WHILE LOADING AN OLD IDLER IN THE BACK OF A PICKUP TRUCK THE VICTIM WAS SCRATCHED ON THE LEFT ELBOW. THE ELBOW BECAME INFECTED ON THE 23RD OF JULY.
EMPLOYEE WAS WORKING ON A FEEDER, WHEN HE SLIPPED AND FELL AGAINST THE FEEDER. HE WENT TO THE DOCTOR THAT MORNING FOR X-RAYS. HE WORKED THE NEXTSHIFT ON LIGHT DUTY.
THE EE WAS CLEANING OUT GRIZZLY BARS W/ A SLEDGEHAMMER WHILE SWINGING THE HAMMER THE EE STRAINEDHIS BACK
EMPLOYEE WAS REMOVING A CAP OFF AN OLD BEARING HOUSING. THE EMPLOYEE RAISED UP AND HIT HIS BACK ON A PIECE OF ANGLE IRON. HE WENT TO THE DOCTOR AND RECEIVED A SHOT TO PREVENT POSSIBLE TETANUS.
THE EMPLOYEE ON DATE OF THE INJURY PICKED UP ON AN IMPACTOR HAMMER. HE STRAINED HIS SIDE BY DOING THIS TASK AFTER SEVERAL MONTHS WAITING FOR IT TO IMPROVE HE WAS SENT TO A BACK SPECIALIST ON JULY 12.
THE EE WAS HAULING MATERIAL OUT OF THE PIT ON THE MAIN HAUL ROAD. THE DRIVE SHAFT FAILED ALLOWING THE TRUCK TO ROLL. BACKWARDS. BEING UNABLE TO STOP THE TRUCK IT STRUCK A BERM RIPPING OFF THE REAR AXUSS. THE TRUCK THEN SLID OVER THE HIGHWALL AND FALL 88' FEET. THE DRIVER ONLY HAD BRUISES.
EE WAS WELDING. A LARGE ROCK SLID OFF HOPPER WALL. STRUCK EE ABOVE KNEE ON LEFT LEG. WENT TO ER, XRAYS TAKEN. PAIN PRESCRIPTION GIVEN.
THE EE WAS WORKING A HAND CRANE FOR A RAM PUMP HE STRAINED HIS SHOULDER WHILE OPERATING THE CRANE. HE THOUGHT IT WOULD GET BETTER SO HE DIDN'T REPORT THE EVENT UNTIL THE 24TH HE WAS SENT TO DR WHO PUT HIM ON RESTRICTED DUTY
STRAINED BACK WHILE LIFTING A GRILLE FOR A HAUL TRUCK INTO PLACE.
FRONT END LOADER OPERATOR CB RAWLS WAS ALLEDGEDLY RETRIVING FUSES FOR ANOTHER LOCATION OUT OF THE ABANDONED SF-4 PLANT COMPRESSOR CONTROL BOX AT APPROX. 745 A.M. AFTER THE FUSES WERE OUT AND O N TOP OF ANOTHER ADJOINING CABINET, HE WAS IN FRONT OF THE 480V, DISCONNECT WHEN THE INTERIOR LOCKING NUT FOR THE SECONDARY WIRING SUDDENLY FELL ONTO THE ENERGIZED PRIMARY FEED CAUSING A PHAS
THE EE WAS MOVING SOME WOODEN BLOCKS USED AS CRIBBING WHEN HE FELT A POP IN HIS BACK. HE REPORTED IT BUT DIDN'T GO TO THE DR BECAUSE HE FELT NO PAIN. A FEW DAYS LATER HIS BACK STARTED TO HURT. HE WENT TO THE DR TO GET CHECKED OUT ON. AT THIS TIME HE WAS PLACED ON RESTRICTED DUTY.
EE SLIPPED ON ICE AND STRAINED GROIN. TAKEN TO ER. PRESCRIPTION GIVEN. OFF WORK 1/4, BACK 1/5.
THE EMPLOYEE WAS LIFTING BATTERIES OVER A RAILING, WHEN HE PULLED A MUSCLE IN HIS BACK.
