Mostrando los 44Employee was traveling down decline from the portals to the shop area of the underground mine when a mechanical failure occurred in the rear end of the battery personnel carrier EE was operating, which caused excessive speed before overturning the battery powered personnel carrier.
Employee Stated that they were removing a rock off of a Shuttle Car and felt pain in shoulder.
On January 14th, 2026, a shuttle car operator was handling an energized electrical cable, whenever EE was exposed to arc flash, which resulted in burns to the hands and facial area, due to short circuiting of the energized cable.
Employee was checking a water pump and lost balance and put a hand down to catch themselves on the pump. The pump was sitting in a wire cage, and the wire cut their right pinky finger. The laceration required two stitches to close the wound.
Employee stated they dropped piece of belt rail on their foot right they took to er and checked and X-ray negate for any broke bones
A contract employee stated that EE dropped a single belt rail on EE's right foot. EE's foot was X-rayed and was negative for any broken bones.
On September 10th, 2025, a non-fatal accident occurred during the process of retreat mining on the 005-0 MMU. A rock struck a mobile roof support operator, which resulted in injuries to their lower leg areas.
The employee was taken off work by employee's physician on 10/28/2024. Employee was terminated per company policy on 01/13/2025.The company was presented a positive Chest Radiograph on 04/07/2025.
Employee was moving a piece of rock from a shuttle car. Employee was swinging the rock when their hand made contact with the canopy of the shuttle car causing the rock to cut their hand just below right hand. 5 stitches to close the wound.
Employee was installing rib bolts and placed hand on canopy while raising and crushed EE's left pinky between the canopy and a cable bolt head. This injury resulted in stitches and compound fracture of the tip of EE's finger.

Employee was pulling an MRS while another employee was pushing with a scoop. While pulling the MRS the scoop jack knifed and struck the rib causing employee to hit shoulder on the inside of the deck of the scoop. Original accident date was 09/30/2024 but employee continued to work until 01/29/2025.

Employee is scheduled to return to work on 11/08/2024.
Employee was attempting to remove a large piece of rock that had hung in the conveyor belt. Employee was attempting to lift the rock over the hand rail to let it fall to the ground when the employee lost grip on the rock and it pinned employee's hand against the hand rail.
Employee picked up a bucket of plaster and twisted and felt a sharp pain in the lower back. Pain got worse throughout the shift.
Employee stated that EE arrived on the section on the mantrip and that EE was packing a bucket of bolter bits and dust filters from the mantrip towards EE's bolt machine and stated EE fell causing EE to strike EE's wrist on the bucket of bits.
Employee was in the parking lot of the mine office and was standing observing a machine part being loaded into a scoop bucket. The employee started turning in circles and fell face down into the gravel and started seizing. The fall caused cuts to the face and head.
Bolt machine operator stated that had lost suction on the roof bolter. EE assumed something was clogging the pre-dump area and attempted to clear any obstructions. In the process of cleaning out the obstruction, two of EE's fingers were smashed causing a laceration to the fingers.
Attempting to loosen a hydraulic fitting and felt something pop in EE's shoulder.
A non injury roof fall has occurred in the 1 right bleeder panel off the 2 NW Submains. The fall was discovered while doing a weekly exam. The fall is approximately 30ft long 18ft wide and 4ft thick. The fall is located at spad#698 in the #5 entry.
Employee was unloading an electric motor and motor flipped from pallet and struck their leg.
Was adding oil to bolt machine, slipped on knees and arm hit straps, cutting EE's arm.

A diesel lo- trac that was located at Cross Cut #7 on the 6 left panel of the active 006 MMU caught fire. There was nobody operating or around the machine when it caught fire. There was no injuries associated with the incident and the fire was extinguished safely.
