While relocating piping to another jobsite an employee was walking along side a all terrain forklift when his foot got under the tire. Employee was taken to emergency room and x ray showed fracture in big toe and a laceration between the toes.

Injured was loading trucks from a stockpile with a 980H loader when a large piece of material rolled down and struck the loader arms jarring the machine. Injured initially requested medical attention, but was released with no restrictions. Injured subsequently was diagnosed with a hernia, underwent surgery on 5/21, and was unable to return for scheduled work on 5/23.
Employee strained his back while putting on a harness style seat belt.
Employee claims that his neck has been hurting because of driving a haul truck on the quarry roads. There was nothing specific noted for the cause. The pain has gradually gotten worse over the past few weeks. This was initially reported on 2/8/10 and employee asked to see a dr on 3/4/10.
Mechanic was in a JLG man-basket welding on kiln. Welding lead was secured to the man-basket and as it traveled upwards, the lead caught the toe board of the platform. While observing the snag and bringing the basket back down, the welding lead snapped causing the man-basket to jump from it's present location.
Quarry loader operator was filling in as a quarry haul truck driver was jarred around when another loader operator dropped a boulder into the bed of his haul truck.
Driver failed to depressurize his trailer. he attempted to open the dome lid when pressure blew the lid open under pressure. Driver didn't tie off harness. he was blinded & inhaled lime He fractured leg hips pelvis.
Employee was operating a CAT 966. After a shot, he was moving rock. He traveled into the rock pile and the bucket hit a toe. The loader stopped and jarred him. He was wearing a seatbelt. Diagnosed with Lumbar Radiculopathy (strain) on 4/7/06.
WHILE LOADING HAUL TRUCKS AT THE BENCH ON ROUGH GROUND THE BOUNCING OF THE LOADER CAUSED A NECK MUSCLE TO BECOME STRAINED.
THREE MECHANICS WERE CHANGING CONVEYOR BELT ON INCLINED SLOPE BELT 14-100. INJURED EE WAS GUIDEING THE BELT ACROSS THE TOP OF RETURN ROLLERS ONBOTTOM SIDE OF BELT. RETURN ROLLER CAME OUT OF HANGER WHICH IN TURN STRUCK EE ON RIGHT SIDE OF LOWER BACK RESULTING IN CONTUSION AND ALSO STRAIN OF LOWER BACK. EE PLACED ON RESTRICTED DUTY DUE TO LIFTING .
EMPLOYEE WAS OPERATING HAUL TRUCK. COMPLAINED OF PAIN IN LOWER BACK. THE MEDICAL EXAM SHOWED A BRUISED TAIL BONE. THE SEAT WAS IMPROPERLY ADJUSTED TO ALLOW FOR CUSHIONING OF RIDE.
EE WAS RIDING ON SLOPE HOIST CAR ENTERING MINE. SLOPE CAR HIT A ROCK WHICH WAS ON THE TRACK, CAUSING A SUDDEN JARRING OF THE EE. STRAIN TO BACK.
EMPLOYEE SAID DUST OR FOREIGN OBJECT FELL OFF THE ROOF OF 988 LOADER INTO HIS LEFT EYE.

STRAIN OR INJURY OTHER WAS BRINING THE HIGHSCALER BASKET TO THE GROUND TO TALK TO EE AND THE DOWN LEVER STUCK CAUSING THE BASKET TO HIT THE GROUND JARRING EE.
EMPLOYEE WAS OFF LOADING 4'X 8' X 3/16" STEEL SHEETS W/FORK TRUCK, THE SLINGS SLIPPED OFF THE FORK TRUCK CAUSING PART OF THE STEEL PLATES TO LAND ON HIS LEFT FOOT.

OPERATOR RAN HAUL TRUCK INTO A STONE WALL.
EE WAS OPERATING A LOADER TO AND FROM THE RIVER HOPPER AND ADVISED HE WAS EXPERIENCING BACK PAIN BECAUSE OF THE SEAT IN LOADER. EE'S FIRST DAY LOST WAS JUNE 16, 2000.
CAUGHT IN BETWEEN THE DUMPSTER AND FELL OFF THE FORKS OF THE FORK TRUCK. HE WAS STRUCK BY THE DUMPSTER IN THE CHEST, HAND AND LEFT FOOT. THE DUMPSTER HAD BEEN RAISED BY FORKLIFT TO ALLOW ACCES S TO WORK ON THE TRACK OF THE DUMPSTER.
EMPLOYEE WAS ENTERING PLANT AND DIDN'T HEAR THE TRAIN AS HE WAS APPROACHING THE ENTRANCE. THE TRAIN HI THE LEFT SIDE OF THE EMPLOYEE'S VAN. THE TRAIN WAS ON THE MAIN LINE, NOT ON PLANT PROPERTY.
BASKET BOOM WAS CAUGHT AGAINST TRUCK WHEN IT CAME FREE, SLUNG BASKET OTHER DIRECTION, USED HAND TO BRACE HIMSELF, STUCK HAND AGAINST RAILING, ONE DAY RESTRICTED DUTY.
EMPLOYEE FAILED TO USE PROPER JUDGEMENT AT INTERSECTION BY YIELDING THE RIGHT OF WAY TO A LOADED TRUCK. LOADED TRUCK STRUCK UNLOADED TRUCK A GLANCING BLOW.
