Employee was getting out of the Diesel scoop when they put pressure on right knee it buckled causing immediate pain around the knee cap.
Employee came out of the restroom and stepped the wrong way and slipped and fell hurting a hand.
Employee tripped hitting EE's knee on mine floor. Employee was transported to get x-rays and they was negative. EE received a full release. Two shifts later EE went back and was referred to a specialist, on March 23rd EE was diagnosed with a torn meniscus. Due to covid-19 and furloughed we are just returning since March 20th.
Employee stepped off the push arm of a dozer and right foot slid in mud and hit a rock breaking a bone in right foot.
An employee fell and struck a belt tailpiece and cut their arm. Sutures were required and they returned back to work full duty.
Employee slipped and hit right knee on bolt while working in grid box of 830E. The employee spent the majority of the afternoon working in a crouched position in the grid box. When they got up the knee was stiff, hurting and swollen.
Employee stated that EE was in the process of checking the section feeder and was walking around it when EE stepped into some muddy conditions and twisted EE's right knee.
Employee stated EE was in the process of hanging fly pads in the #4 entry, on the Unit 2 Section. In the process of EE hanging a fly pad from the 6" ladder, the ladder tipped over, and EE fell to the ground. This caused EE to land on EE's head, shoulder, and back. Note: EE was standing on the bottom 2nd step of the ladder when this incident occurred.
WET, MUDDY, UNEVEN BOTTOM. DRAGGING CURTAIN, FELL GRABBED A RIB BOLT TO CATCH SELF AND SEPARATED BICEP.
EE stated was in the process of assisting with a belt move, EE further stated was attempting to pick up a 60 inch top roller. While attempting to pick up the roller, EE slipped and fell causing right knee to land on a piece of belt structure that was laying on the ground.
EE stated that EE and another employee were attempting to hang continuous miner cable on the Unit 1 Section. In the process of the EE lifting the cable, EE slipped, and felt a pain in lower back.
Employee slipped while descending a slope, falling on the chainsaw EE was carrying, cutting the left upper arm.
Employee was setting a dewatering pump along a beltline. While trying to get the pump into the correct position the latch on hook broke. The pump moved causing the employee to slip and employee's left knee struck the pump.
Employee stated employee was traveling the in #6 entry just outby the section power center. Employee stated employee walked over a small ledge in the mine floor, and hyperextended right leg, causing employee to fall to the ground injuring right knee.
Employee was starting equipment prior to other employees running the equipment. EE had started a D11 dozer and when EE stepped off the platform onto the track, EE's foot slipped off the wet track causing left ankle to roll to the side causing a severe strain.
Employee stated was walking back to the section in the #7 Entry (Return) when employee stepped in a hole and felt a pain in back.
Employee stated EE was attempting to pull the oil feel hose out to put oil in the MRS when the hose came loose employee fell against the plow of the MRS hurting EE's ribs
Employee stated that EE was in the process of pulling cable off of the #362 Shuttle Car when EE slipped on slick mine floor causing EE to twist EE's right knee. Employee stated that EE's knee started to tighten up immediately.
Footing got stuck in mud carrying a drill reamer. Fell due to feet stuck in mud and injured lower back.
Employee stated they were walking on section when they stepped into a hole and pulled something in their buttocks. Employee sought medical treatment and was placed off work.
Employee was climbing onto an excavator, standing on track and went to twist foot around. EE's body did not turn and EE felt something pull or tear on left leg inside of knee.
Employee was cutting brush/tree saplings on an impoundment dam and slipped and fell. EE sustained a cut to left leg/shin that required sutures.
Employee stated was attempting to service the CM when the employee slipped on a slick rock and leg slid under the back of the machine. This caused pain in left knee.
FOOT CAUGHT ON CENTER BLOCK AND FELL ON KNEES STOMACH AND CHEST KNEES AND ELBOWS SCRAPPED AND BACK HURTING
Employee stated was attempting to locate a rattle on the 992 G End loader when employee stepped on a loose platform. This caused employee to lose balance and fall causing pain in right shoulder, right arm, right leg, and lower back. Employee was taken off work by a physician.
Employee stated EE was attempting to cross over a 6" water line when EE's rescuer got caught on a piece of guarding causing EE to fall backwards off the waterline injuring EE's back. Employee sought medical attention and was diagnosed with a fractured L5, employee was placed off work.
