EE was coming down from the loader when EE thought EE was on the last step of the ladder and fell on EE's right side injuring EE's right wrist. Weather conditions were very windy that day.
EE noticed the forklift EE was operating was running low on propane, so EE decided to replace the almost empty propane cylinder. As EE was putting the empty propane cylinder in the slot, EE's right ring finger was caught in between two propane cylinders and felt a sharp pain on EE's finger. EE ended up with a deep laceration on EE's right ring finger.
During the process of replacing clips (brackets) in the Filter Press, EE was accessing the area behind the door to install on clips, when they suddenly twisted their right ankle.
Miner sustained a shoulder injury while using a sledgehammer to hit a pipe to clear a plugged line.
EE had clocked out and was in the contractor parking lot at EE's car. EE removed EE's boots and banged them together to shake off excess mud, dirt, and lime, which got into EE's left eye.
Employee is currently in the ICU unit. Crew was installing cross beams and noticed beams were to close together. Foreman went back to scaffolding and tied off to back rail, tried to pin and move beam over when structure clip slipped off of chime and employee fell with the beam. Employee was tied off to the structure.
The IE was walking down a flight of steps. On the last one they stepped awkwardly causing them to roll ankle. The X-ray came back negative and the employee was diagnosed with a sprained ankle. A follow up appointment on 2/13 resulted in restrictions.
Loadout operator was starting the process of opening lids of a OTR truck at that time EE twisted ankle and fell.

Employee (EE) was walking around a pilar to collect tools for a job when EE's feet slipped out from under EE and EE fell to the ground on EE's left shoulder. EE was seen at the clinic and diagnosed with a left shoulder strain and a left knee strain and released with restrictions

Employee experiencing pain and loss of strength in both hands. After initial review by Occ Med department at local hospital, employee was referred to Orthopedic Surgeon for review of symptoms. Surgeon found that employee had carpal tunnel in both wrists recommended surgery. Surgery was performed on the right hand on 3/20/26 with surgery to follow on the left hand on 4/3/26.
Employee was walking along the combing on a barge that was brought in and rolled EE's ankle. At the time there was just some swelling and no further treatment. Employee stated in March EE was still feeling pain periodically. An MRI was ordered and referral to an Orthopedic. The Orthopedic surgeon provided a prescription for Physical Therapy on 4/6/2026 to strengthen ankle.
Employee was cleaning a chute and their hand was jammed into the side of the chute opening, causing a cut on their right index finger. The employee received 4 stitches at the ER. No restrictions were issued and returned to work full duty.
EE and cohorts had lowered the counterweight following a repair from the previous day. EE went up in a manlift to observe operation when they became entangled in moving machine parts of the 48-inch conveyor. EE was degloved from the shoulder to the forearm, has multiple fractures including wrist, thumb, scapula and shoulder. This injury will require skin graphing as well.
Employee was walking in the work area when EE tripped and fell over a pipe that was in EE's path of travel.

Employee was hydroblasting a scrubber and material fell from the wall hitting employee in the back of the head. This resulted in the employee receiving 8 staples in the head. Employee was wearing a hardhat and material knocked hardhat off.
ROOT CAUSE - cylinder punctured and cracked the cylinder housing causing the trailer to drop down approximately 4-5 feet without the cylinder engaged. Worker was injured due to a safety violation (not wearing a seatbelt at all times). CONTRIBUTING FACTOR - normal wear and tear from daily use.
The employee was installing brackets for form scaffolding. While EE was trying to shift an alignment rail, EE had to loosen the alignment bolt. While hitting the bolt, the hammer ricocheted off the bolt and into EE's left hand EE had placed on a 4x4 form brace.
Scraping / cleaning hydrator product chute / duct. After initially seeing Occupational Medicine department, with no pain not resolving, employee was referred to an MRI and saw an Orthopedic surgeon on 4/14/25 and was put on work restrictions and scheduled for surgery.
While moving equipment, the EE pick up an electrical box and swung it to the opposite side of the room. While swinging the box, their glove became stuck. The momentum carried them to the wall, and they jammed their neck. EE was put on restricted duty on 2/12/25.
Employee was using a Front End Loader to feed material to the plants. As EE backed up to get another load of material, the employee backed into another parked front end loader. The employee received a work restriction due to a muscle strain to EE's neck and right shoulder from the impact. Employee is due back at the doctor on 1/17/2025.
While walking across a snow-covered, icy parking lot at the mine, an employee was taking EE's time and shuffling EE's feet due to the slippery conditions. Unfortunately, EE lost EE's footing and fell, landing heavily on EE's left elbow.
Tripped over pipe while carrying samples. Worked without restriction until surgery on 4/23/2025.

Injured employee was opening hatch lids on top of trucks when they strained their back. Employee was working with case management for remedies when they decided to be seen by a doctor which then elevated case to reportable.
On 11/13/24 the miner reported pain on the right knee as a result of a workplace incident that occurred on 11/01/24. Miner was sent for medical evaluation, was placed on restricted work activities and referred for physical therapy.
While operating a Kubota RTV, the vehicle hit a pot hole and jolted the miner who sustained a back sprain.
Employee was walking along rail line and stepped in a small hole. The employee never fell to the ground but did injure their left knee. The employee went to a clinic on 09/24/24 and received work restrictions.
One LNA EE was operating forklift and three iTEC EE's were rigging and positioning new MCC section into the Extruder MCC Room. iTEC EE was positioning dolly when rigging slipped off the forklift forks causing contact between MCC section and iTEC EE. iTEC EE sustained injury to right knee.
Miner was on top of the rail car opening the lids and strained ligaments of lumbar spine.

On 08/01/2024 miner was changing a drive belt using a wrench when injury occurred to shoulder joint. On 08/06/2024 miner was issued restricted duty by medical clinic.
IE was raking scale from kiln #4 when EE felt a sharp pain in right elbow. When IE returned to control room, IE noticed numbness in right hand. IE was diagnosed with strained right elbow. IE was prescribed PT on 8/9/2024.
