On 11/20/2025 at Melvin Stone, employees were removing the bucket from EX1200 (22030002). When the final pin was pressed out, the stick settled and caught EE's left hand between the stick and bucket cylinder. First aid was given and EMS transported EE for treatment. No fractures reported.

Employee smashed finger in the door while exiting the crusher tower after operating rock breaker.
The employee was attempting to avoid being stung by a bee and hastily exited from a catwalk down the stairs. EE missed one tread and landed awkwardly causing knee pain. EE denied medical treatment. On Monday 6/23/2025 the employee did notify management that EE would seek medical treatment.
Employee was laying on ground for maintenance on wash plant. When employee was getting up off the ground they felt discomfort in their left knee when standing up straight.
Employee was holding the grinder in one hand. Employee was looking at the screen and the grinder had drifted down touched the screen and then skipped over cutting through employee's glove.
Changing wear bars in head box. When the bolts were loosened the bar flipped hitting the thigh of the operator. The employee was given an Ibuprofen injection by the attending physician at the ER.
EE was walking on the ground and had a fall from the same level. EE sustained a broken rib on right side. Restricted duty for 2-3 weeks.
Attempting to reverse the direction of a conveyor motor.
Employees were putting on the hoist cables for the dredge bucket while pulling cable up. Employee was feeding the cable over the drum to other employees. The cable had pinched employee's hand/finger into the hoist drum cutting through the gloves and cutting their right index finger.
Contractor drill rig caught fire. 2 extinguishers deployed. 911 called. Fire department extinguished.
Employee was operating a propane fueled forklift. The engine stopped. The employee thought the propane tank valve was closed. While attempting to unscrew the supply line, EE's left hand and fingers were burned by the cold propane release.
Employee was removing cable clips between a 40 mm and 3/4 cable the 3/4 cable was broke and was frayed {had cable wires sticking out} while removing cable clips it allowed the 3/4 cable to slide down and cut their wrist.
Employee had locked and tagged out the crusher to adjust the curtain. The rotor was on the "up-stroke" and the employee pushed the rotor over with their hand rather than using a board or steel bar and EE's glove got caught between the rotor and housing. This caused a cut to EE's right thumb requiring 6 stitches.
Individual was cleaning a catwalk by oversized collector on plant# 2. When EE stood back up, EE stepped on a rock and rolled ankle falling to the ground. Sought medical treatment and therapy on own. Reported occupational injury on 2/6/24. Had therapy and requires surgery on 5/29/24. Will have restrictions post surgery.
The employee slipped and fell off of a conveyor belt approximately 5 feet to a concrete floor.
Employee was handling a steel plate when employee slipped and lost control of the plate and caught a finger between the plate and the deck of the dredge.
Employee was engulfed when they entered the primary feed hopper/bin to clear clogged material.
Employee was helping to install a ladder that had a hinged section. When one of the bolts was removed the upper section hinged down knocking their hand into the side of the ladder and pinched their right thumb.