EE WAS CLEANING IN THE SURGE TUNNEL USING A SHOVEL AND PUTTING THE MATERIAL ONTO THE BELT THE SHOVEL CAME INTO CONTACT WITH THE BELT CAUSING THE SHOVEL TO KICK BACK STRIKING HIM IN HIS RIGHT E YE. HE WAS WEARING HIS AFETY GLASSES BUT THE LENS BROKE AND GLASS GOT INTO HIS EYE. THERE WAS NO DAYS LOST.
EE WAS ON CATWALK OF CONVEYOR WORKING. BOLTS SHEARED IN THE MIDDLE OF CONVEYOR, DROPPING IT AND THE EE APPROX 6 TO 8 FT TO THE GROUND.
EE WAS REPALCING THE 1200 AMP FUSES TO THE EAST MAIN 480 VOLT DISCONNECT FOR THE SHELL PLANT, AFTER A 4 DAY SHUTDOWN PERIOD DUE TO HURRICANE GEORGES. PRIOR TO THIS HE HAD MEGGERED THE BOTTOM A ND LEADS OF THE DISCONNECT, AND EVERYTHING APPEARED DRY AND TESTED CLEAR. WITH THE DISCONNECT IN THE OFF POSITION, HE WAS USING HIS LEATHER/INSULATED ELECTRICIANS GLOVE AND FUSE TONGS TO REPL
EE DISASSEMBLING CONVEYOR FRAMEWORK. HAND CAUGHT IN PINCH POINT.
AT 4:00 AM ON 8-1-98 THE SHAFT HOIST WENT OUT OF SERVICE DUE TO A MALFUNCTION OF THE SCR ON THE UPLIFT CONTROL BANK. THE OTHER MEANS OF ESCAPE WERE STILL AVAILABLE. THE SHAFT HOIST WAS CLEAR T O RUN AT 8:30AM ON 8-1-98
EE WAS PLACING SCREENS IN A SCREEN BOX. AS HE STEPPED INTO THE BOX HE BANGED HIS LEFT KNEE. HE DIDN'T NOTICE ANY PROBLEMS UNTIL 11:30AM AT THISPOINT HE WAS SENT TO DR
WHILE STANDING ON A STEEL FUELING PLATFORM, FUELING A LOADER, LIGHTENING STRUCK THE AREA & CONTACTED HIM.
AT 1905 THE SHAFT HOIST WENT OUT OF SERVICE DUE TO A MALFUNCTION OF THE SCR ON THE DC DRIVE PREVENTING USE OF THE HOIST. TWO OTHER MEANS OF ESCAPE WERE STILL AVAILABLE. THE SHAFT HOIST WAS CLE AR TO RUN AT 0900 ON 7-7.
BURNING EXPANDED METAL TO REPAIR GUARD RAIL ON SCREW COUPLING RUST POPPED OFF AND WENT UNDER SAFETY FLASSES HIT LEFT EYE.
EMPLOYEE WAS CLEANING THE DREDGE CONVEYOR AND WHILE TRAVERSING THE CATWALK HE LOST HIS FOOTING AND CAUGHT HIMSELF FROM FALLING ON THE CATWALK BY BRABBING THE HANDRAIL WIHT HIS LEFT ARM, AND IN JURED HIS LEFT SHOULDER.
EE WAS TRYING TO BREAK LARGE ROCKS WHICH PLUGGED THE #1 JAW CRUSHER. WHILE HE WAS STRIKING THE ROCK WITH A SLEDGE HAMMER A PIECE OF ROCK FLEW UP STRIKING HIS ARM. THE ROCK CUT HIS ARM REQUIRIN G 5 STITCHES.
AT 3:30AM THE ELECTRIC POWER CABLE WHICH FEED ELECTRIC TO THE SHAFT HOIST MALFUNCTIONED PREVENTING THE USE OF THE SHAFT HOIST. TWO OTHER MEANSOF ESCAPE WERE STILL AVAILABLE. THE SHAFT HOIST WAS CLEAR TO RUN AT 6:5OAM
EMPLOYEE WAS USING A CUTTING TORCH WHEN THE FLAME WENT OUT. THE EMPLOYEE THEN ATTEMPTED TO REMOVE THE TIP TO CLEAN THE TIP. HE DID NOT REALIZE THE FLAME WAS BURNING IN THE MIXING CHAMBER. HE A CCIDENTLY BUMPED THE OXYGEN SENDING THE FLAME OUT BURNING THROUGH HIS GLOVES CAUSING A SECOND DEGREE BURN.
