EE WAS OPERATING A 35 TON HAUL TRUCKAND PREPARING TO DUMP AT A STOCKPILE LOCATED AT THE PRIMARY CRUSHER. BEFORE HE COULD BEGIN BACKING, THE BRAKING SYSTEM FAILED AND THE STRUCK THE PIT WALL AT THE END OF THERAMP. EE WAS WERAING A SEAT BELT.
EMP CLIMBED AN ACTIVE STOCKPILE OF 1 1/4" CRUSHED STONE TO CHECK ON CONVEYOR HEAD PULLEY. EMP STUCK HIS ARMS OUT IN FRONT OF HIM TO KEEP FROM FALLING DOWN. EMP'S LEFT FINGER GOT CAUGHT IN HEAD PULLEY BEARING ASSEMBLY. EMP HAD LEFT INDEX FINGER AMPUTATED TO FIRST KNUCKLE. EMP WAS NOT SUPPOSED TO BE ON STOCKPILE.
EE'S WERE ATTEMPTING TO PULL A LOADER (THAT WAS STUCK IN SAND), WITH ANOTHER LOADER USING A CHAIN. THE CHAIN BROKE CAUSING A BROKEN PIECE OF CHAIN TO TRAVEL OVER 100 FT STRIKING EE IN LEFT EYE OVER EYEBROW.
EE OPERATED VEHICLE TOO CLOSE TO ROAD EDGE. HEAVY RAIN CONDITIONS CREATED SOFT SOIL CONDITIONS, WHEN VEHICLE PASSED AREA IN RETURNING SOIL SOLLAPSED CAUSING VEHICLE TO LAY OVER ON SIDE. ELEVAT ION CAHNGE OF APPROX. 4-6'. EE EXTRICATED AND SENT TO HOSPITAL FOR EXAM AND VEHICLE UPRIGHTED. AREA BLOCKED OFF TO PREVENT FUTURE ENTRY.
INJURED EMPLOYEE WAS HELPING CO-WORKERS REPLACE A HEAD PULLEY MOTOR SPROCKET, HE WAS STANDING ON THE HEAD PULLEY (CONVEYOR WAS DE-ENERGIZED AND LOCKED OUT) WHEN THE BELT BEGAN TO MOVE BACKWARD CARRYING HIM UNDER THE MOTOR FRAME AND A DISCHARGE CHUTE.
EMPLOYEE WAS USING HIGH PRESSURE HOSE TO WASH OFF SPILLAGE FROM CONCRETE PAD. A ROCK FELL FROM AN UNKNOWN LOCATION (POSSIBLY A BELT CONVEYOR APPROX. 15' ABOVE) AND STRUCK HIS HAND.
EMPLOYEE ATTEMPTED TO BREAK LOOSE A ROCK WEDGED BETWEEN CONVEYOR BELT & FRAME. THE BELT CONVEYOR WAS OPERATING. AS HE STRUCK THE ROCK WITH A HAMMER, IT BOUNCED BACK & THE HANDLE WAS PULLED INT O A PINCH POINT ALONG WITH HIS HAND.
HAUL TRUCK ROLLED OVER ON SIDE. THE DRIVER DID NOT FOLLOW PROPER PROCEDURE AND WAS THROWN AROUND INSIDE CAB CAUSING MULTIPLE INTERNAL AND EXTERNAL BRUISES.

EMPLOYEE WAS OPERATING FRONT-END LOADER AND WAS BACKING AWAY FROM BELT CONVEYOR BEING REPAIRED. THE RIGHT FRONT TIRE DROPPED DOWN INTO DEPRESSION IN GROUND CAUSING THE CAB DOOR TO SHUT ON HIS LEFT HAND THAT WAS HOLDING DOOR JAM.

EMPLOYEE WAS EXTENDING CHUTE WITH STEEL PLATE TO BE WELDED INTO PLACE. HE WAS STANDING ON EXTENDED HYDRAULIC BUCKET WHEN IT SWUNG TO THE SIDE. HE STRAINED HIS BACK. AS HE GRABBED ONTO CHUTE TO KEEP FROMFALLING.
EMPLOYEE WAS STANDING WITH BACK FACING ELECTRIC POWER CABLE. THE CABLE WAS BEING MOVED WITH TRUCK. THE CABLE STRUCK EMPLOYEE'S FEET CAUSING HIM TO FALL ON HIS LEFT ARM.
DRIVING 50 TON KOMATSO TRUCK IN PIT. HE HAD DUMPED HIS LOAD OF ROCK AT PRIMARY CRUSHER AND HAD HEADED TO LEDGE FOR ANOTHER LOAD. HE TURNED ON STRAIGHT AWAY AT FINISHING PLANT WHEN HE HAD A HEA D ON WRECK WITH 40 TON 3307 TEREX TRUCK DRIVER -COOK HAD STOPED HIS TRUCK ON LEFTSIDE OF RD TO GET COEK HE WAS COMING BACK ACROSS RD TO GET TO HIS SIDE WHEN THE WRECK OCCURRED.
EMPLOYEE WAS ATTEMPTING TO REMOVE A BOLT FROM A BELT SKIRT BAR WHILE THE CONVEYOR BELT WAS RUNNING. HIS INDEX FINGER WAS CAUGHT BETWEEN THE SKIRT BAR AND THE MOVING BELT.

WHILE DRIVING TRUCK ON PIT NORTH HAUL ROAD, HE NOTICED A TRCK COMING TO HIM. TO AVOID A COLLISON HE DROVE HIS TRUCK INTO A PILE OF MATERIAL, CAUSING HIS TRUCK TO FLIP OVER.
I.NAME TIGHTENING THE SAFETY CHAIN ON THE OXYGEN TANK ON REAR OF THE JEEP. DRIVER OF JEEP WAS UNAWARE THAT I.NAME WAS ON THE REAR OF THE JEEP AND BEGAN DRIVING AWAY, THROWING HIM TO THE GROUND , CAUSING I.NAME TO HURT HIS LET SHOULDER.

THE INJURED WAS3BACKING A 50 TON UCLID TRUCK DOWN AN APPROX. 10% RAMP TO AN EXISTING STOCK PILE. AS HE APPROACHED THE STOCK PILE THE TRUCKS RIGHT REAR WHEELS RAN UP ON TO THE PILE OF STONE, TH AT HE HAD DUMPE8 ON HIS LAST TRIP AS HE DID THIS THE TRUCK CLIMBING UP ONTO THE PILE RAISING THE RIGHT SIDE ENOUGHT TO CAUSE THE TRUCK TO TIP OVER ONTO IT LEFT SIDE.
THE INJ WAS STA3DING ON THE EAST SIDE OF A CONVEYOR HE USED A 3 FOOT LENGTH OF PIPE AND ATTEMPTED TO GUIDE THE BELT IN WESTWARD DIRECTION ONTO THE IDLERS. THE MECHANIC HELPER JOGGED THE BELT TO TEST THE CON5EYOR AS HE DID SO THE PIPE SLIPPED CAUSING THE INJUREDS HAND TO BE PULLED AND SUBSEQUENTLY CAUGHT BETWEEN BELT AND IDLERS
THERE WAS A EMP7Y RAIL ROAD CAR POSTIONED OVER THE COAL CRUSHER HOPPER, AND AFULL CAR BLOCKED BEHIND THE HOPPER AND A FULL CAR LOCATED 150 FEET UP THE TRACK. THE INJURED AFTER CHECKING THE CA R UP THE TRACK,3MOUNTED THE EMPTY CAR AND RELEASED THE HAND BRAKE. AS HE DID SO THE CAR STARTED ROLLING.
