BURNING HIM.

EE WAS CHANGING OIL IN HIS LOADER. HE WAS USING A OIL DRAIN PAN TO CATCH THE OIL. THE OIL DRAIN PAN WAS A 55 GAL BARREL THAT HAD BEEN CUT DOWN APPROX 1/3 OF IT ORIGINAL HEIGHT. EE DRAINED THE OIL IN THE BARREL AND WAS ATTEMPTING TO PICK IT UP. AS HE DID HE BACK POPPED. EE SAID THAT HE DID NOT USE HIS LEGS TO DO THE LIFTING. THE WEIGHTOF THE PAN & OIL HAVE BEEN AROUND 50LBS.

AS EMP MOVED DRILL STEP ON WHICH HE WAS STANDING RAN OVER 8" ROCK CAUSING STEP TO BE THRUST UPWARD AND TH EN CAME DOWN SHARPLY. AS EMP CAME DOWN KNEE WAS SOMEWHAT TWISTED. THIS CAUSED A SPRAIN TO RIGHT KNEE. LEDGE WILL BE FREE OF ANY LOOSE OR FOREIGN MATERIAL BEFORE DRILLING.
EE AND A FELLOW EE WERE ADJUSTING DRIVE BELTS ON THE BREAKER BELT. EE WAS USING A TORCH TO HEAT UP THE ADJUSTER BOLTS. AFTER HEATING THE ADJUSTER BOLTS, THE TORCH WAS LAID ON THE BELT STILL LI T. EE STEPPED BACK CLOSE TO THE TORCH AND BURNEDHIS LEFT FOOT. HE HAS BEEN INSTRUCTED NOT TO SET TORCH DOWN WHILE LIT.

EE WAS PREPARING TO UNLOAD THE POWDER PICKUP. EE HELPS ANOTHER EE, THE DRILLER FOR PLANT #5 . AS EE WAS CARRYING A 55 LB CASE OF TOVEX HE SLIPPED IN SOME MUD ON THE LEDGE. EE SUFFERED LOWER BA CK PAIN. EE STARTED USING A BACK BELT WHEN HE RETURNED TO WORK. EE HAS BEEN TRAINED IN THE USE OF BACK BELTS.
THE CONDITIONS AT THE QUARRY WERE DAMP AND THE MATERIAL INTEH BINS HAVE TO BE OCCASIONALLY SCRAPED OUT. HE WAS BENT OVER THE RAILING WHILE SCRAPING THE WALLS OF THE BIN. THE SCRAPER IS APPROX 14' IN LENGTH. WHILE SCRAPING PULLED A MUSCLE IN HIS BACK. HE WAS JUST TRAINED IN PROPER LIFTING AND IN THE POSITION ONE SHOULDER NOT BE IN WHEN PRYING, LIFTING AND PULLING.
THE INJURED PERSON WAS STARTING A WATER PUMP WHEN THE DISCHARGBE HOSE RELEASED FROM THE PUMP. THE FORCE OF THE WATER CAUSED HIM TO FALL TO THE GROUND LANDING ON HIS RIGHT ARM.
