On 11/20/2025 at Melvin Stone, employees were removing the bucket from EX1200 (22030002). When the final pin was pressed out, the stick settled and caught EE's left hand between the stick and bucket cylinder. First aid was given and EMS transported EE for treatment. No fractures reported.

EE was walking on the ground and had a fall from the same level. EE sustained a broken rib on right side. Restricted duty for 2-3 weeks.
Changing wear bars in head box. When the bolts were loosened the bar flipped hitting the thigh of the operator. The employee was given an Ibuprofen injection by the attending physician at the ER.
Attempting to reverse the direction of a conveyor motor.
Contractor drill rig caught fire. 2 extinguishers deployed. 911 called. Fire department extinguished.
Employee was operating a propane fueled forklift. The engine stopped. The employee thought the propane tank valve was closed. While attempting to unscrew the supply line, EE's left hand and fingers were burned by the cold propane release.
Employee had locked and tagged out the crusher to adjust the curtain. The rotor was on the "up-stroke" and the employee pushed the rotor over with their hand rather than using a board or steel bar and EE's glove got caught between the rotor and housing. This caused a cut to EE's right thumb requiring 6 stitches.
The employee slipped and fell off of a conveyor belt approximately 5 feet to a concrete floor.
Employee was helping to install a ladder that had a hinged section. When one of the bolts was removed the upper section hinged down knocking their hand into the side of the ladder and pinched their right thumb.
Three miners were cleaning up. One was operating a wheel loader and the other two were throwing scrap steel into the bucket to be taken to a dumpster. The loader operator thought one of the other miners signaled operator to lower the bucket. Operator then lowered the bucket onto one of the miners foot.
Driller was burned by a busted hyd hose when it shot fluid up onto the engine turbo burning his right check, right ear and top of his nose
Miner was attempting to move conveyor with chain slings and excavator. Chain slipped on conveyor causing it to swing. Miner attempted to stop swinging conveyor and was pinned between suspended conveyor and adjacent fixed conveyor.
Miner was dismounting ladder on 980 G FEL. Right knee buckled when miner placed right foot on ground. Miner was on flat clean surface.
Miner one was attempting to break miner two's fall as miner two was descending from a conveyor. Miner one was successful in breaking miner two's fall, and in the process, miner one received a torn bicep to the right arm.

Miner was preparing to cut the inner race of a bearing off of an overflow catch pan on a rip-rap machine, when pressure inside of catch pan forced adjacent bearing off of catch pan housing striking miner in left side causing contusion. Miner was taken to medical facility for observation.

WHILE USING A 4-LB. HAMMER TO STRAIGHTEN OUT THE ANGLE IRON ON A 70-FT. PORTABLE CONVEYOR, EE MISSED THE ANGLE AND STRUCK HIS LEFT THUMB. 5 STITCHES WERE REQUIRED TO CLOSE THE WOUND.
WORKER WAS WALKING DOWN A SMALL BANK TO PLATFORM TO START A DE-WATERING PUMP. AS HE WAS WALKING DOWN THE PATH HE STEPPED ON A LARGE ROCK THAT WAS PROTRUDING AND TRIPPED, FRACTURING HIS LOWER LEG.
EE WAS USING 3/8" DRIVE IMPACT TO REMOVE BOLTS FROM GAS TANKS STRAPS FOR REMOVAL OF TANK. BOLT HE WAS LOOSENING WITH IMPACT WAS SLIPPING. HE THEN NEEDED A WRENCH TO HOLD THE BACK SIDE. HE PLACED OTHER HAND ON BACK SIDE OF BOLT TO FEEL WHAT SIZE NUT IT WAS. AS HE PLACED HIS FINGERS ON THE NUT HE ACCIDENTALLY TURNED ON THE AIR GUN SPINNING NUT CUTTING FINGER.
TRYING TO CATCH SELF FROM FALLING FROM LADDER ON 988B LOADER. BENT RIGHT ARM AND RIGHT THUMB BACK AND HYPER EXTENDED THEM.
WHILE CHECKING ON PUMPS AROUND POND, STEPPED ON SOME SHOT ROCK AND CAUSED KNEE TO TWIST. HE COULD FEEL HIS KNEE CAP POP. WENT HOME AND HAD HIS WIFE TAKE HIM TO THE HOSPITAL.
DRIVING HAUL TRUCK THROUGH TUNNEL. ANOTHER TRUCK COMING FROM OPPOSITE DIRECTION. SECOND TRUCK FISHTAILED WHEN DRIVER HIT BRAKES, AND HIT LEFT CAB SIDE OF HAUL TRUCK.
EMPLOYEE'S EYE FELT IRRITATION IN RIGHT EYE. FLUSHED WITH WATER HIMSELF. STILL FELT LIKE SOMETHING IN EYE. WENT TO ER AT FAYETTE CO MEMORIAL HOSP. DR FOUND SMALL PIECE OF DIRT IN EYE. REMOV ED AND FLUSHED OUT EYE.
DRIVING HAUL TRUCK THRU TUNNEL WITH BED RAISED & CRASHED TRUCK INTO TUNNEL OVERHEAD.
TWO MEN WERE WORKING TO FREE A BIG ROCK STUCK IN THE JAW CRUSHER THROAT BY THROWING OTHER ROCKS BEHIND IT TO HOPEFULLY WEDGE AGAINST IT TO BREAK IT LOOSE. WHILE DOING SO, A ROCK ABOUT 75-100LB CAME LOOSE FROM THE ROCKS PILED IN THE FEEDER. IT SLID DOWN THE PILE AND CAUGHT ONE EMPLOYEE'S LEFT THUMB BETWEEN IT AND THE STEEL FEED THROAT OF THE CRUSHER, BREAKING HIS LEFT THUMB.
OPERATING HAUL TRUCK HE HIT A HOLE CAUSING TRUCKTO BOUNCE. THIS CAUSED A LOWER BACK PAIN AND NUMBNESS IN LEFT LEG. DOCTOR EXAM SHOWED A STRAINEDMUSCLE.
DOING MAINTENANCE ON DRY SCREEN, REPLACING THE RUBBER BELTING. TRIMMING THE SIDE, WHEN KNIFE SLIPPED, CUTTING LEFT INDEX FINGER. FINGER LACERATIONS REQUIRED 4 STITCHES.
IN AN ATTEMPT TO UNPLUG THE NORTH CHUTE A LARGE CHUNK FO ROCK FELL FROM A SIDE HITTING THE DI-N-BAR, CAUSING THE BAR TO SLIP FROM HANDS AND HIT EE IN THE JAW. SUFFERED A BROKEN TOOTH ON LOWER RT. JAWLINE.
REBUILDING SPOKANE CRUSHER, LINING UP THE TOP SHOE COVER PLATE. IT SLIPPED AND SMASHED HIS 3RD AND 4TH RINGERS ON HIS RIGHT HAND.

DIAGNOSIS WITH RIGHT LUMBOSACRAL STRAIN. DR. PUT HIM OF RESTRICTED WORK DUTIES OF NO CLIMBING, LIFTING, AND REPETITIVE USE OF PEDALS. EE TOOK PLANT OPERATOR POSITION BECAUSE IT MEETS HI
WALKING ON CATWALK, TRIPPED ON VALVE HANDLE, FALLING AND CATCHING LEFT FOREARM ON TOP ADGE OF SCREEN THAT WAS LEANING UP AGAINST THE HANDRAIL OFTHE WET SCREEN, LACERATION OF LEFT FOREARM.
