacking conveyor. Loader began backing, chain tightened, and employee did not release chain. Employee strained right shoulder.

Employee was driving haul truck dumping shot rock at the top of a ramp being constructed from the primary crusher level down to the bench below. While preparing to dump the ground apparently gave way resulting in the truck overturning and sliding down along side of the ramp being bulit. Driver was wearing seat belt assembly and remained in the truck.

An employee under his own independent action stuck his hand into the return side of the belt to get a rock that was hitting an idler roller. The idler roller sucked his hand in and he broke his right forearm. The pull cord was activated and the belt stopped. He called for help and was removed from the pinch point.

EE was working from a manlift basket removing frozen material from a conveyor belt. While leaning over the manlift basket, ee right thumb was pinched between the manbasket and the conveyor frame resulting in a fracture right thumb.

Employee was on the road in Pit #3. While turning the corner near Conveyors 13 & 14, the truck struck a pile of silt material on the right side of the road near the conveyor. The driver was distracted by passenger and was going too fast for the conditions.

Driving the grease/fuel truck in pit area when he hit a rut in road causing him to hit his head on roof of truck. He was wearing hard hat & seatbelt. Heavy rains had washed rut in road. Employee is experiencing headaches & sore neck. No lost or restricted duty but had MRI in Nov. & may have physical therapy. Company didn't get results of MRI till Nov. 16.

While loading truck, the left tilt cyl. hose pulled out of hose fitting. Hose whipped into windshield, breaking windshield, covering operator with hot oil and glass.

Dump truck driver hauling clay. Had to dump some off, had his hands under tailgate when clods fell out causing fingers to be pinched when tail gate shut.

EE WAS DRIVING THE SKID STEER ACROSS THE YARD AND HIT A POTHOLE. IT THREW HIM AROUND IN THE CAB AND STRAINED HIS NECK SO WE SENT HIM TO HAVE IT CHECKED OUT JUST TO BE ON THE SAFE SIDE.

Truck driver was pulling down roadway into pit and hit his head on roof of truck from a bump in roadway, hurting his neck.

The conditions contributing to the injury are unclear. The employee believes his back strain was received from the seat of his loader. He stated that he increases the air in his seat causing him to stretch to reach the floor pedals. Prior to working in the loader, he was shoveling in a awkward position in a tight area.

CAT 980F loader control's adjustment jammed and operator unable to adjust for neutral positioning during equipment's operation. Several days of operation resulting in muscular repeatative motion injury to soft tissue, posterier of left shoulder. Injured employee tardy in prompt reporting to management.

EE was descending into the quarry pit when he had break in the bulk truck that he was driving. The truck traveled about 1070 feet and ended up crashing into a pile of rip rap.

CLEANING OFF TOP OF GRIZZLY AND PUSHED HOPPER OVER WITH A LOADER RESULTING IN 4 DAYS OF LOST CO PRODUCTION SENT FOR DRUG TEST, HAD DR EXAMS BOTTOM OF LEG MUSCLE.

CO-WORKERS WERE TRYING TO GET AN OVERLOADED BELT TO RUN. INJURED CLIMBED ONTO THE BELT TO SHOVEL OFF MATERIAL, BUT HE DID NOT REALIZE THAT THE HEADWHEEL WAS STILL TURNING. THE BELT WAS NOT MOV ING. WHEN HE STEPPED ONTO THE BELT, IT TOOK OFF. HE SLIPPED AND FELL TO THE GROUND. HE SUFFERED BRUISES TO HIS NECK, BACK AND LEFT ARM.

EMPLOYEE WAS INSTALLING A DRIVE SHAFT ON A EUCLID R 50. THE PARKING BRAKE WAS RELEASED BEFORE ENGINE WAS SHUT OFF WHICH ALLOWED THE DRIVE SHAFT TO TURN SMASHING AND FRACTURING EMPLOYEE'S THUMB .

EMPLOYEE STATES HE WAS ATTEMPTING TO PIN AN EUCLID BOX IN THE UPRIGHT POSITION WHILE THE BOX WASIN MOTION. WHILE IN THE POSITION HIS RIGHT HAND WAS PINCHED BETWEEN THE PIN AND THE BOX, PINCHIN G 3 FINGERS ON HIS RIGHT HAND,(BABY,RING,MIDDLE)

COMING DOWN RAMP EMPTY, TRUCK VEERED DOWN OVER SIDE OF RAMP, FRONT BUMPER WENT INTO PILE OF GRAVEL.

