Employee injured when boom lift overturned. Employee was coming down for break when EE drove the lift too close to sloped area causing the lift to overturn. EE sustained serious cut to facial area and other unknown trauma.
Employee was operating a walk-behind skid steer cleaning material under a dust collector. EE ran the machine under a screw conveyor housing and when EE raised the bucket, the machine lurched striking EE's hand between the control bar and the conveyor housing. Employee was taken to OMP for medical evaluation, no fracture, but splinted thumb and placed on restricted duty
Employee was operating a 988F end loader. The air suspension compressor for the operators seat had a blown fuse. The repetitive bouncing through normal operation of the loader caused lower back pain.
Employee was operating a 988F loader. As he was cleaning the floor of the clinker building, the bucket hit an obstruction, causing the loader to come to an abrupt stop, throwing employee against his seat belt. Employee reported it the next day, was seen by a physician, & placed on restricted duty over the weekend. Employee was released to full duty on Monday
Employee was using a skid steer loader to clean material from ground in finish room. As he was scraping the bucket contacted hardened material, stopping machine and throwing employee against safety bar. Employee was wearing seat belt. Employee was diagnosed w/bruised ribs and placed on restricted duty.(No lift, push pull>10lbs.
Employee reported a chronic pain in his left hand, possibly attributed to operating rocker switch for forward and reverse on 988H Loader. Employee was sent to physician, placed on restricted duty (avoid using left hand)to see if symptoms improved
Employee was working on the south shipping load out sock. The employee who was in the basket of a manlift (Model JLG 860SJ) had completed the task and was maneuvering the basket downward. He apparently caught the basket on the load out causing the basket to move upward. He was caught in a pinch point.
When the injured climbed onto a forklift she jammed her hard hat against the roll cage and sustained a cervical sprain.
Driver was raising dump bed and truck and trailer turned over
A contract truck driver was unloading clinker, when the trailer tipped to the right,also turning the tractor over. Driver was wearing seatbelt,and climbed out of cab. Mine personnel administered first aid.Driver had small cut on right ear.EMS took driver to ER-received a few stitches in his r.ear.Note:outside driver-some info-exact DOB,experience has been estimated
Employee removing rock from screen deck in Quarry, when rock from hopper above the screen came down and hit thumb on left hand, catching it between two rocks and cutting it.
1st Employee was walking out of the north doorway of the mill basement through the hanging curtains. Another employee operating the New Holland skid steer was driving into the basement at the same time. 1st employee was struck in the left leg by the bucket of the skid steer loader. Employee complained he had knee and back pain.
Loader Operator was trying to shift the loader from reverse to forward. Operator's hand slipped off the shift lever and struck the dash of loader. This caused a contusion on the left pinky finger requiring no medical treatment on 1/5/09. The contusion became infected on 1/7/09 and was sent to the clinic. The clinic drained the wound and prescribed antibiotics on 1/7/09.
Employee trying to close bulkhead on loaded boxcar. He was using drift bar to align holes with bulkhead and receiving bracket. Another employee using forklift to push bulkhead in. Drift bar slipped and struck employee's thumb on left hand.
Employee was assisting in high reach at A-frame area. As high reach was ascending, high reach basket bumped the hand rail and shook the basket. Right knee of employee gave out and was injured.
EMPLOYEE WAS CARRYING 2 PALLETS OF REFRACTORY BRICK ON FORKLIFT. FORKS WERE ELEVATED WHILE HE WAS BACKING UP WITH LOAD. FORKLIFT TILTED FORWARD DUE TO HEAVY LOAD CAUSING BRICKS TO SLIDE OFF FORKS AND EMPLOYEE WAS THROWN FROM INSIDE FORKLIFT CAB TO THE GROUND. EMPLOYEE WAS NOT WEARING SEATBELT INSIDE FORKLIFT.
Employee was assigned to clean 3rd deck of preheater area. While operating the fork lift, the employee placed left hand between the hopper and the forks which caused the dump part to kick back and severed his index finger (L) below the 1st joint area.
The injured was checking air flow on the Cat 988 loader when he contacted the unit's operating fan, lacerating his right ring and middle finger. Required stitches and pain medication; no lost time.
Working on the vac truck, hose sucked up chunk of rock and stuck to floor. Hose tangled up employee and threw her to the ground twisting her ankle causing her to land on her wrist.
EMPLOYEE WAS IN GOLF CART RETURNING TO LAB AFTER COLLECTING SAMPLES. WHILE DRIVING UNDER #2 KILN FLAKES FELL INTO LEFT EYE. OINTMENT WAS PRESCRIBED BY DOCTOR AFTER EXAMINATION AT HOSPITAL.
EE WAS USING VAC TRUCK IN THE ABC TUNNEL. HOSE CAUGHT IN PILE AND JERKED FORWARD PUSHING HER THUMB BACKWARDS.
