Employee was moving explosives from magazine to pickup and slipped on condensation from the humidity causing pain in lower back and leg. Did not go to Dr. until 7/18; doctor released him to work and recommended physical therapy. Physical therapist recommended he be taken off work; first missed day was 7/21. Will be off work until follow up appointment on 7/28.

Loader was backing away from pile. While he was preparing to turn and move forward, a pickup drove in front of him. Person in pickup was taken to hospital for back pain and released. Missed 3 days of work, released with no restrictions.

Removing a belly pan off of a truck, there was a block in place to prevent pan from falling to ground. When pan was released, landed on injured's hand. Went to the doctor, X-rays were negative for a break. Doc told him to ice it and keep it elevated, and come back the 12th. Oct 12th was released back to work with a 10lb lifting restriction for one week. Missed one day of work.
Employee was cutting boxes, the knife slipped and cut his right index finger. Required 3 stitches. No restrictions, no lost time.

EE was in quarry truck and reached for something, causing a pain in his back. The doctor has removed him from work until 11/21/08.
Working on fuel tank and got a metal flake in his eye. Flushed it out and kept working. Was still irritated the next day, went to dr to have object removed and wash released back to work with no restrictions.
Cleaning rock from belt, hit right thumb on guard and needed stitches.

Fell from rock bin. Shirt caught on guard, lost footing and fell to ground. Landed on back, hit head on ground. Went to dr to get checked out. Released to come back to work 9/24 with 10 lb restrictions.
There are hoods covering the padlocks on the caphouse. When he went to unlock it he was bitten by a bat. There were no lost days, he had to have rabies shots.
He is a loader operator and has had problems with his back before. On July 26, 2007 he had back pain from using levers in loader. Missed work July 27th for Dr appointment. Returned July 30th for an hour and a half, then went home because of pain. Has not been released to full duty yet.
Operating loader, extending back to reach levers and steering, and noticed a sharp pain in his lower back. He missed 11 days of work and was released to come back 5/8/07 with 30 lbs. Lifting restriction. He had elective non-work related surgery on 5/8/07 and did not return to work until 5/15/07. He was released from lifting restrictions on 5/18/07.

Employee was changing oil in a haul truck when he slipped and caught himself pulling and tearing his left shoulder.
The employee was changing oil in trucks at the mine site. He had opened the hood of the Volvo truck. He let the hood down easy and went on about his business. He returned to finish the truck and did not realize that the hood had latched and gave a strong tug to open it, straining his left shoulder.
Employee was helping move a 6" water line. While pulling on hose, he felt back pop.

EE WAS HAULING ROCK FROM QUARRY WHEN HE SAW A ROCK ON THE HAUL ROAD. EE GOT OUT OF HIS TRUCK TO REMOVE THE ROCK. WHILE LIFTING THE ROCK, EE SLIPPED AND TWISTED HIS BACK.

EMPLOYEE GOT A PIECE OF ROCK DUST IN HIS EYE WHILE DRIVING A HAUL TRUCK DUE TO HEAVY WIND. WAS GIVEN A PRESCRIPTION FOR EYE.

EE WAS WORKING ON CHANGING SCREENS IN CEDAR RAPIDS SCREEN UNIT #1815. EE WAS PULLING ON A BOLT WITH A WRENCH, IN A HARD TO REACH PLACE AND PULLED A MUSCLE IN HIS SHOULDER.
EMPLOYEE WAS LIFTING A DRILL STEEL ROD AND RECEIVED A HERNIA OF THE STOMACH MUSCLES. HE RETURNEDTO WORK THE SAME DAY BUT IS SCHEDULED FOR SURGERY ON 08/31/99.
ANOTHER DRIVER FAILED TO SET THE BRAKES ON HIS TRUCK AND IT BEGAN TO ROLL AWAY TOWARD THE HAMMERMILL. EE RAN AND JUMPED THROUGH THE WINDOW, BANGING HIS RIGHT KNEE ON THE DOOR BEFORE STOPPING T HE VEHICLE.
EE WAS MOVING 55 GAL OIL DRUM THAT WERE FILLED WITH WASTE OIL. HE WOULD LOWER DRUMS DOWN AND THEN ROLL THE DRUMS IN THE THE LOADER BUCKET. AFTER DRUMS WERE MOVED TO A DIFFERENT LOCATION, EE WO ULD THEN SET THEM BACK UP TO A VERTICLE POSITION. HIS BACK BECAME SORE AFTER THIS WORK AND WAS TOLD TO TAKE SOME TIME OFF.

