EE was pulling transloading hose and strained right shoulder. Work Care agency was called with permission to reported back to work. EE felt pain in right shoulder and made a clinic visit. The clinic assessed EE shoulder and found partial displacement, reset, and recommended a specialist.

EE was assisting a mechanic testing the pins on the skid steer bucket. While the bucket was raised in the air the TM pulled and shook the bucket when EE felt an abdomen strain. The result from a previous coughing episode of exertion and the extension of EE's arms pushing and pulling on the bucket resulted in a hernia. EE reported to a physician which resulted in surgery.

EE was sliding a dunnage board across an empty flatbed trailer when the board caught an unknown obstruction causing the board to recoil back into the lip of the EE which cut the bottom lip requiring 4 stiches. The EE was released to full duty.

Employee was checking the oil on the dozer when the high wind gust blew the compartment door closing on left hand causing a small laceration on the third knuckle of the pinky finger. This required three stitches to close wound. Employee was released to full duty and no medications were prescribed.

Team member stepped in a rut at the facility and twisted an ankle. The team member was taken to the ER and diagnosed with a sprain. At follow up appointment MRI was prescribed and determined a hard boot should be required on March 5, 2020. The team member continues to be able to perform work duties and has not missed any work.

Approximately 3:30pm, Labor Finders received a phone call that EE was not breathing, had been administered CPR, and was being transported to a hospital. An employee from Labor Finders met with Savage Management at the hospital and was told that EE had passed away. The medical professionals suspected a massive heart attack.

While dumping coal stuck in bed of trailer and truck and trailer over turned.

Coal was frozen in bed. Temperature was 10 degrees. Trailer was not completely level when attempting to dump. Truck tipped over when bed was raised.

Employee was helping position the crude oil rack on the next rail car to be loaded. When the rack was close to being in position, employee stepped on a yellow concrete block to signal the driver to stop. Employee slipped off the yellow block and fractured his left ankle. Employee was driven to the hospital and treated.

EE was holding a wrench, another EE was tightening with 3/4" impact gun; Another EE was swinging a 12 lb. hammer when the head of the hammer fell off and struck EE's hard hat on the right side of the head.

EE had just clocked in and had sat down on a chair in the break room. He then passed out and fell to the floor striking his face when he landed. This caused a 3/4 inch long laceration across his upper lip that required stitches. He was cleared to return to work with no restrictions.

Individual was picking up a bag of ready mix when he felt a sharp pain in his lower back. That caused him to let go of the bag and feeling sharp pain in his back. He was taken to the office were he was picked up and taken to the local Emergency room for observation. It was discovered that the individual had had an earlier back injury in a car accident in 2005.

A contractor (driver) was unloading on a grizzly. He got out of his truck to restart the grizzly and was not paying attention when he walked too close to the grizzly. He slipped and suffered an abrasion to his right leg. He suffered no impairement, just soreness for a few days.

Employee was opening a bag of floor dry with the help of the injured worker. The blade he was using slipped, went through the glove of the injured worker and cut his thumb. He received three stitches.

Worker was flopping the gate on the #5 tube to go to the #5A tube when the come-a-long chain came off and hit him in the lip. He was cut and recieved stitches. Returned to work. No impairment.

Driver, who works for another Savage operation, was at coal terminal unloading coal. She exited truck to check fuel level in left side tank. Driver removed fuel tank. The fumes ignited, burning the driver's face and right hand. Off-work, still in treatment. Cause still unknown.

Contractor mechanic was stepping from a D11 track to dozer arm to get down from machine when his foot slipped, causing him to fall to the ground and hit his shoulder and head. He bruised his shoulder (rotator cuff). No loss time-light duty.

EE was delivering coal and got stuck-successfully towed. Unhooked cable, noted tire on cable. Climbed back into truck when operator unexpectedly moved forward. Jostled about hitting right leg and right shoulder. Iniitally reported no treatment needed. Went to hospital later. Got tetnus shot and pain medication.

The employee was attempting to cut a small metal strap on some building siding using 4' grinder kicked back and struck the EE's right arm. The laceration required 5 sutures to close.

EE was assisting in attaching a chocker to a piece of equipment that was going to be installed As the employee climbed into the equipment he slipped and bruised the back side of his left leg behind the knee.

Moving grizzley with loader, grizzley shifted traping ankle.

Dark night- sleet- checking coal in front trailer (used ladder on rear trailer) stepped onto tongue and slipped and hit cheek on tongue of trailer.

The manlift was parked at noon on 8-7-04 and was observed to be on fire at 3:00 pm on 8-8-04. No one had operated nor been near the manlift since it had been parked.

CLEANING WINDSHIELD - LOST BALANCE AND JUMPED TO THE GROUND (ABOUT 7 FEET). THOUGHT HE HAD ONLY SPRAINED ANKLE AND FINISHED SHIFT, REPORTED AND WENT TO DOCTOR THE NEXT DAY.

EE WAS PUSHING COAL INTO FEEDERS UNDER COAL PILE-COAL BRIDGED AND EE BACKED INTO HOLE WHEN THE BRIDGE FELL IN. HE FELL BACKWARDS 40-60 FT AND THEN COAL COVERED UP THE CAT.

WHILE CHECKING COAL SAMPLER HE PLACED HIS HAND ON THE V BELT GUARD AND HIS FINGERS SLIPPED BEHIND THE GUARD INTO THE V BELT.