Employee was cleaning clinker off of the reclaim grates when the lip of the loader bucket struck the concrete around the edge of the grates. The loader came to a sudden stop and her right hand struck the gear shift jamming her right shoulder. ***THIS IS REPORTABLE BECAUSE SHE WAS SCHEDULED FOR PHYS. THERAPY.**
EE was using the Vac Truck in the apron. As he was pulling the hose around the tail wheel, the hose went into a pile snapping his wrist and thumb forward. He had a hold of the stick attached to the hose. ***This was not reportable until 05/30/07 when he had surgery.***
EE was hauling clay in the CAT 777 haul truck. He took the truck on top of the clay pile and backed towards the southwest corner of the clay pile. Either when the truck stopped or when he went to raise the bed the face sloughed off under the weight of the rear wheels causing the truck to go down the face tail first overturning anc coming to rest upside down.
Accident occurred to delivery truck driver. Our EE unloading pallet w/compressor w/forklift off boxtruck. EE positioned forks under pallet, warned driver to stay clear. Driver moved out of way; pallet & compressor rolled off forks, struck driver & was found on ground. Driver complained of pain in right leg, contusion to forehead, abrasions to back. No loss of consciousness.
Two employees were installing return roller on the clinker cross belt. The victim was holding a board to support the belting. The other employee was using the manlift and was maneuvering the basket to better access the roll. The victim's arm was wedged between the basket and the board that was fixed to support structure. *reportable 4/28/06 when she was sent for phys. therapy.*
Employee was cleaning in front of 2C mill with the suck truck hose. She was cleaning up a large amount of material. The hose picked it up all at once causing the hose to jump and strike her in the leg.
EMPLOYEE WAS ADJUSTING THE AIR FLOW FROM THE LOADER AIR CONDITIONER. SHE DID NOT REALIZE THAT THE GUARD WAS MISSING AND HER FINGER GOT CAUGHT IN THE FAN BLADE.
Vacuum truck hose became clogged. ee looked up in the bed of the truck to see if the hose was clean. the hose was hammered and she got covered in dust. when she took her hat off dust went into her eyes.
Employee believes that his back pain is caused by operating the crane.
CLAIMANT STATES HE WAS OPERATING YARD LOADER, DIGGING HARD MATERIAL UP & FELT PAIN IN BACK, NECK& SHOULDER BLADE.
EE WAS WALKING TO THE UPPER LAB WHEN TRUCK YARD PICK UP TRUCK EE MAKING HIM FALL BACKWARDS. TRUCK DRIVER PUT ON BRAKES BEFORE STRIKING EE BUT IT WAS A LITTLE LATE.
INJURED WAS OPERATING ROAD SWEEPER ON PUBLIC ROAD IN FRONT OF PLANT WHEN A PRIVATE VEHICLE STRUCK THE SWEEPER FROM THE REAR. SWEEPER WAS DAMAGED REQUIRING REPAIR.
EE WAS OPERATING 980 LOADER AT PLT. II. BEGAN FEELING PAIN & DISCOMFORT IN RIGHT LOWER BACK.
EMPLOYEE WAS USING 950E LOADER TO SCRAPE AWAY MUD FROM THE ROADWAY WHERE CEMENT TRUCKS BLOW OFF CEMENT INTO PLANT SILOS. EMPLOYEE HIT SOMETHING HARD IN THE GROUND WITH THE BLADE OF LOADER BRAK E CAUSING AN ABRUPT STOP AND EMPLOYEE STRUCK HEAD ON STEERING WHEEL, CAUSED CUT ON LEFT SIDE OF FOREHEAD.
EE BUMPED THE WALL WITH THE FORKLIFT, KNOCKING DOWN SOME DUST OFF A BEAM. HE LOOKED TO BE SURE NO BIG CHUNKS WERE ABOVE AND GOT SOME OF THE DUST IN HIS EYES.
EMPLOYEE STATED THAT HE WAS LEARNING TO DRIVE THE HAUL TRUCK AND WAS RIDING IN THE JUMPSEAT BESIDE DRIVER IN CRAMPED AREA.
EE WAS TRAVELING TO NEW ASSIGNMENT WHEN HE PASSED A FORKLIFT IN THE PACKAGE WAREHOUSE FORKLIFT OPERATOR FAILED TO COMPLETELY CHECK BEHIND HIM BEFORE BACKING UP & TURNING, EE WAS STRUCK BY FOR KLIFT BEHIND RIGHT FOOT
CLAIMANT STATES HE WAS MOVIN GSKIDLOADER OUT OF BASEMENT AFTER DROPPING OFF PART, CAME UP RAMP HALFWAY LOADER STALLED OUT COULD NOT RESTART, LOADER WENT BACK DOWN RAMP HITTING #2 RAW MILL PIER HARD.
EE WAS OPERATING A SKID LOADER TO PICK UP BALLS TO CHARGE A MILL. EE HAD TO OPERATE THE MACHINE AGGRESSIVELY TO PICK UP THE BALLS. EE COMPLAINED OF A SORE KNEE AND IT WAS REPORTED TO HIS SUPER VISOR ON MONDAY. HE THOUGHT HE DIDN'T NEED MEDICAL ATTENTION UNTIL IT STARTED TO BOTHER HIM INCREASINGLY ON 5/3/00.
EMPLOYEE WAS RIDING IN A PICKUP TRUCK WITH SUPERVISOR WHEN THEY HIT A BUMP IN THE ROAD. EMPLOYEE JARRED HIS NECK.