OUTSIDE OF GUARDSHACK RIGHT FOOT SLIPPED ON GRAVEL HURT BACK AND KNEE.
EE stated EE was in the process of climbing off a scaffold when EE's foot slipped, causing EE to strike the ground. On 10/08/2019, the employee was taken off work. This incident is being reported as an LTA.
Employee was standing on steps of 16 M grader cleaning windows and the door came back on EE knocking EE off the steps and hitting EE's head on corner of grader blade. Required 12 staples.
Employee stated was rock dusting #8 rt entry when foot struck a rock causing employee to stumble while trying to catch self. Employee states during this motion employee strained lower back and left knee. Employee sought medical treatment on 5/21/2019 and was placed under doctor's care.
Employee was exiting a 4400 rock truck and slipped coming down the steps and fell breaking a bone in left hand.
Employee stated ee was traveling across the section when ee stepped on a slick spot where the shuttle cars were traveling, causing ee to slip and twist left knee. Employee sought medical treatment and was placed off work due to a strain.
Employee was exiting the mantrip after arriving to the slope bottom when EE placed EE's left foot on the barrel connected the throw arm of the switch causing EE's ankle to roll which resulted in a fracture of the distal end of the left tibia. This incident is being reported as a Lost Time incident.
Employee stated EE was working to remove suction line on the bottom floor. Employee stated EE was standing in coal fines when EE turned to reach for a wrench but EE's foot remained stuck causing EE to twist EE's knee and ankle. Employee reported incident on 5/8/2019 and sought medical treatment. Doctor placed injured employee on restrictions due to sprain ankle until 5/13/2019
Removing tire from truck tire went to fall jumped out of the way landing wrong on foot.
Dozer operator stepped out of equipment cab onto tracks, slipped and fell on track fracturing 3 ribs.
Employee slipped and fell in an area with a slight incline which resulted in a dislocated right elbow. Employee received medical treatment and returned to work on EE's next scheduled shift without restrictions.
Employee was firebossing the 3 south belt. Went to step over a twenty-four inch piece of poly pipe that was twenty-four inches off the mine floor. Foot slipped in mud causing them to strike the other foot against a 6 inch crib block, resulting in a fracture to a metatarsal bone.
Employee stepped off of a conveyor belt tailpiece and felt a pain in EE's back. EE started missing work on 4/22/2019
Employee was dismounting a bolter when their feet became tangled and fell to the mine floor, injuring their back and right shoulder. Employee sought medical treatment on 4/10/2019 and was taken off work under doctors care.
Employee stated EE was crawling on top belt running a shiv rope when the belt slid down between rollers causing EE to fall to the mine floor. Employee sought medical treatment and was diagnosed with a strained back. Employee was given days away from work. Note: Belt was locked & Tag
Employee stated EE and another employee backed the #420 Roof Bolting Machine out by the No. 7 face after bolting it on the Unit 2 Section. The employee stated EE was going to walk around the machine to check EE's dust box. In the process of walking around the machine EE slipped and fell landing on EE's shoulder resulting in a shoulder sprain.
Employee stated that EE was standing on top of the tailpiece hanging strings to set the tail piece. As EE leaned over to tie the string EE grabbed a monkey face to brace self. When EE grabbed the monkey face it spun causing EE to stumble back and EE's foot became caught in between the frame of the tail piece injuring EE's right ankle.
Employee stated was walking backwards as EE was backing the miner up when EE twisted and injured the knee. Employee sought medical attention and was placed off work under doctors care.
Employee slipped on a patch of ice while walking to a company vehicle which resulted in a sprain/strain injury to the left knee. On 5/22/2019 the employee was placed off work due to a surgery that was performed at which time this became a Lost Time Incident.
Employee stated was climbing in the #382 Shuttle Car on the Unit 1 Section. In the process of getting into the shuttle car EE's left knee gave out and EE fell to the ground.
Employee was walking diversion ditch and was climbing onto the berm and slipped and fell on right thumb.
Employee stated EE was helping remove 60" belt structure from the longwall belt line. As EE was pulling a piece of top structure the employee's feet slipped causing EE to fall backwards. The structure fell on EE's feet twisting EE's left ankle.
