EE working on primary pit crusher when somewhere in the line of equipment was struck by lightning which resulted in the current flowing through the wrench EE was using on the equipment. The electrical current exited EE's arm. EE was knocked onto the ground. Emergency services called and EE was taken to the ER.

EE was dismounting last rung of ladder on a front-end loader and sustained a muscle strain in their right thigh.

A employee sustained a bursting laceration to the middle finger of their left hand when it became pinched between two pieces of 3/4-inch steel.

While assisting in barge movement on the lower dock, an employee was struck by a 2" Three Strand twisted polypropylene rope snapback after the rope failed under tension. The force of the line propelled the employee's arm into their chest and mouth, resulting in a fractured tooth requiring a crown, along with contusions and to the lower arm and hand.

Employee was climbing down two steps off the ANFO rig, placed EE's left ankle on the ground (approximately 12 inches from bottom step) and it rolled when EE released the handrails on the step. Immediately reported to management and eventually found to be non-displaced fracture of the lateral Malleolus of the left fibula.

Sections of the back between 01SB-02SB and 10EB-11EB fell past the anchorage zone at some point in time and was discovered on 12/3/25. The entire area has been bermed off since 2020 so the area is considered inactive. All entrances to the entire bench cell have been barricaded against entry at this time.

The back in 4SB and 27WB fell above the anchorage and was discovered at approximately 9:30am on 11/3/25. Workplace exams from the intersection showed no hazards at approximately 4pm on 11/1/25, so the accident occurred sometime between then. The area was immediately barricaded off and it did not block travel routes or impeded escape. No miners or equipment were hurt/damaged.

Employee was reeling in a water hose on a water truck and sprained their shoulder.

While offloading material from a haul truck, a hydraulic cylinder broke, causing the bed to fall unexpectedly. The employee suffered a strained neck and was initially treated with First Aid. After five days, they sought medical attention, were diagnosed with an irritated nerve, and returned to work with restrictions.

Employee slipped and fell while descending a sloped gravel embankment after completing an inspection at the screen tower. The employee sustained a fractured left fibula.

Employee was removing screens (est. weight of approximately 70 lbs each) from the manlift when the screens began to slide. While attempting to prevent them from falling, the screens slid across the employee's wrist, causing a laceration that required 8 stitches. The employee was returned to work with medical restrictions

While dismounting from the driver's side of a water truck, the employee missed the bottom step and fell backward, landing on their backside. Initial injuries included a scrape to the arm and leg and back soreness (first aid). On 2/11, they reported neck stiffness, received medication, and were recommended physical therapy starting 2/21. Work status: No restrictions.

Employee was riding in a side by side utility vehicle that flipped over on its side and EE sustained an injury to their right knee. EE was wearing the seat belt at the time of the incident

Employee struck their left thumb with a hammer while tightening a bolt with a slug wrench.

After making repairs to the radial stacker, the employee sat on the tire and hopped down, approximately 6-8" from the tire to the ground. The employee felt pain in their left knee and was treated as a first aid. Sought medical attention on 10/8/2024 and was placed on restriction-no squatting, climbing, or crawling. Walking as tolerated. Avoid lifting >20lbs.

The employee was washing at the plant station with a 1.5" water hose when the employee's left forearm began to swell. Due to a pre-existing personal injury, they had recently been released to full duty.

An employee was using a chisel and hammer to remove a bearing. The chisel splintered and a metal fragment hit employee on the forearm causing a small laceration. A month later, employee started feeling discomfort when moving finger. An X-ray was done on 9/16/24 showing a foreign body embedded in their arm. Surgery will be required to remove it.

Employee was helping a customer hook up a water hose on the water truck and their shoulder began hurting. Employee did first aid. Employee was then seen at a clinic. F/U to clinic employee was given orders to start physical therapy on 7/30/24.

The injured employee was observing / assisting another employee with removing a stuck piece of material from a chute. The employee dropped the pry bar onto the injured employee's left hand.

Crew was called in for a trouble call and while checking secondary voltage on a Pad mount transformer an arc flash occurred. Employee received burns to the face and was transported to the hospital in stable condition.

Bolt broke on motor causing it to fall onto employees left arm.

Employee was spraying down the material build up on the rerun stacker belt. Water splashed up onto EE's face and something went into EE's eye.

While exiting the LD11 (980k) loader employee missed a step and fell to the ground no more than 4ft.

An employee was returning a troughing idler into position when it slipped off of the inside of the framework and pinched EE's left thumb in between the idler and the conveyor frame. The employee was taken to the clinic for evaluation where EE was placed on modified duty and given a prescription antibiotic.