EE WAS WALKING THROUGH THE SHOP AS HE STEPPED OVER A TRAILER TONGUE HE BANGED HIS SHIN. SEVERAL WEEKS LATER INFECTION SET IN CAUSING THE REMOVE OF DEAD TISSUE BY THE DR 6-16-98.
EE PULLED HIS BACK WHILE TUGGING ON DECK WINCH CABLE.
EMPLOYEE ALLEGES THAT HE BUMPED HIS KNEE AS HE WAS CRAWLING ACROSS A SCREW CONVEYOR. HE STATED THAT HE HIT A CLAMP THAT WAS HOLDING THE COVER ON THE SCREW HOUSING. HE HAD SURGERY ON 4.27.98 TO HAVE A BONE CHIP REMOVED. HE STATED ON A SEPARATE OCCASION THAT HE HURT IT SOMEWHERE ELSE. HIS FINAL STATEMENT WAS THAT HE HURT IT ON THE CLAMP
EE WAS DISMOUNTING A CAT HAUL TRUCK, WHEN IS FOOT SLIPPED OFF THE LADDER AND LANDED ON A ROCK TWISTING IS LEFT KNEE. THIS ACCIDENT OCCURRED IN THE 16-NORTH PARKING AREA.
EE WAS HAULING BLASTED ROCK TO THE CRUSHER WITH A CAT 631D HAUL TRUCK, WHEN AS HE WENT THRU A CROSSCUT THE RIGHT FRONT TIRE BLEW OUT CAUSING THE TRUCK TO HIT THE RIB ON THE RIGHT SIDE. HE SUBS TAINED 2 SMALL CUTS ON HIS HEAD & ONE REQUIRED 2 STITCHES.
EMPLOYEE WAS HAND SCLAING OUT OF AMADOR HIGH SCALER WHEN A LARGE SLAB 10' LONG 7' WIDE AND 24' WIDE THAT HE WAS TRYING TO SCALE OFF RIB CAME DOWN AND STRUCK OUT RIGGER OF THE MACHINE. THE MACH INE WENT DOWN ON THAT SIDE AND THE EMPLOYEE WAS THROWN OUT OF THE BASKET. HE WAS WEARING A FALL PROTECTION HARNESS AND WAS SECURED TO THE BASKET BY A 3' LANYARD.HE RECEIVED A FRACTURED SKULL
EE WAS HELPING HAND SCALE WHEN A LARGE SLAB 10' LONG, 7' WIDE AND 24' THICK, THE THEY WERE TRYING TO SCALE OFF RIB, CAME DOWN AND STRUCK THE OUTRIGGER OF THE MACHINE. THE MACHINE WENT DOWN AND EE WAS THROWN FROM BASKET. WEARING FALL PROTECTION. RECEIVED A LACERATION TO THE HEAD AND SOME BRUISING TO THE BODY. MACHINE POSITIONED PROPERLY.
EE WAS USING A PRY BAR TO ALIGN HOLES IN GRAVEL CHUTE THE BAR SLIPPED AND CAUSED HIM TO BANG HIS RIGHT ELBOW AGAINST I BEAM.
EE WAS WELDING A SKIRT IN JAW HOPPER. A ROCK, WEIGHING APPROX. 8#, ROLLED DOWN A SLOPE ABOUT 4' WHEN IT HIT SKIRT IT BOUNCED AND HIT HIM IN THE MIDDLE OF HIS BACK.
WENT FROM DREDGE TO DERICK TO PUMP DERICK OUT. STARTED TO HOOK BOAT TO DERICK BOAT, STARTED GOING BACK, STEPPED ON TOP OF MOTOR COVER TO SPEED BOAT UP. HE THEN TURNED AND STEPPED IN TO UNCOVE RED HOLE WHICH IS THERE TO CHECK OIL.
