Employee at Mechanicsville Sand and Gravel Site sustained a left foot injury sprain/fracture. Injury happed while exiting the plant access point. Employee stepped down from the concrete step/edge on to uneven ground/material and immediately felt pain and discomfort. Employee went to the local ER, and an Xray was taken and confirmed a left foot fracture

Note: Third-party independent over-the-road (dump truck) hauler injury occurred on property associated with sand & gravel extraction activities. Independent hauler was cleaning tailgate off when their finger became pinned between tailgate and asphalt lip.

EE let go of a grinder and it hit EE's leg.

On the morning of 8/26/2023, employees arrived on site to run the plant. A small fire was discovered under the main feed hopper. Further investigation determined a larger fire had engulfed the entire plant overnight. Site security camera footage noted the fire broke out on the wet plant on 8/25/2023 at approximately 9:00 pm.

Injured miner was on a small personnel transport boat at the facility, and was refueling it with a portable gas can. Some gasoline was spilled during the process, and absorbed into the injured miner's pants material. This resulted in a minor chemical burn.

Contractor maintenance work (installing a sheet metal patch) was being performed on a bin under a vibrating screen at the facility wash plant. Welding slag blew through exterior wall of bin, igniting combustible rubber liners inside bin, causing a fire which impacted the bin, a vibrating screen, four conveyor belts, and adjacent infrastructure.

Customer Over-the-Road Truck Driver Injury (not a mining facility employee or contractor). The driver was dumping excess material from truck being purchased & hauled from Sussex facility. Used bare hand to brush excess material away, when finger was pinched between the "dog latch" & "gate closer" of the dump truck tailgate.

Employee was sliding forks in for a Genie GTH-844 Telehandler. After sliding the forks in, he let the front of the fork drop pinching his 5 digit of his right hand between the heel of the fork and the mast of the telehandler

Employee was climbing stairs to the sand and gravel plant when his foot slipped. He was holding onto the railings, so he did not fall, but reported that he twisted his right knee.

Employee mechanic was at our mine site doing PM to our excavator #18. He was underneath the machine, on the ground in a squatting position, installing the belly pan twisting from side to side and in the process twisted his left knee.

Employee was using John Deere 350 D Excavator to load dirt and he hit a hard spot in the ground. That caused the machine to raise up on the back side. When the material broke loose, the machine fell back to the ground hard.

Employee was getting out of his loader when his feet slipped on the mud on the step. He caught himself from falling off loader with his right arm and sprained his right shoulder.

EE alleges he was dismantling the shaker for replacement (catwalk).

EE alleges while he was cutting a piece of tubing with a cut-off wheel, the blade got pinched and the piece of steel moved. As it moved, he grabbed the steel and the blade went across his left thumbnail and side of thumb cutting it.

Grates used for guarding were stacked against a wall. He was trying to lift one that fell on the ground. It was partially under the others. As he lifted, the other started to fall forward. He caught them with his hands and thigh. They came to rest on his thighs and knee cap. No cuts, there was bruising and soreness to knee.

HURT LOWER BACK STEPPING OFF MACHINE. TRACKS OF THE EXCAVATOR WERE WET AND MUDDY.

EMPLOYEE STATES: "PULLING WASHOUT HOSE - INJURED LOWER BACK."

EMPLOYEE STATES:"SHOVELING SAND AND ROCK, FELT PULL IN LOWER BACK. NUMBNESS AND TINGLING IN FEET."

EE WAS PICKING UP ODDS & ENDS AROUND PLANT. HE TRIED TO PICK UP SOME 3" REINFORCED HOSE OUT OF THE MUD. THE HOSE WAS STUCK, SO HE HOOKED THE END OF THE HOSE THROUGH HOLE IN LOADER BUCKET. WHEN TENSIONS WAS PUT ON HOSE IT CAME LOOSE FROM LOADER BUCKET AND HIT EE IN THE FACE.
