Employee was applying the seat belt of the equipment EE was operating. As EE pulled the seatbelt it locked up and caused the employee to strike self in the mouth with the metal portion of the belt, chipping a tooth.

Employee finished changing the brake calipers on the hose reel of the highwall miner. EE was retrieving a fitting that was laying on the hose reel frame when EE slipped and fell. EE tried to catch self with left hand and EE's fingers went through the metal grating injuring the hand and fingers. EE also struck EE's head as EE fell resulting in a small cut above the right eye.
While traveling in a rail mounted mantrip, miner injured head when the mantrip struck a broken rail causing the machine to come to a sudden stop. Injury was a laceration requiring 6 staples.

Employee states that while operating the continuous mining machine, a piece of debris was blown into right eye by the machine's scrubber fan. Employee finished shift and went to ER for treatment. Employee's right eye was flushed out and was treated with antibiotic eye drops.
Employee was raising low boy trailer deck by using the hydraulic levers located near the cab end of the trailer. A chain was hooked to the gooseneck of the trailer to secure it, the chain came under tension while adjusting deck causing clevis to break and struck employee in knee. Operator was notified of PT on 07/17/2019.
Employee noticed something wrong with eye on way home from work. When ee woke up it was irritated and ee notice something stuck to eye. Employee received medical treatment for this injury at an optometrist office on 10/05/18.

In a dip on old #6 Belt Track, the employee was a passenger in the first of two man buses. As the first man bus went down the dip and was topping over, it flashed its lights for the second bus to proceed, when the first bus stalled out and rolled back down causing the second bus to hit it. This employee began missing work on 06/04/2019.

The employee stated that as they were pushing a cable up to the top they felt a pain in the lower abdomen area. The employee finished this shift and the following shift. The first day missed was 8-22-2018.

Employee was installing lower head bolts on a continuous miner. When aligning the bolts, ee was using a hammerer. While swinging the hammer the employee slipped and struck left index finger. The employee continued to work and reported to the doctor on 9/11/2018. After and x-ray was performed, it identified a minor fracture to the left index finger.
EE squatted down to look for an oil leak on the shuttle car, when a piece of draw rock fell striking EE on the hard hat and shoulder. This caused EE to cut EE's forehead. Required seven sutures.