Miners went underground at or around 0600 hrs, after driving through work areas, a roof fall was found at or around 0630 hrs. The area was coned off, management went and looked at area and called MSHA 1-800 number to report incident. No damage to personnel or to equipment, fall did not block travel routes or impede escape routes.

An employee was dismounting a front end loader when the employee mistook the second from bottom step, for the bottom step. This resulted in a larger than expected step. When the employee stepped down the force from the large step caused a pop in the left knee. After visiting the ER, it was discovered that the employee had broken the tibia.

Employee states their foot slipped off first rung of haul truck ladder while boarding and ankle got progressively worse throughout shift. The employee reported the injury by note to the supervisor who was out running for parts. The employee is on modified work duty and in an ankle brace.

Mechanic was repairing dust control air cylinder on rig 56, left hand slipped against filter box causing minor contusion/abrasion.

Employee stated they were using 3 points of contact while descending the engine access ladder when they lost footing and fell aprox. 3' to the ground injuring left leg, knee and ankle.

While performing maintenance on the self cleaning tail pulley bearing, a miner had the loose side of the pulley propped up with a pry bar, while another miner was sizing the bearing to the shaft. The bar slipped causing the tail pulley to drop injuring the right index finger of the miner requiring 16 stitches.

Around 13:00 a blaster was traveling main haul road and noticed, in a mined out room section of the back had fallen. They immediately reported the situation to the supervisor who went to assess scene with the Plant Manager and Leadman. The area was immediately bermed off and MSHA IR criteria was reviewed. A decision was made it was not immediately reportable. MSHA disagreed.

Employee was not using proper lifting techniques or body alignment while shoveling spillage out from under S12 take up.

Employee was connecting 16" pipe flange. While putting a nut on the bolt on the left side of flange, when the pipe shifted causing the middle finger on left hand to be pinched between the bolt and pipe.

The injured ee was climbing into loader at the beginning of the shift and felt a pull in the center of their back. The back got worse over the next few hours and the injury was called in. EE went to a clinic and given Rx muscle relaxer and put on modified duty. Per ee, nothing was different with how ee entered the loader. Acute muscle strain.

A 777 haul truck was getting loaded by a 992 loader when a rock hit the bed hard enough to make the driver bite their tongue and pop their back.

Two employees were working to shotcrete an area. The shotcrete nozzle handle became stuck and a wrench was used to turn it. The injured employee had a finger in the line of fire and left pinky was struck with the wrench causing a distal phalanx fracture.

Employee alleges to being bent over using water hose to clean beneath CV46 when felt sharp pain to lower back.

Employee was working on One Jaw Crusher when the left hand was pinched by the shims. This resulted in two broken fingers and required surgery.

Employee was leaning on spray bar changing out screens when employee felt a tightness in chest. Confirmed non-remarkable x-rays on 5/7/21. Bruising to the chest only - prescriptions given (NSAIDs and muscle relaxers)

(First aid) Employee misplaced their footing and fell while reaching above their head for bolts. 4/21/2021 Became medical due to starting prescription medication.

Employee delayed reporting the incident. Injury causation determination made known on 4/5/21. EE was performing hot work at an obscure angle. A piece of hot material fell into employee's ear. 4 days post incident, the employee went to a clinic and was diagnosed with a perforated ear drum.

Employee was operating a CAT 745C underground hauling material from a heading to the primary crusher. While reversing to be loaded, the HT shifted due to a change in ground conditions and the employee felt pain in EE's back. The injury has been under investigation.

Employee was operating front end loader with windows closed when employee felt something in eye. The employee was unable to flush the foreign body out and went to a specialist. The foreign body was a piece of metal and was able to be removed through irrigation (it was not embedded). Employee was given prescriptive eye drops and returned to work full duty.

Laying on right side extended right arm holding end of return roller, pulled roller up towards body and injured bicep.

While loading haul truck boulder caused rocking motion of truck, resulting in restricted duty injury to the haul truck driver.

Employee was walking in the plant and twisted the left knee.

Technician was removing the left rear cab bolt from a 982M wheel loader using a 3/4" ratchet wrench. Technician was pulling ratchet when they felt a pain in lower abdomen area. Following morning, technician was in pain and went to emergency room. Technician was diagnosed with a hernia.

Lifting tool box (40 pounds) - modified work days injury to back

Employee struck chin against handrail resulting in a laceration to the chin.

EE was changing out roller cans on CV-23. EE bent over to be able to get between the belt and the roller and was extended to the other side of the conveyor frame. When trying to grab the roller can it was still stuck. When it finally broke loose and the weight of the can transferred to the injured employee's arms, pain was felt in the groin area.