EE IS NOT SURE IF HE HURT HIS BACK WHILE DRIVING IN THE END DUMP OR IF HE HURT IT WHILEHE WAS TWISTING OUT THE WINDOW TO UNLOAD THE PLANT BINS.

EE IS NOT SURE WHEN THE INJURY HAPPENED, BUT CLAIMS IT MUST HAVE HAPPENED WHEN HE WAS DIGGINGIN THE BANK DOWN IN THE PIT ON SAT.MAY 16 1998. THIS INCIDENT WAS REPORTED TO ME AS A WORK RELAT ED INJURY UNTIL 1PM ON 5/21/98

TRUCK DRIVR HAULING WAST DIRT. WIND BLEW DIRT IN EYES.

THIS WAS NOT REPORTED BY EE UNTIL 4:45 PM 5-1-97. EE STATES HE HURT HIS BACK WHILE DRIVING A EUCLID 35 TON HAUL TRUCK SOMETIME BEFORE NOON ON 4-29-97. HE SAID HE HIT A HOLE IN THE HAUL ROAD. E E FINISHED HIS SHIFT (5:00PM) AND SAID NOTHING. CALLED IN 4-30-97 NOT COMING IN. CAME TO WORK 5-1-97, AND REPORTED INCIDENT AT 4:45 PM. WENT TO DR 5-2-97. OFF FOR UNDETERMINED TIME.

EE WAS ASSISTING MOVING A PIPE FROM THE PLANT THE PIPE WAS BEING DRAGGED BY A TRUCK WHEN THE PIPE CAUGHT AND ROLLED SIDEWAY ON EE FOOT

EE WAS CLEANING UNDER CONVEYOR. AN EIGHT INCH ROCK FELL OFF THE CONVEYOR, HITTING THE EE IN THE BACK.

WHILE DISLOGING A ROCK IN THE GRAVITY T.U. PULLEY EE BECAME ENTANGLED IN PULLEY WHEN BELT WAS INADVERTENTLY STARTED BY THE MUD PICKER.

EMPLOYEE DROVE DUMP TRUCK UNDER STACKING CONVEYOR WITH THE BOX UP. THE BOX HIT THE CONVEYOR CAUSING IT TO TIP OVER ON THE CAB OF THE TRUCK. EMPLOYEE RECEIVED A CUT ON THE LIP.

EE WAS ASKED TO CHECK LOOSE BELT ON POWER STEERING BY DRIVER OF HAUL TRUCK. WHILE CHECKING BELT HE CUT SECOND FINGER ON LEFT HAND WHEN DRIVER HIT THE STARTER SWITCH. AFTER MEDICAL TREATMENT HE RETURNED TO WORK WITH NO LOST TIME OR RESTRICTED WORK.

EE WAS OPERATING ENDLOADER WHEN HIT CHUCKHOLE IN ROAD.

HYDRAUCLICLY DRIVEN CONVEYOR BECAME OVERLOADED WITH MATERIAL & STOPPED. INJURED EMPLOYEE & OTHER CAME TO SHOVEL IT OFF. 2 CONVEYORS WERE DRIVEN FROM SAME ENGINE. THEY SHUT OFF VALVE TO ONE CON VEYOR. AS INJURED EMPLOYEE SHOVELED OFF MATERIAL THE CONVEYOR STARTED MOVING CARRYING EMPLOYEE & DROPPING HIM ONTO A GRIZZY SCREEN.

TIGHTENING THE HEAD DOWN A CRUSHER WITH CABLE AND LOADER. THE PROBLEM IS THAT A CHAIN INSTEAD OF CLEVIS WAS USED TO FASTEN CABLE TO PAYLOADER BUCKET. THE HEAD STOPPED, CHAIN BROKE AND THE CABL E FLEW AND CAUGHT HIM WHERE HE WAS STANDING WAS NOT A SAFE LOCATION.

CROSSING ROAD IN FRONT OF OFFICE,LOADER BOUNCED AND WHIPPED CAUSING BACK TO BACK TO BE BOUNCED AND INJURY OCCURRED.

LOADING STONE WITH A 988B CAT LOADER IN (SHOT ROCK) BUCKET HIT A BURIED ROCK STOPPING THE LOADER IMMEDIATELY THUS JARRYING THE OPERATOR. AND RENCHING HIS BACK.
