EE WAS HELPING MOVE A LARGE ROCK FROM THE THROAT OF A CEDAR RAPIDS IMPACT BREAKER. HE WAS USING A LONG PRY BAR TO TURN THE ROCK SO IT WOULD EVENTUALLY DROP DOWN AND GO IN THE BREAKER. EE WAS S TANDING ON THE CATWALK COMPLETELY TO THE SIDE OF THE THROAT AS THE ROCK WAS TURNED IT WENT INTO THE BREAKER, FRAGMENTED THEN A SMALL PIECE HIT THE CHAINS AND BOUNCED OFF AND HIT EE.

EE HAD FINISHED HIS WORK ON THE 15TH AND WAS DISMOUNTING HIS CAT. IT WAS MUDDY FROM ALL THE RAIN THAT WE HAD RECEIVED AT I80 QUARRY. INSTEAD OF CRAWLING OF THE CAT AND USING THE 3-POINT CONTAC T RULE. HE JUMPED OFF TRACKS TO THE GROUND WHEN WAS ABOUT 4 FT. EE DID NOT WANT TO GET HIS CLOTHES MUDDY SO HE JUMPED THIS WAS A DISGREGARD TO.

EE WAS STRIPPING DOZER MECHANIC WAS LINING UP A HHOLE WITH A 4' PRY BAR. AS HE WA APPLYING PRESSUR EON TEH BA WHEN IT SLIPPED AND HIT HIM ON IN THE EYE. SUSTAINED A SMALL CUT AND BLACK EYE. EE WAS WEARING SAFETY GLASSES BUT HAD TAKNE THE HOLE UP. THIS WAS A UNSAFE ACT. THIS IS AGAINST COMPANY POLICY.

EMP WAS HELPING MECHANIC REPAIR STINGER THAT FITS ON CAT. EMP WAS USING GRINDER ON STINGER IN PREPARATION FOR SOME WELDING. AS EMP WAS GRINDING, GRINDER SLIPPED OUT OF HAND AND STRUCK HIM ON L EG. GRINDER IS LARGE AND POWERFUL. IT WAS DISCOVERED THAT BETWEEN THE WIEHGT, THRUST AND LOOSE GRIP OF GRINDER, THAT THIS ACCIDENT WAS BOUND TO HAPPEN. EMP SUFFERED LACERATION APPROX 2" LONG.

EMPLOYEE HAD SECURED THE 463F SCRAPER AND WAS CHANGING THE WORN BITS. HIS BACK WAS TURNED TOWARDS THE LIP APRON. AS HE WAS REPLACING THE BITS A 4" CLUMP OF DIRT FELL FROM THE INSIDE OF THE APR ON. THE CLUMP OF DIRT HIT ORVAL IN THE BACK. THEDIRT HAD FALLEN APPROX. 5 FEET. EMPLOYEE USUALLY USES A SPADE TO SCRAPE AWAY ANY MATERIAL FROM THE INSIDE OF THE SCRAPER. HE DID NOT THIS TIME.

STILL WEDGED HE BENT OVER AND LOOKED CLOSELY AT THE ACTIVATOR. SUDDENLY THE ACTIVATOR CAME LOOSE STRIKING HIM IN THE LIP. SHOULD HAVE NOT PUT HIMSELF IN THE POSITION TO BE INJURED. NEEDED T

AT NOON BREAK HE STARTED TO GET DOWN OFF THE LOADER WHEN HE CAUGHT THE RING ON HIS FINGER ON T HE DOOR HING OF THE LOADER AS HE WAS STEPPING DOWN. RING FINGER ON THE LEFT HAND WAS AMPUTATED BELOW THE SECOND JOINT.
