The acid tails pipeline was plugged and EE was attempting to clear it. He attempted to remove one (1) of four (4) bolts from a six inch coupling, which was holding two sections of the line together, while the line was pressurized with compressed air. The line came apart striking EE on the head and knocking him to the ground.

Employee stepped on the PVC pipe when working on the leach pad and fell onto his right shoulder. He was given a full duty release at this time and sought medical treatment on a later date and was recommended for surgery for a rotator cuff tear 6/28/05. *Original on-line EDoc # 211279*

While walking on a leachpad liner, employee slipped and his feet went out from under him. He landed on the back of his head and back. This incident occured on 12/02/03. He was taken off the job on 05/21/04 for physical therapy and rehabilitation with the possibility of corrective surgery.

EE WAS OPERATING A CATERPILLAR 631 SERIES II SCRAPER HAULING EARTHEN MATERIAL WHEN HE REACHED FOR HIS CONTROL LEVER AND FELT THE TOP OF HIS HAND ""POP."" THE EE SUFFERED A STRAIN TO THE TOP OF H IS RIGHT HAND. EE GIVEN A WRIST BRACE AND RELEASED TO FULL DUTY.

EE WAS OPERATING A CATERPILLAR 631 SERIES II SCRAPER HAULING MATERIAL FROM THE ""BORROW AREA"" TO THE ""FILL AREA"" WHEN THE SEAT ""BOTTOMED OUT."" THE EE SUFFERED A STRAIN TO HIS LOWER BACK.