EE STEPPED OUT THE BACK DOOR OF PREP LAB ONTO THE GROUND. THERE WAS A BUILDUP OF LIME JUST OUTSIDE THE OVERHEAD TYPE DOOR OPENING. HE STEPPED ONTO THE LIME INSTEAD OF GOING AROUND. THE UNEVEN SURFACE OF THE LIME CAUSED THE RIGHT ANKLE TO TURN SIDEWAYS.
EE WAS HELPING REMOVE OLD KILM BRICKS & LIME OUT THE MANHOLE OPENING IN #3 KILN. WIND BLEW THE LIME BACK UP INTO THE MANHOLE OPENING & INTO HIS LEFT EYE. HE WAS WEARING SAFETY GLASSES. HE DID NOT GET SOMEONE TO REMOVE THE LIME & A PIECE REMAINED & BECAME STUCK REQUIRING A DOCTOR TO REMOVE. NO TIME LOST...REPPORTABLE ONLY.
EE WAS HIRED WITH A KNOW, DIAGNOSED HERNIA THAT WAS FOUND DURING PRE EMPLOYMENT PHYSICAL. ADA REGULATIONS PROHIBITS NOT HIRING FOR SUCH SITUATIONS. HE WAS PUT TO WORK WITHOUT ANY PROBLEMS WITH HIS HERNIA. HE SET A DATE TO GET IT FIXED & BEFORE HE WENT TO THE DOCTOR FOR SURGERY, THE HERNIA DROPPED. HE IS NOW OFF WORK. HE LEFT WORK ON 1/19/98. THIS IS A PREEXISTING CONDITION.

EE WAS USING AN AIR HOSE WITH A BLOW PIPE CONNECTED WITH ABOUT 80 PSI TO BLOW LIME BUILDUP FROM 3 KILN GRATES SO THE GRATES COULD BE REMOVED. SOME LIME PARTICLES BLEW BACK DURING THIS PROCESS CAUSING AN ABRASION THE RIGHT CORNEA.
EE WAS GREASING THE PLANT WHEN HE FELT A POP IN HIS KNEE. HE AHD NO PROBLEMS FOR AWHLE,BUT IT THEN STARTED TO BOTHER HIM AFTER A WEEK. HE WEN TO A DR BUT WAS RELEASED WITH NO RESTRICTIONS. IT STILL BOTHERED HIM, SO HE WAS SENT TO THE DR ON 1-12-98. THIS TIME HE WAS PLACED ON RESTRICTED UNTL A SPECIALIST COULD LOOK AT IT.
EE WAS DISMOUNTING THE CATERPILLAR 950F FRONT END LOADER. HE MISSED THE LOWER STEP. THIS CAUSED HIM TO FALL TO THE GROUND AND LAND ON HIS RIGHT HIP. THIS CAUSED PAIN IN HIS BACK AND HIP. THE S TEP WAS SLIGHTLY BENT.
EE WAS RIDING CARS DOWN. HE HAD LEFT 4 CARS BEHIND. THOSE 4 CARS STARTED ROLLING AND CAUGHT UP WITH THE ONES HE WAS RIDING DOWN. BUMPING THOSE CARS CAUSING HIM TO FALL FROM CAR HE WAS ON.
A ROOF FALL OCCURRED IN 38 CROSSCUT OFF F-NORTH BENCH IN THE 2 NORTH PANEL OF THE MINE. THE FALL WAS APROX. 100'L, 30'W. IT WAS ABOUT 4'T AT IT THICKEST POINT & FELL OUT IN A LOT OF PIECES. IT DID NOT FALL ABOVE THE ANCORAGE. IT DID HOWEVERBLOCK THE PASSAGEWAY OUT. THERE WAS NO VENTALATION CHANGE SINCE THE ROOM WAS APROX 60'T. NO ONE WAS INJURED & MSHA INSPECTORS, ON REG. INSP HELP
EMPLOYEE WAS WORKING ON CRUSHER, SLIPPED AND FELL. HE TRIED TO CATCH HIMSELF WITH ONE HAND AND PULLED SHOULDER.