EE WAS LOADING OUT SHOT ROCK IN THE PIT AT CRESCENT QUARRY. HE WAS CLEANING UP THE ROCK IN FRONT OF THE LEDGE. HE SUDDENLY HIT THE LIMESTONE MATERIAL THAT WAS NOT LOOSE CAUSING HIM TO STOP ABR UPTLY HE INDICATED HIS NECK WAS SORE SO WAS TAKEN TO HOSPITAL JUST TO BE SURE HE WAS OK. EVERYTHING CHECKED OUT OK AND HE RETURNED BACK TO WORK.
EE WAS CLEANING UP MUD ON FLOOR OF POT. RAIN HAD WASH MUD DOWN AROUND AREA WHERE A SMALL SUMP WAS USED. AS EE RAN LOADER ACROSS FLOOR. THE BUCKET TIP CAUGHT EDGE OF SUMP. THE STOPPED HE ALMOST IMMEDIATELY. IT WAS DETERMINED THAT HE HAD HIS RIGHT HAND ON THE HOIST CONTROL LEVER WHICH JARRED HIS RIGHT SHOULDER.
EE WAS BACKING HIS EUCLID 4UD ON 1 1/2 CLEAN STOCKPILE. THE STOCKPILE WAS SMALL IN SIZE & WAS NOT PACKED AT EDGES. AS EE WAS BACKING HE GOT OVER TO FAR INTO THE BERN BEFORE HE STOPPED. ON STOP PING THE TRUCK HIS R REAR WHEELS SANK DOWN CAUSING THE WEIGHT OF THE TRUCK TO SHIFT. THE EDGE GAVE WAY & HE WENT OVER EDGE. SEAT BELT WAS ON. EE BRUSIED HIS SHOULDER.

EMPLOYEE WAS ASKED TO MOVE UNIT #383 TO ANOTHER LOCATION WITHIN THE QUARRY, HE WAS ATTEMPTING TO CLIMB UP ON THE DOZER & START THE PONY MOTOR. CONDITIONS OUTSIDE WERE NOT FAVORABLE FOR OBTAINI NG THE PROPER FOOTING NEEDED TO SAFELY COMPLETE HIS TASK. ICE & SNOW HAD PREVIOUSLY FALLEN & EXTRA PRECAUTIONARY PROCEEDURES FOR THIS CONDITION SHOULD HAVE BEEN IMPLEMENTED. LOST FOOTING, FEL

PLANT 3 HAD MOVED TO CRESCENT QUARRY AND STARTED REMOVING SHALE. THE OTHER MEN STARTED ASSEMBLING THE PLANT. HE EVIDENTLY WAS PULLING ON SUPPORT BRACING THAT MUST BE INSTALLED WHEN UNFOLDING T HE FINAL DISCHARGE BELT ON THE SCREENING PLANT. HE IS NOT SURE WHEN THE ACCIDENT HAPPENED AND HIS BACK DID NOT START HURTING UNTIL 11-11-92. THE DOCTOR SAID HE HAD PULLED MUSCLES.

GETTING OUT OF LOADER SLIPPED AND TWISTED BACK
THE INJURED PERSON WAS OPERATING AN ENDLOADER, LOADING TRUCKS WITH SHOT ROCK AND SPOIL SHALE IN A STRIPPING OPERATION. THE CONSTANT JARRING AND RUNNING ON ROUGH TERRAIN CAUSED A STIFF NECK ADN SORE SHOULDER.

THE INJURED PERSON WAS HELPING TO MOUNT AN END LOADER TIRE.HE ATTEMPTED TO LIFT THE TIRE TO BE REPLACED AND INJURED HIS LOWER BACK.
THE INJURED PERSON WAS ON A DOZER CLEANING OFF THE TOP OF THE LEDGE FOR PREPARATION TO BEGIN DRILLING. WHILE STEPPING OFF THE DOZER TO HOOK-UP THE DRILL, HE SLIPPED ON THE DOZER TRACK WHICH WA S MUDDY. IN ATTEMPTING TO GAIN HIS BALANCE HE TWISTED HIS BACK.
HE WAS GETTING OUT OF ENDLOADER & SLIPPED WHILE GOING DOWN LADDER. WHEN HE TRIED TO CATCH HIMSELF HE TWISTED BACK.

THE INJURED MAN3WAS IN A MAINTENANCE BLDG NEAR THE CRUSHING PLANT HE THOUGHT HE HEARD A FOREIGN OBJECT RUN THROUGH THE PLANT HAMMERMILL. HE RAN OUT THE DOOR AND ABOUT MIDWAY TO THE PLANT HE FELL OVER A STA7K OF HAMMERMILL HAMMERS. HE LANDED ON THE PILE OF HAMMERS ON HIS LEFT SIDE CRACKING ONE RIB
